Their nervous systems detect and process harmful stimuli, and pain can distress them. What has not been established is whether a cat’s private, conscious experience is exactly identical to a human’s. In practice, the useful question is whether your cat has changed from normal and needs veterinary assessment.
What it means to say cats feel pain
International Cat Care puts the basic point plainly: “Just like you and I, cats can experience pain associated with illness, injury or surgical procedures.” That statement supports a real welfare concern, not merely a reflex or an owner’s interpretation. A painful cat may alter its movement, appetite, posture, grooming, social behavior or use of the litter box.

“In the same way” needs a careful qualification. Cats and people share the biological machinery that detects tissue damage and produces protective responses, so it is reasonable to say that both species experience pain. Veterinary guidance does not, however, provide a direct measure of whether a cat’s subjective experience is identical to a person’s in intensity, quality or emotional meaning. A squint, a hiss or a crouched posture should not be translated into a precise human feeling. The reliable conclusion is narrower and more useful: a cat can be hurting, and the pain deserves assessment and treatment.
Why owners can miss feline pain
Cats cannot describe a symptom, rate it from zero to ten or tell you when it began. The American Association of Feline Practitioners (AAFP) therefore treats behavior change as a primary indicator. Signs may be subtle, and a cat may continue to perform parts of its normal routine despite discomfort. Your cat’s own baseline is the comparison that matters: a quiet cat that becomes unusually withdrawn has changed, just as a normally independent cat that suddenly seeks constant contact has changed.

A single sign is not a diagnosis. Pain can produce the behaviors below, but illness, fear, medication effects and other problems can also do so. Record the pattern and contact your veterinary practice when a new or persistent change appears.
Signs that may indicate pain
| What you may notice | Why it matters |
|---|---|
| Hiding or withdrawal | Spending more time under furniture, avoiding people or other pets, or becoming less interactive can signal discomfort. |
| Appetite change | Eating less, refusing food or showing less interest in treats is important, especially when it is a departure from the usual routine. |
| Reduced movement | Less activity, difficulty standing or walking, hesitation before jumping, or abandoning favorite perches can indicate pain. |
| Grooming change | A painful cat may groom less. An area may also receive unusual attention, although the AAFP handout emphasizes decreased grooming as a common clue. |
| Posture and face | A hunched posture, squinting or a guarded stance can accompany pain. These observations are most useful when they are new for your cat. |
| Touch sensitivity or vocalization | Flinching, resisting handling, growling, hissing or vocalizing when touched warrants professional advice rather than repeated home examination. |
| Elimination changes | Changes in urination or defecation, including altered litter-box behavior, can occur with pain or another medical problem. |
| Temperament shift | A substantial change in friendliness, irritability, tolerance or social behavior is meaningful even when no wound is visible. |
Look for clusters and trends rather than waiting for dramatic crying. A cat that is eating less, jumping less and hiding more has supplied several pieces of evidence. Note the date, what changed, how often it happens and what activities now seem difficult. A short video of walking, jumping or using the litter box can help a veterinarian see a behavior that does not occur in the clinic.
Acute and chronic pain are both possible
Acute pain
Acute pain begins with a recent event such as an injury, an operation or a sudden illness. It may be obvious, but it can also appear as a quiet withdrawal, reluctance to move or a facial change. Veterinary teams use structured acute-pain tools because observation at one moment can otherwise be inconsistent.

Chronic pain
Chronic pain can develop gradually, so the change may be easier to miss. Degenerative joint disease, stomatitis and disc disease are examples of conditions associated with ongoing pain. Instead of one dramatic symptom, you may notice that a cat no longer climbs, grooms thoroughly, plays, stretches or uses a high-sided litter box. Compare current habits with videos or memories from a few months earlier and mention slow changes even if your cat is still eating.
How veterinarians assess a cat’s pain
A veterinary assessment combines history, physical examination and observation. Tell the clinician what is different from normal, when it started, whether it is worsening and whether appetite, drinking, elimination, mobility or sleep have changed. The cause cannot be identified from a facial expression or a single score alone.
The Feline Grimace Scale
The Feline Grimace Scale is a structured tool for assessing acute pain. It scores five facial action units:

- ear position;
- orbital tightening around the eyes;
- muzzle tension;
- whisker position; and
- head position.
Each action unit receives a score, producing a standardized observation that can support a discussion with a veterinary professional. It is not a stand-alone diagnosis and does not reveal whether the underlying problem is dental, orthopedic, abdominal or something else. The scale can be useful for tracking a change, but owners should not use a score as permission to give medication.
When the Feline Grimace Scale smartphone app was announced in January 2022, Dr Paulo Steagall, Professor of Veterinary Anesthesia and Pain Management, described it as “a reliable tool which can be used in the home environment for the assessment of acute pain in cats.” Home observation can add valuable information, while the decision about cause and treatment remains clinical.
Other structured scales
World Small Animal Veterinary Association (WSAVA) pain materials also describe the UNESP-Botucatu and Glasgow composite feline scales. These tools combine behavioral or postural observations and apply their own scoring thresholds. They differ in what they emphasize—facial expression, posture, activity, attitude or response to touch—and in the pain contexts for which they were developed.

| Tool or approach | Main observations | How to use the result |
|---|---|---|
| Feline Grimace Scale | Five facial action units: ears, orbital tightening, muzzle, whiskers and head position. | Supports acute-pain assessment and communication; it does not identify the cause. |
| UNESP-Botucatu feline scale | Behavioral and postural items with a defined scoring threshold. | Used by trained teams as part of acute-pain assessment. |
| Glasgow composite feline scale | Composite behavioral and clinical observations with its own threshold. | Helps a veterinary team judge acute pain and response to treatment. |
| Routine history and examination | Baseline behavior, mobility, appetite, elimination, examination findings and owner videos. | Provides context, helps locate a problem and guides an individualized plan for acute or chronic pain. |
WSAVA’s updated 2022 global pain guidelines emphasize pain scales for both acute and chronic pain in companion animals. Their clinical protocols cover contexts ranging from feline sterilization procedures to degenerative joint disease. A 2021 WSAVA article reported that its first global pain guidelines had been downloaded in full almost 40,000 times from the Journal of Small Animal Practice website and viewed more than 6,000 times on the WSAVA website during the preceding year. Those are historical engagement figures, not measurements of how many cats are in pain or how severe their pain is.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What to do if you think your cat hurts
- Compare with the normal routine. Write down the specific change: less food, hiding, a missed jump, a hunched posture, a squint, altered litter-box use or a reaction to touch.
- Record timing and progression. Note when it began, whether it is constant or intermittent and what makes it better or worse. Take a brief video from a distance if movement is involved.
- Contact your veterinary practice. The AAFP advises contacting the practice when concerning behavior changes appear. Call even when signs are subtle if they persist or several occur together.
- Describe the full pattern. Include age, existing conditions, recent procedures or injuries, appetite, drinking, urination, defecation, grooming, mobility and temperament.
- Follow the examination and monitoring plan. The veterinarian may recommend an examination, tests, pain scoring, treatment, a recheck or home monitoring. Ask what improvement should look like and when to call back.
Do not repeatedly press, bend or manipulate a painful area to “test” it. A cat may protect itself by scratching or biting, and the response does not identify the cause. Transport and handling instructions from the veterinary team are safer than improvised examination.

Why human painkillers are unsafe to assume
Never infer that a human painkiller is suitable for a cat. A drug, dose or combination that is used for people can be inappropriate or dangerous for a cat, and a medication can also interact with an existing condition or treatment. Do not give anything unless a veterinarian specifically directs you, including a product left over from a previous illness.

The American Veterinary Medical Association describes pain management as an evidence-based decision tailored to the individual. Relevant factors include species, age, health status, degree of pain, tissue trauma and the procedure involved. That is why two cats with apparently similar behavior may receive different plans, and why a prescription for one cat should not be shared with another.
Questions that make a veterinary visit more useful
- Could the change indicate pain, or are other causes more likely?
- Which observations should I track at home, and how often?
- Would a pain scale or video help at the recheck?
- What signs mean the current plan is not working?
- How should food, water, litter-box access and activity be adjusted while my cat recovers?
- Which medicines, supplements or over-the-counter products must be avoided?
Bring a list of every product your cat receives and the time each dose was given. Clear information helps the clinician distinguish a new symptom from a medication effect and prevents accidental duplication.
What the evidence can—and cannot—tell us
Veterinary organizations agree on the practical points: cats experience pain; behavior changes are important evidence; pain may be acute or chronic; and structured assessment improves recognition and management. The available guidance does not establish that feline consciousness produces an experience exactly the same as a human’s. It also does not support estimating a cat’s pain intensity from one facial movement or assuming that a quiet cat is comfortable.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteThe most responsible comparison is therefore biological and behavioral, not philosophical. Treat a cat’s pain as real, use the cat’s normal behavior as your reference, and involve a veterinarian when the pattern changes. Early attention is especially valuable when reduced appetite, mobility difficulty, altered elimination, hunched posture or marked sensitivity occur, because those signs can accompany pain as well as other conditions requiring care.
