Call your veterinarian or the nearest emergency veterinary hospital now, describe what happened and ask how they want you to proceed. Then take your puppy in promptly, moving them as gently and as little as possible. First aid can help during transport, but it does not replace veterinary care.
Call the vet before you move your puppy
Tell the clinic what happened, what injuries or symptoms you can see, and whether your puppy is bleeding, struggling to breathe, vomiting or unable to stand. Ask for any instructions specific to the situation and where to go. The American Veterinary Medical Association’s first-aid guidance recommends calling the veterinarian or emergency hospital and getting help from another person if possible. If your regular clinic is unavailable, call an emergency hospital.
Ask someone else to drive or help with the puppy if you can. This lets the driver focus on the road while another person watches the puppy and helps keep them secure. Do not delay departure to carry out complicated first aid; follow the clinic’s directions while arranging transport.
Choose a way to contain and move a small puppy
For a puppy small enough to fit comfortably, use a sturdy carrier or ventilated box that allows adequate airflow and keeps them from shifting around unnecessarily. A carrier with a removable top can make access easier: instead of pushing an injured puppy through a small door, remove the top and lower them in gently. A ventilated box is another option if it can safely contain the puppy.

Ask for help when available. Move the puppy slowly, supporting their body, and avoid twisting, pulling or repeatedly repositioning them. A blanket or coat can help you maneuver them gently into a carrier or other container. Keep the container secure during the drive, and avoid placing anything over the puppy’s face or restricting airflow.
If a spinal injury may be possible
If the puppy was hit by a vehicle, fell from a height or has a back or neck injury that makes spinal damage a concern, minimize movement and keep the head and neck aligned with the body. VCA’s general first-aid guidance for dogs describes using a board and securing the dog while keeping the head and neck immobilized. The AVMA also identifies a board, blanket or similar item as a possible stretcher, depending on the pet and situation. These are general dog and pet recommendations, not a puppy-specific handling protocol; ask the clinic for advice, and do not attempt a rigid support or tight restraint if doing so would require unsafe handling.

Protect yourself without restricting breathing
Pain, fear or disorientation can make even a normally friendly dog bite. Approach cautiously, keep your face away from the puppy’s mouth and have another person assist if possible. Use only restraint that does not interfere with breathing. Do not muzzle a puppy that is vomiting or having difficulty breathing; breathing trouble calls for immediate veterinary attention.
If your puppy is bleeding
For external bleeding, place clean, absorbent material such as dry gauze over the wound and apply direct pressure. Cover the wound for transport. Do not apply ointments, creams, disinfectants or other chemicals unless a veterinarian tells you to. If an object is embedded in the wound, do not pull it out; tell the veterinary team and avoid pressing on the object itself.

These steps are meant to support your puppy on the way to care, not to treat the injury at home. The AVMA states that first aid is not a substitute for veterinary care. Follow the hospital’s instructions and continue to the clinic promptly.

If doing so will not slow departure, take any relevant medication information and note when and how the injury happened. Tell the veterinary team what you observed, what first aid you provided and whether your puppy’s condition changed during the trip. Keep the puppy contained and minimize handling until the team can assess them.
