Before the first appointment, confirm the new clinic received and can read the records; keep copies of key documents in case the transfer is delayed or incomplete.
How to arrange the transfer
- Ask the new clinic what it needs. Confirm whether its staff will request the records from the previous clinic or whether you should do it. Ask how the clinic accepts records and whether it requires a signed release or other authorization.
- Contact the previous clinic. Give it your dog’s name and identifying details, your contact information, and the new clinic’s name and contact details. Request the complete available medical record for continuity of care, and ask whether you can receive a copy too.
- Authorize release if required. The clinics can explain what consent or other paperwork they need. Requirements for access and release vary by location; if you encounter a dispute, ask the local veterinary regulator what rules apply.
- Confirm delivery before the appointment. Check with the receiving clinic that the records arrived and are accessible. If the transfer is still pending or some records are missing, bring your own copies of important documents.
What records to request
Ask for the full available history rather than only a vaccine summary. A useful transfer can include:
- Visit notes, diagnoses, and details of procedures.
- Laboratory results and imaging reports or results.
- Current and past medications, including the name, dose, and frequency when available.
- Known allergies and adverse reactions.
- Vaccination history, including the rabies certificate when applicable.
- Your dog’s microchip number, if applicable.
The receiving veterinarian can use this information to understand prior care and avoid relying on an incomplete history. AVMA guidance also recommends keeping copies of vaccination records, important test results, medication details, and the microchip number (AVMA: Saving the Whole Family: Veterinary Records and Proof of Ownership).
Keep your own copies
Store vaccination documents, medication details, important test results, and microchip information somewhere secure and easy to find. A paper folder or pet medical-record book can help keep owner-held copies organized and updated; it complements, but does not replace, the clinic’s detailed record.

A microchip identifies your dog; it does not store medical history. Keep the chip number with your records and make sure your contact details in the registry are current (AVMA: Microchipping: FAQs for Pet Owners).
Rabies documents and international travel
For an ordinary transfer between clinics, ask the receiving veterinarian what rabies documentation it needs and include the certificate if you have it. CDC says owners can provide veterinary records, including a rabies vaccination certificate, to another clinic with a USDA-accredited veterinarian to establish a record there (CDC: Frequently Asked Questions on Dog Importations).

Do not assume that ordinary clinic-transfer paperwork is enough for travel or importation. Documentation can depend on where the dog was vaccinated and the travel circumstances. For dogs being imported into the United States, check the CDC’s current instructions and confirm requirements with the appropriate USDA-accredited veterinarian; CDC’s veterinarian forms page is dated April 28, 2026 (CDC: Documents for Veterinarians to Complete for Dogs Being Imported into the United States).

There is no single transfer process established for every location. For example, the Veterinary Practitioners Registration Board of Victoria says veterinary records belong to the practice or practitioner and that owner consent is needed for a transfer. That is Victoria-specific guidance, not a universal rule (Veterinary Practitioners Registration Board of Victoria: Guideline 07: Veterinary Medical Records).

Ask both clinics directly about timing, any fee, and the file format they use; these logistics are not established universally. A new veterinarian may be able to request records, but the clinic’s process and any authorization it needs still matter (VCA Animal Hospitals: The Importance of Sharing Medical Records).
