A very small puppy is not automatically sick, and being undersized is not a diagnosis. But poor or stalled growth can be a sign of a health problem, while inadequate nutrition can contribute to low blood sugar in toy-breed puppies. Other concerns associated with some small or toy breeds include portosystemic shunts, hydrocephalus, and tracheal collapse. Veterinary sources do not provide prevalence data for undersized toy-breed puppies, so none of these conditions can responsibly be called the most common in this group.
First distinguish small size from poor growth
A puppy may be naturally tiny because of its breed, family size, or individual development. The concern is a puppy that is not growing as expected, is losing condition, or has symptoms alongside its small size. A veterinarian can assess growth and body condition in context; size alone cannot identify a disease.
The conditions below are reasons to watch for specific signs and seek veterinary advice—not predictions that a particular puppy has a disorder.
Low blood sugar: a concern when nutrition or intake is inadequate
VCA lists inadequate nutrition in toy-breed puppies as one possible cause of hypoglycemia, or low blood glucose. Small size alone is not established as a cause. Signs may include unusual sleepiness or inactivity, weakness, muscle twitching, seizures, or convulsions. VCA’s guidance on hypoglycemia in dogs also notes that a low laboratory result needs interpretation: mishandling a blood sample can produce a falsely low reading, and a veterinarian may repeat testing if an animal appears healthy and has no compatible signs.

Marked lethargy, weakness, collapse, or seizures need prompt veterinary guidance; collapse or seizures warrant urgent assessment. Do not try to confirm or manage suspected hypoglycemia with a home diagnosis.
Poor growth and poor muscle development: ask about a liver shunt
A congenital portosystemic shunt is an abnormal blood vessel that lets blood bypass the liver. Because the liver does not receive and process blood normally, affected puppies can show stunted growth or poor muscle development. Other possible signs include disorientation, staring, circling, head pressing, seizures, vomiting, or diarrhea. Small-breed dogs usually have extrahepatic shunts, according to VCA’s overview of portosystemic shunts.

These signs are not specific to a shunt. A veterinarian may use blood and urine tests, bile-acid testing, ultrasound with Doppler, CT, or other imaging to investigate. Poor growth, especially with neurologic changes or recurring digestive signs, deserves veterinary assessment.
A soft spot is not the same as hydrocephalus
An open fontanelle—a soft area where skull bones have not fully closed—can persist as a genetic trait in some small breeds. By itself, it does not establish hydrocephalus; VCA notes that most persistent open fontanelles are not associated with the condition. VCA’s explanation of open fontanelles distinguishes this finding from a broader pattern of concerning signs.

A dome-shaped skull or large fontanelle together with neurologic signs is more concerning. Signs can include abnormal walking, behavior changes, circling, falling, head pressing, or vision loss. A veterinarian should evaluate these changes; VCA’s hydrocephalus guidance describes the condition and its possible signs.
Persistent harsh cough: consider tracheal collapse among other causes
Tracheal collapse is reported in several toy breeds, including Chihuahuas, Pomeranians, Shih Tzus, Lhasa Apsos, toy poodles, and Yorkshire terriers. A persistent, dry, harsh cough—sometimes described as a “goose honk”—is a common sign, but cough can have other causes, including heart disease. Breed and cough alone do not confirm the diagnosis.
Veterinary evaluation may involve radiographs, fluoroscopy, or endoscopy. For dogs diagnosed with tracheal collapse, VCA recommends a harness rather than a collar; this is advice for affected dogs, not a treatment or a requirement for every toy puppy. See VCA’s guide to tracheal collapse in dogs.

- Arrange prompt advice for poor or stalled growth, poor muscle development, persistent cough, unusual behavior, difficulty walking, circling, head pressing, weakness, or recurring vomiting or diarrhea.
- Seek urgent or emergency assessment for collapse, seizures, or difficulty breathing.
These signs can have more than one cause. A veterinarian can assess the puppy’s growth, examine them, and decide whether testing is needed.

Can a tiny puppy have low blood sugar?
Yes. Inadequate nutrition is one possible cause of hypoglycemia in toy-breed puppies, but the available guidance does not establish that small size alone causes it. Unusual lethargy, weakness, collapse, twitching, or seizures should prompt veterinary advice rather than a home diagnosis.
