New or repeated peeing outside the litter box can signal pain or illness, even when a cat otherwise seems well. If your cat is straining and passing little or no urine, seek emergency veterinary care immediately; a blockage can be life-threatening.
First, check for urinary warning signs
Contact a veterinarian promptly for repeated attempts to urinate, painful or difficult urination, crying in the box, blood in the urine, frequent small amounts, or persistent licking around the genitals. If your cat strains but produces little or no urine, especially while becoming more distressed, go to an emergency veterinary clinic now. Do not wait to see whether a litter-box change helps. A urethral obstruction is potentially life-threatening, and male cats are at higher risk because of their anatomy. Even without emergency signs, book a veterinary assessment for new or persistent accidents. Urinary inflammation, stones, kidney disease, diabetes, thyroid disease, age-related mobility problems, and other conditions can disrupt normal litter-box use. Cats may not look sick. Signs alone cannot establish the cause; a veterinarian may begin with an exam and urinalysis, then recommend bloodwork, imaging, or a urine culture depending on the findings. The pattern can help you describe what is happening to the veterinarian, but it does not replace a medical check. Spraying is usually a small amount of urine deposited on a vertical surface while the cat stands, raises its tail, and quivers it. A larger puddle on a horizontal carpet is more consistent with bladder-emptying outside the box. If you have several cats or have not seen the incident, observe discreetly or use a camera to identify the cat and posture.

Spraying may be related to territorial signaling or perceived threats, such as an unfamiliar outdoor cat, a new pet, household changes, or frustration. A carpet puddle may point toward box access, litter preference, discomfort, or a learned preference for that spot. More than one cause can be present. Make the litter box easier to use
Review the setup after arranging veterinary care. Cats can avoid a box that is dirty, difficult to enter, poorly placed, or associated with unpleasant litter. Check each factor below and change one at a time so you can tell what your cat accepts.

- Size and design: Consider a spacious, uncovered box. A low entry may help kittens, older cats, or cats with mobility limitations.
- Litter: Many cats prefer fine-textured, unscented litter. If switching brands or textures, transition gradually rather than abruptly.
- Location and access: Put boxes in quiet, easy-to-reach places. Ensure another pet cannot block or ambush the cat on the way in or out.
- Separate options: In a multi-cat home, provide accessible boxes in separate locations instead of clustering every box together.
These are common preferences, not rules for every cat. If your cat keeps choosing the carpet
A repeated choice of a soft horizontal surface may reflect a preference for that texture or location. Try a finer, softer litter, or temporarily place a box near the preferred spot. If your cat uses it reliably, move it gradually toward a more suitable location. If accidents resume, stop moving it and reassess the setup with your veterinarian. Avoid forcing a sudden change that leaves the cat without an acceptable option.

Reduce stress and prevent conflict
Look for what may have changed around the time the accidents began: a new animal, a household move, altered routines, unfamiliar outdoor cats, or tension between pets. Watch whether one cat guards the route to a box or startles another there. Add accessible choices and enrichment, and ask your veterinarian whether medical treatment or a behavior plan is appropriate. Do not punish, yell at, hit, or rub your cat’s face in urine; these responses do not solve the cause and can increase anxiety and house soiling. Clean the carpet without masking the odor
Remove urine promptly and use a carpet-appropriate product intended to neutralize odor, following its label and testing it first in an inconspicuous spot. Cornell emphasizes neutralizing rather than merely covering the smell, since lingering scent can encourage a cat to soil the same area again. It warns against cleaners containing ammonia or vinegar because their smell may resemble urine to a cat and they may irritate. Cornell does not endorse a particular cleaner or formula. Cleaning helps remove a cue; it does not address a medical, litter-box, or stress-related cause.

Keep a short record and follow up
Note which cat is involved, when and where accidents occur, whether the cat sprays or empties its bladder, any urinary signs, and what changes you make to the box. Share the record with your veterinarian. If the problem continues despite a medical assessment and careful setup changes, return for renewed medical and behavioral evaluation; overlapping causes are possible.
Cornell reports that spraying occurs in 10% of neutered male cats and 5% of neutered female cats; those figures concern spraying, not all carpet urination, and come from its page updated in January 2015. The same page says that in households with more than seven cats, it is likely one or more will spray; treat that as a source-specific observation, not a prediction for every home.
