Inflammatory Bowel Disease (IBD) in Cats: Symptoms, Diagnosis, and Treatment

Feline IBD can cause recurring digestive signs, but diagnosis requires veterinary evaluation because other conditions can look similar. Learn about testing, biopsy decisions, diet trials, treatment, and monitoring.

Inflammatory bowel disease (IBD) in cats can cause recurring vomiting, diarrhea, appetite changes, or weight loss, but those signs alone cannot confirm IBD. A veterinarian assesses the cat, checks for other possible causes, and decides whether a food trial, imaging, or intestinal biopsy is appropriate. Treatment is tailored to the individual cat and may control signs, though flare-ups can recur.

What is IBD in cats?

Feline IBD describes chronic inflammation in the gastrointestinal tract associated with persistent or recurring digestive signs. The location of the inflammation can affect what a cat experiences: stomach or upper small-intestinal disease may be associated with vomiting, while inflammation in the colon more often causes diarrhea, sometimes with blood. Appetite changes and weight loss may also occur.

These signs have other possible causes, including parasites, infection, metabolic disease, hyperthyroidism, food-responsive disorders, and cancer. IBD is therefore a diagnosis reached through veterinary evaluation and consideration of plausible alternatives, not a conclusion that can be drawn from symptoms alone. Cornell University’s Feline Health Center describes this diagnostic approach in its April 2018 IBD guidance.

What symptoms can IBD cause?

  • Recurring vomiting
  • Diarrhea, sometimes with blood
  • Changes in appetite
  • Weight loss

The pattern depends partly on which part of the gastrointestinal tract is affected. Because other conditions can produce similar signs, tell the veterinarian when the symptoms began, how often they occur, and whether appetite or weight has changed.

How do veterinarians diagnose IBD?

There is no single home test or routine laboratory result that establishes IBD. The veterinarian builds a picture from the cat’s history and examination, tests that look for other causes, and—when warranted—imaging or tissue sampling. The sequence and tests vary with the cat’s condition and findings.

  1. Clinical assessment: The veterinarian examines the cat and reviews the digestive signs, appetite, and weight history.
  2. Tests for other causes and complications: Depending on the case, the workup may include blood and fecal tests, urinalysis, and measurement of cobalamin (vitamin B12) and folate. These nutrient levels can be relevant because intestinal disease may interfere with absorption.
  3. Imaging: Abdominal ultrasound may help assess the gastrointestinal tract and inform whether further investigation is appropriate.
  4. Consider a controlled food trial: A veterinarian may recommend a diet trial when a food-responsive disorder is a plausible explanation. The trial needs to be conducted as directed to be informative.
  5. Decide whether tissue sampling is needed: Biopsy allows microscopic assessment of tissue and can help characterize inflammation and distinguish competing diagnoses. It is not automatically appropriate for every cat with chronic gastrointestinal signs.

Cornell describes biopsy as necessary for a definitive histologic diagnosis of feline IBD. ACVIM guidance adds a practical distinction: biopsy is not warranted in every animal with chronic gastrointestinal disease, so its timing depends on the clinical picture and the results of earlier evaluation.

When might a veterinarian consider biopsy earlier?

ACVIM guidance identifies severe weight loss, poor body condition, severe anorexia, low albumin, substantial infiltrative findings on ultrasound, or a combination of these factors as reasons a veterinarian may consider earlier endoscopy. These findings do not, by themselves, establish IBD or lymphoma.

Endoscopic and surgical biopsy

Endoscopy and surgical biopsy are different ways to obtain tissue, and both require anesthesia. The choice depends on the suspected distribution of disease, whether other organs need assessment or sampling, the cat’s clinical status, and the veterinarian’s judgment. A normal-looking intestinal surface does not rule out microscopic disease. The ACVIM GI Standardization Group’s 2010 guidelines address biopsy and histopathologic evaluation.

How is IBD distinguished from intestinal lymphoma?

Low-grade intestinal lymphoma can resemble inflammatory enteropathy in cats. The 2023 ACVIM consensus statement emphasizes that laboratory tests alone cannot reliably distinguish the two. Tissue interpretation may be needed, sometimes alongside additional diagnostic methods, and results must be considered in the context of the cat’s clinical findings. A biopsy can therefore help answer a question that routine bloodwork cannot settle by itself.

How is IBD treated?

Treatment is individualized and may change as the cat’s response and monitoring results become clearer. A plan can include parasite treatment when indicated, a veterinarian-directed diet trial, medication to reduce inflammation or modulate immune activity, and supplementation when testing identifies a deficiency. There is no single regimen established as best for every cat.

Diet trials

A veterinarian may select a hydrolyzed-protein diet or a novel-protein diet based on the cat’s food history. “Novel” means a protein the individual cat has not previously eaten; a diet is not novel for every cat simply because it is marketed that way. Food choice also needs to account for nutritional suitability and whether the cat will eat it.

For a controlled trial, follow the veterinarian’s instructions consistently. Other foods, treats, table scraps, and flavored medications can undermine the result. Ask the veterinarian how to manage necessary medications and feeding in a multi-pet household. The evidence supports these diet categories as options for a supervised trial; it does not establish a particular brand or food as a cure.

Medication and supplements

Medication decisions belong with the veterinarian. Cornell lists metronidazole and corticosteroids among therapies that may be considered. The Merck Veterinary Manual identifies prednisolone with dietary management as the treatment of choice for feline chronic colitis due to IBD; that guidance is specific to chronic colitis, not a universal prescription for every form of IBD. Merck also cautions that sulfasalazine is not recommended for cats because salicylates have prolonged half-lives in this species.

Cobalamin and folate may be measured because intestinal disease can affect absorption. Supplementation is considered when testing identifies a deficiency; symptoms alone do not show that a cat needs either supplement. Do not start, stop, or change a medication or supplement without veterinary direction.

Can IBD in cats be cured?

IBD can often be controlled so a cat remains comfortable, but signs may come and go. Cornell does not give a universal cure rate or life-expectancy figure. If signs recur, contact the veterinarian for reassessment rather than changing the food or medication plan on your own.
