Megacolon is persistent enlargement of the colon combined with severely reduced movement of stool. The colon holds feces, absorbs more water from it, and can leave a cat chronically constipated or completely unable to defecate (obstipated). It is more serious than an occasional difficult bowel movement and needs veterinary evaluation.
Some cats have idiopathic disease, meaning no cause is found after testing. Others have a treatable obstruction, nerve problem, metabolic disorder, dehydration, medication effect, or a history of pelvic injury. Treatment may range from rehydration and veterinary evacuation of impacted feces to long-term medication or subtotal colectomy. The best plan depends on why the colon stopped working, how advanced the changes are, and whether medical treatment still controls the problem.

What causes megacolon in cats?
Normally, the colon stores feces and moves it toward the rectum. In megacolon, it remains abnormally dilated and loses effective contractions. Feces stay in place, become progressively drier as water is absorbed, and stretch the colon further. Long-standing constipation can eventually damage colonic muscle and nerves, creating a self-perpetuating cycle.
Idiopathic megacolon is diagnosed when evaluation finds no specific cause. Cornell Feline Health Center reports that idiopathic megacolon accounts for more than half of obstipation cases discussed in its clinical article; that figure describes the cases in that context, not the prevalence of megacolon in all cats.

Mechanical narrowing or obstruction
A narrowed outlet can prevent otherwise normal stool from passing. Causes include a poorly healed pelvic fracture, congenital pelvic abnormalities, rectal or anal masses, strictures, and hernias. A mass or narrowing may require treatment directed at the lesion rather than medication aimed only at colon motility.
Nerve, spinal, and pelvic disease
Damage to the spinal cord, pelvic nerves, or the nerves supplying the colon can impair propulsion. Trauma and spinal or pelvic changes may be found on imaging or during examination.
Metabolic, hydration, and medication factors
Dehydration makes feces harder and can worsen an existing motility problem. Electrolyte and other metabolic abnormalities may contribute. Drugs that slow bowel function can also play a role. Your veterinarian will review every prescription, over-the-counter product, and supplement rather than assuming the problem is idiopathic.

Why the distinction matters
An outlet obstruction and idiopathic colonic inertia can look similar at home but have different treatment priorities. The veterinarian must look for reversible causes before labeling the condition idiopathic.

What are the symptoms of megacolon?
Signs vary with how much feces is retained and how ill the cat has become. Watch for a change from your cat’s usual pattern rather than relying on one universal number of bowel movements; outdoor cats may defecate unseen, and some cats hide illness.
- Repeated straining in the litter box or repeated attempts with little result
- Infrequent bowel movements or no observed stool
- Small, hard, dry pieces of feces
- Abdominal discomfort, restlessness, or hiding
- Reduced appetite
- Lethargy or reluctance to move
- Vomiting, especially when constipation is severe
Straining can sometimes be mistaken for difficulty urinating. A cat making frequent trips to the box, crying, or producing little urine may have a urinary emergency, so tell the clinic exactly what you have observed.
When to call a veterinarian
Contact a veterinarian promptly if straining keeps recurring, little or no stool is being produced, or your cat is vomiting, not eating, painful, or lethargic. Do not wait for a home “cutoff” based on hours or days; severity and the individual cat’s history matter more than a single schedule.

The American College of Veterinary Surgeons states: “Diagnosis of megacolon is based on history and physical exam, and is confirmed with radiographs (x-rays) of the abdomen.” Diagnosis usually proceeds in stages.

- History. The veterinarian asks about stool frequency and appearance, previous constipation, pelvic trauma, diet, water intake, medications, appetite, vomiting, and weight change.
- Physical examination. Abdominal palpation may reveal a distended colon packed with feces. A rectal examination can help identify retained stool, pelvic narrowing, a mass, or a hernia when it is safe and practical.
- Abdominal radiographs. X-rays show whether the colon is persistently enlarged and may reveal pelvic fractures, spinal changes, or an obstructing mass.
- Laboratory testing. Blood tests can identify dehydration, electrolyte disturbances, kidney disease, or other metabolic problems that could worsen constipation or affect anesthesia.
- Additional tests when needed. Ultrasound, contrast studies, or colonoscopy may be recommended if the cause remains uncertain or a mass, stricture, or other lesion needs closer assessment.
Idiopathic megacolon is therefore a diagnosis of exclusion. A large colon on one X-ray does not, by itself, establish that no underlying cause exists.
How is megacolon treated?
Treatment is individualized. A cat that is dehydrated and impacted needs stabilization and evacuation; a cat with an obstruction may need a procedure to address that obstruction; and a cat with irreversible loss of motility may need surgery.
Initial stabilization and fecal evacuation
The clinic may correct dehydration with fluids, soften stool, and remove retained feces. Severe impaction or obstipation may require enemas and manual removal under sedation or anesthesia. The veterinarian may repeat evacuation until the colon is adequately cleared and the cat is stable.
Never give an enema at home unless your veterinarian specifically instructs you. The American College of Veterinary Surgeons warns: “NEVER attempt to give a cat an enema at home unless instructed by your veterinarian, and NEVER use an over-the-counter Fleet phosphate enema (infant enema), which is toxic to cats.”

Medication and diet after evacuation
Veterinarians may prescribe laxatives or stool softeners and, when useful, a prokinetic drug such as cisapride to encourage colonic movement. Doses depend on the cat’s hydration, kidney function, obstruction risk, and other medicines; do not copy a dose from a website or another cat’s prescription.
Merck Veterinary Manual notes that cisapride is not conventionally manufactured and must be compounded in the United States. Cats with long-standing obstipation and established megacolon are unlikely to improve with cisapride alone, so a poor response does not necessarily mean the dose should simply be increased.
Diet must match the diagnosis and the colon’s remaining motility:
| Clinical situation | Diet implication |
|---|---|
| Some uncomplicated or mild constipation cases | Soluble or insoluble fiber may help when selected by a veterinarian; Merck discusses high-fiber diets in some mild cases. |
| Idiopathic megacolon with poor propulsion | The American College of Veterinary Surgeons favors a low-residue diet and cautions that high-fiber bulking agents can worsen signs. |
| Dehydration or recurrent hard stool | A plan that improves water intake and stool moisture may be used, but the food and moisture target are patient-specific. |
Because these recommendations can point in opposite directions, do not add bran, pumpkin, or another fiber product without asking the treating veterinarian. Merck recommends adequate hydration and ready access to water. A fountain may make water available to a cat that prefers moving water, but it is optional and is not a treatment for megacolon.

When is surgery considered?
If repeated evacuation, medication, hydration, and an individualized diet no longer control the problem, a surgeon may recommend a subtotal colectomy, removal of most of the colon. The exact extent is adjusted to the cat’s anatomy and the cause; an obstruction, pelvic abnormality, or diseased segment may change the operative plan.
How chronicity affects the discussion
Duration is one factor in deciding whether the colon can recover. MSD Veterinary Manual reports that clinical signs lasting longer than six months have been associated with irreversible changes. In its discussion of pelvic-fracture-associated megacolon, the American College of Veterinary Surgeons describes loss of function after roughly four to six months. These are source-specific clinical observations, not a rigid deadline that applies to every cat. A cat with a shorter history can have severe disease, and a cat with a longer history may still require an individualized assessment.
Questions to ask before consenting
- What cause is suspected, and has an obstruction or pelvic narrowing been ruled out?
- How much colon is expected to be removed, and could the plan change during surgery?
- What medical options remain, and what evidence shows they are failing?
- What anesthetic, surgical, and patient-specific risks apply to this cat?
- How will fluids, pain, feeding, and stool output be monitored after the operation?
- What signs after discharge require an immediate call or emergency visit?
What should you expect after colectomy?
Soft stool or diarrhea is common during recovery and can persist for weeks to months while the remaining bowel adapts. Some cats continue to have soft stool; others experience recurrent constipation. The available veterinary references do not promise one universal long-term stool consistency.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsSerious complications are uncommon enough that they should not overshadow the usual recovery discussion, but they must be understood. Failure of the intestinal anastomosis (the surgical join) can cause septic peritonitis and requires urgent treatment. Your surgeon can explain the risks in the context of your cat’s age, hydration, blood work, colon condition, and other illnesses.
The American College of Veterinary Surgeons reports that most cats have an excellent quality of life after surgery. MSD Veterinary Manual also notes that some cats never return to normal stool. Quality of life, appetite, comfort, hydration, and the ability to pass stool are more meaningful measures than expecting a perfectly formed bowel movement every time.
Medical management versus surgery: how veterinarians weigh the options
| Question | Medical management | Subtotal colectomy |
|---|---|---|
| Best fit | A cat that improves after hydration, evacuation, medication, and a suitable diet, or one whose underlying cause is treatable without removing the colon. | A cat with persistent loss of colonic function or repeated relapse despite properly supervised medical care. |
| Main ongoing commitment | Medication, diet, water access, stool monitoring, and periodic rechecks. | Hospitalization, postoperative monitoring, gradual dietary management, and observation for complications or altered stool. |
| Effect of advanced disease | Long-standing obstipation and megacolon are less likely to respond to cisapride alone. | Removes most of the poorly functioning reservoir, but does not guarantee normal stool. |
| Key uncertainty | Recurrence if motility remains inadequate or an uncorrected obstruction persists. | Soft stool or diarrhea, recurrent constipation, and surgical complications such as anastomotic failure. |
The decision is not simply “medication versus an operation.” It incorporates reversibility of the cause, current severity, response to treatment, duration of signs, anesthesia risk, and the practical care your household can provide.
What you can monitor at home
Home observation supports the veterinary plan but cannot replace it. Keep a record of appetite, water intake, vomiting, litter-box attempts, stool amount and texture, medications given, and changes in energy. Report worsening straining, repeated vomiting, refusal to eat, marked lethargy, abdominal pain, or inability to pass stool. Give only the products and doses prescribed for that cat, and never substitute a human enema or laxative without explicit veterinary direction.
