The right dose and monitoring plan are individual; give or change it only as prescribed by your veterinarian.
What methimazole treats—and what it does not
Feline hyperthyroidism means the thyroid produces too much hormone, most often because of benign thyroid enlargement. Signs can include weight loss despite a strong appetite, increased thirst and urination, vomiting or diarrhea, hyperactivity, and changes to the coat. A veterinarian assesses the cat’s signs and overall health and commonly measures total T4, while considering heart and kidney status. A T4 result within the reference range does not always rule out the disease if the signs remain persuasive; further testing may be appropriate. Cornell Feline Health Center
Methimazole inhibits thyroid hormone synthesis. It controls hormone production but does not destroy the abnormal thyroid tissue, which makes it a management option rather than a cure when used by itself. It is available in oral tablets and liquids; transdermal preparations are also described. How dosing is determined
There is no one dose that is right for every cat. The veterinarian selects and adjusts the dose based on the diagnosis, the cat’s health, the formulation, thyroid test results, and clinical response. Published dosing information is context for understanding veterinary care—not a home dosing plan.

For context, the U.S. regulation for methimazole tablets lists a starting dose of 2.5 mg every 12 hours, with titration after three weeks based on serum total T4 and clinical response. It specifies adjustments in 2.5 mg increments, a maximum total dose of 20 mg per day divided, and no single administration above 10 mg; use is restricted to a licensed veterinarian’s order. 21 CFR § 520.1375 Merck’s 2024 veterinary review describes a different initial range: 1.25–2.5 mg per cat orally or transdermally every 12 hours, with adjustments at two-week intervals to bring T4 into the low-to-mid reference interval. These are distinct dose contexts, not interchangeable instructions; the cat’s prescription and applicable product labeling govern. Merck Veterinary Manual
- Follow the exact prescription label and your veterinarian’s directions.
- Do not start, increase, decrease, or stop methimazole on your own—even if your cat seems better or develops a possible side effect. Contact the veterinary team for instructions.
- If a dose is missed or difficult to give, ask the prescribing veterinarian what to do rather than guessing or doubling a dose.
Side effects and when to call the veterinary team
Digestive upset is the most common adverse effect identified by Merck. Vomiting and reduced appetite are also reported; a veterinarian may consider a transdermal formulation if gastrointestinal intolerance is a concern. Other possible effects include fever, anemia, and lethargy. Less common but serious reactions can affect blood cells, the liver, or skin. Merck reports that serious adverse effects develop in fewer than 5% of treated cats; listed reactions include agranulocytosis, thrombocytopenia, liver injury (hepatopathy), and severe facial itching. Contact your veterinary team promptly if your cat has marked appetite loss, repeated vomiting, profound lethargy, fever, unusual bleeding, yellowing of the skin or eyes, facial itching or swelling, or another abrupt change. Do not decide independently to stop or alter a prescribed dose; ask the veterinarian how to proceed.

Why blood tests and follow-up matter
Follow-up checks help the veterinarian determine whether treatment is controlling thyroid hormone levels and watch for adverse effects. During dose adjustment and early treatment, veterinary references call for repeat thyroid testing and blood and chemistry monitoring. Merck describes checks every two weeks during the first three months, followed by T4 checks every three to six months. The schedule may differ with the cat’s response and health. Kidney function is an important part of that follow-up. Hyperthyroidism can mask underlying kidney dysfunction, and normalizing thyroid hormone can make it apparent—even if earlier kidney markers looked normal. This does not mean methimazole directly causes all kidney disease. Depending on the cat, the veterinarian may monitor kidney values, urine concentration, and blood pressure. Handling precautions for caregivers
