Laparoscopic vs. Traditional Spay: Which Is Right for Your Dog?

Laparoscopic spay may mean smaller incisions and less postoperative pain, but the right choice depends on the surgeon, your dog’s health and which organs will be removed.

For a healthy dog having an elective spay, laparoscopy can offer smaller incisions and may reduce postoperative pain and wound problems. It is not automatically the better choice: results depend on the surgeon’s experience, your dog’s health and the exact operation. Ask two separate questions: will the surgeon use an open or laparoscopic approach, and will they remove the ovaries alone or the ovaries and uterus?

What “laparoscopic” and “traditional” mean

These terms describe how the surgeon reaches the reproductive organs, not necessarily which organs are removed. In an open, or traditional, procedure, the surgeon operates through an abdominal incision. In laparoscopy, a camera and instruments pass through small openings, and carbon dioxide is used to expand the abdomen so the surgeon can see and work.

The tissue removed is a separate decision. An ovariectomy removes the ovaries; an ovariohysterectomy removes the ovaries and uterus. Laparoscopic sterilization commonly means ovariectomy, though laparoscopic-assisted ovariohysterectomy is also possible. A dog can be sterilized without having her uterus removed.

The 2024 WSAVA reproductive health guidelines say ovariectomy is sufficient for sterilization in most animals and that uterine removal is not necessary unless uterine pathology or another clinical consideration makes it appropriate. The guidelines note that pyometra does not occur without remnant ovarian tissue or an exogenous source of progesterone. The American College of Veterinary Surgeons (ACVS) says available literature shows no long-term outcome difference between ovariectomy and ovariohysterectomy; they are considered essentially equivalent when appropriately performed.

Potential advantages of laparoscopic spay

Smaller incisions

ACVS describes laparoscopic surgery as typically using two or three incisions, usually less than one inch long. Smaller access points may be appealing, but incision size alone does not determine the right operation or guarantee an uncomplicated recovery.

Possibly less postoperative pain

The WSAVA guidelines characterize laparoscopy as less painful than laparotomy. A 2017 Veterinary Evidence review concluded that available studies suggest laparoscopic ovariectomy can reduce postoperative pain and loss of activity.

One small randomized prospective trial published in 2005 studied 20 healthy dogs weighing more than 10 kg. Based on pain scores, 9 of 10 dogs that had open ovariohysterectomy needed additional pain medication, compared with none of the 10 dogs that had laparoscopic-assisted surgery. The pain assessor was not blinded, and the small trial cannot establish the same result for every dog, surgeon or technique. The trial report.

Fewer wound problems in some studies

A 2019 retrospective study compared open and laparoscopic ovariectomy cases. Under the study’s complication-grading system, complications were recorded in 46 of 106 open cases (44%) and 31 of 154 laparoscopic cases (20%). Wound-healing complications affected 28.3% of the open group and 11% of the laparoscopic group; most were minor. Because the study was not randomized, its figures are not universal risk estimates and do not prove that the approach alone caused the difference. The authors called for a randomized trial to confirm the finding. Charlesworth and Sanchez, 2019.

Visualization and suitability for larger dogs

WSAVA notes that laparoscopy provides improved visualization and safety, particularly in large-breed dogs, and says its benefits are greatest in larger patients. The procedure can be performed in dogs of different sizes; size is a reason to discuss the option, not a rule that determines it.

Trade-offs and limits to consider

  • Specialist equipment and training: Laparoscopy requires appropriate equipment and a surgeon trained in the technique. Availability varies by clinic. Methods also differ: an operation may use one, two or more ports, or be laparoscopic-assisted rather than completed entirely inside the abdomen.
  • It may take longer: In a small randomized trial of dogs being treated for pyometra, median surgery time was 37 minutes for laparoscopic-assisted surgery and 23 minutes for open surgery. This was treatment of uterine disease, not routine elective spaying; the laparoscopic group also required an assistant and specialized equipment. The result should not be used to predict the time for an elective spay.
  • Carbon dioxide must be managed: The abdomen is distended with carbon dioxide during laparoscopy. WSAVA cautions that residual gas can cause discomfort and should be completely evacuated before closure.
  • Comparisons do not always isolate the approach: A 2026 retrospective UK cohort included 519 dogs: 213 had laparoscopic ovariectomy and 306 had open ovariohysterectomy. The laparoscopic group had fewer reported wound signs and fewer postoperative complications requiring veterinary attention, with no significant difference in operation duration. But the groups differed in both surgical access and tissue removed, and the data came from practice records; the results cannot show that laparoscopy alone caused the differences. In that cohort, wound redness or swelling was reported in 16.3% of laparoscopic ovariectomy cases, 36.6% of all open ovariohysterectomy cases and 43.1% of open cases at the comparison practice. Complications requiring veterinary attention were reported in 12.9%, 16.3% and 24.0%, respectively. These are findings from that study, not general rates for all clinics or dogs. Overall, evidence supports smaller incisions and suggests potential benefits for pain and wound outcomes, but it varies by procedure, patient group and study design. Neither approach should be described as pain-free or risk-free, and a fixed recovery advantage cannot be promised.

How to decide with your veterinarian

Laparoscopy is particularly worth discussing for a larger dog if an experienced laparoscopic surgeon is available. Open surgery may be the appropriate choice when it is the surgeon’s established technique, laparoscopy is unavailable, or uterine disease makes removal of the uterus advisable. WSAVA recommends an individualized decision and says ovariohysterectomy may be more appropriate when uterine pathology is present.

Bring these questions to the consultation:

  1. Is the proposed operation an ovariectomy or an ovariohysterectomy, and why?
  2. Will it be open, laparoscopic or laparoscopic-assisted?
  3. How much experience do you have with this approach in dogs like mine?
  4. Does my dog have a health condition or uterine finding that changes which tissue should be removed?
  5. What pain-control plan and activity restrictions do you recommend?
  6. Which symptoms should prompt a call or recheck, and what does the quoted fee include?
  7. What is the total cost at this clinic, and what follow-up visits are planned?

There is no dependable universal price comparison between the procedures in the available sources. Ask the clinic for an itemized, case-specific estimate and compare what each quote includes.

Recovery and aftercare

Follow the operating clinic’s discharge instructions for your dog. ACVS recommends house rest for two weeks, with no running, jumping or rough play. Pain medication is often prescribed for several days. An Elizabethan collar may be needed to prevent licking; use one if the clinic recommends it and follow its guidance on fit and wear.
