Stopping rabies depends on two linked actions: vaccinating dogs to reduce transmission at its source and getting people prompt medical care after a possible exposure. The World Health Organization’s Zero by 30 goal is to end human deaths from dog-mediated rabies by 2030—not to eradicate every rabies virus reservoir worldwide.

Why does rabies prevention need to protect people and animals?

Rabies is a zoonotic viral disease that affects the nervous system. Once clinical symptoms appear, it is fatal; vaccination and timely post-exposure prophylaxis (PEP) can prevent the disease. Dogs are responsible for 99% of human rabies cases, so protecting people requires reducing infection in dogs as well as making human care available after exposure. WHO’s rabies fact sheet says tens of thousands of people die each year and 40% of deaths are among children under 15. Its Zero by 30 overview gives a specific estimate of 59,000 deaths annually and says 95% of cases occur in Africa and Asia.

Rabies prevention therefore has an animal-welfare purpose as well as a public-health one: fewer infected dogs means fewer animals exposed to the disease and fewer opportunities for transmission to people. WHO describes dog-mediated human rabies as eliminable by tackling infection in dogs.

Why is vaccinating dogs the main source-control measure?

Mass vaccination, including vaccination of puppies, protects dogs and interrupts transmission before people are exposed. WHO calls it the most cost-effective strategy for preventing human rabies. Its South-East Asia regional guidance says sustained dog-vaccination coverage of 70% has been shown to control canine rabies. That is a program benchmark, not a guarantee that the same coverage will produce the same result in every setting; local programs need to plan and monitor coverage over time. Culling free-roaming dogs is not an effective substitute. WHO explicitly says it does not control rabies. Vaccination addresses infection at its source, while culling does not provide the sustained immunity needed to interrupt transmission.

What should someone do after a possible exposure?

A bite or scratch from an animal that may have rabies needs prompt medical attention. Do not wait for symptoms or try to decide on your own whether treatment is necessary.

  1. Wash the wound immediately with soap and water for at least 15 minutes.
  2. Seek medical care promptly. A health professional can assess the exposure and determine the appropriate PEP.
  3. Follow the clinical plan. PEP can include a course of rabies vaccine and, when indicated, rabies immunoglobulin or monoclonal antibodies.

For people at elevated risk before any exposure, pre-exposure prophylaxis (PrEP) is a separate vaccination context. It does not replace prompt assessment and PEP after a suspected exposure. WHO’s information on vaccination against rabies covers the role of immunization; vaccine route and schedule are clinical and program decisions, not self-treatment choices.

How can health systems make prevention work in practice?

Dog vaccination and human PEP must be available together. A community can reduce the risk at its source, but people still need timely access to quality-assured care after exposure. WHO’s Zero by 30 plan connects mass dog vaccination and PEP with surveillance, community awareness, health-worker training, investment, and coordination between veterinary and human-health services. This is the practical meaning of a One Health approach: the teams responsible for animal health and human health share information and coordinate action.

How vaccines are delivered also affects access. WHO’s current fact sheet says intradermal administration uses less vaccine and fewer doses and can reduce costs by 60–80% without compromising safety or efficacy. This is a health-system delivery option; clinicians and vaccination programs determine whether it is appropriate and available.

How should progress toward Zero by 30 be understood?

Zero by 30 is specifically a target to end human deaths from dog-mediated rabies by 2030. It should not be described as a promise to eliminate every rabies virus in wildlife or other animal reservoirs by that date. WHO’s global strategic plan sets out the coordinated approach behind the target.

Country status also needs careful wording. In its fact sheet published 17 September 2026, WHO reported that Bhutan, Chile, and Mexico had been validated for elimination of dog-mediated human rabies as a public-health problem. Separately, WHO reported that 17 countries had self-declared free from infection with rabies virus to the World Organisation for Animal Health (WOAH), in line with the 2025 Terrestrial Code. These are distinct statuses, not interchangeable counts of countries free from all rabies risk. WHO rabies fact sheet
