What does kidney disease mean in dogs?

Kidney disease means the kidneys have lost some ability to filter waste and maintain the body’s internal balance. Chronic kidney disease can develop over months or years before it becomes obvious to an owner.

CKD involves prolonged loss of functional kidney tissue. It is often progressive and generally irreversible once clinically apparent, although appropriate care can help manage complications and support quality of life. One abnormal test result or a temporary illness does not, by itself, establish CKD.

AKI is a sudden decline in kidney function. Causes can include toxins, infection, poor blood supply to the kidneys, or prolonged urinary obstruction. In some dogs, the initiating cause is not identified.

AKI can follow sudden insults such as ethylene glycol exposure, ingestion of grapes or raisins, some drugs, infection, poor blood supply, or prolonged urinary obstruction. Any kidney injury that persists or progresses can contribute to chronic disease.

Early CKD may cause no obvious signs. Increased thirst and urination can be early clues, while more advanced disease may cause poor appetite, weight loss, lethargy, dehydration, vomiting, diarrhea, or mouth sores. These signs are not specific to kidney disease, so they need veterinary assessment.

Do not try to treat suspected kidney disease at home with fluids, medicines, supplements, or dietary restrictions unless a veterinarian directs you. Suspected antifreeze or grape or raisin exposure warrants immediate veterinary attention, even if the dog initially looks well.

How are the stages of kidney disease in dogs determined?

IRIS staging is used after CKD has been diagnosed. The veterinarian assesses creatinine and SDMA on at least two occasions in a stable, hydrated patient, then considers urine protein and systolic blood pressure as substages. A single result from a sick or dehydrated dog is not enough to stage CKD.

What should a dog with kidney disease eat?

A veterinarian may recommend a special renal diet as part of CKD care. Whether a diet change is appropriate depends on the dog’s diagnosis and test results. How long can a dog live with kidney disease?

There is no single life-expectancy estimate for every dog with kidney disease. Prognosis depends on whether the condition is acute or chronic, its cause, stage, complications, and response to treatment.

Merck Veterinary Manual notes that animals with CKD may survive for long periods with perhaps 5%–8% of functional renal tissue when they receive appropriate therapy. This is a contextual observation, not a prediction for an individual dog. Some AKI cases improve with prompt treatment, while advanced CKD may require ongoing care.
