A newborn kitten briefly showing its tongue cannot be diagnosed from that behavior alone. The available kitten-care and veterinary guidance does not establish one specific harmless explanation for tongue protrusion, so do not assume the kitten is fine—or assume it is in danger—based only on the photograph or moment you noticed.
Check four things immediately: how the kitten is breathing, whether it responds normally, whether it can feed, and whether the tongue and gums have a healthy-looking color rather than gray, blue, or purple. Open-mouth breathing, gasping, obvious effort, or abnormal blue or purple coloration is an emergency requiring prompt veterinary care.

What a tongue sticking out does—and does not—tell you
A tiny tongue outside the mouth is an observation, not a diagnosis. A newborn may briefly hold its tongue forward while handling, vocalizing, nursing, or settling, but the sources available for neonatal-kitten care do not identify a confirmed cause for this exact behavior. They also do not support declaring it normal from an image or a short video.
Use the tongue as a reason to assess the whole kitten. Look at the chest and abdomen, listen for breathing noise, note whether the kitten reacts to touch, and consider what happened immediately before you noticed the tongue. A kitten that is alert, breathing quietly with its mouth closed, and feeding effectively is a different situation from one that is weak, struggling to breathe, or unable to latch. The visible tongue alone cannot make that distinction.

Four observations to make first
- Breathing: Is the mouth closed, or is the kitten breathing through an open mouth? Watch for gasping, unusually noisy or rapid breaths, or pronounced abdominal movement.
- Responsiveness: Does the kitten react to gentle touch and handle itself as it normally does? Marked weakness, limpness, or failure to respond warrants professional advice.
- Feeding: Is the kitten nursing or latching normally? A kitten that cannot latch, repeatedly refuses food, or becomes too weak to feed needs help rather than repeated forced attempts.
- Color: Examine the tongue and gums. Gray, blue, or blue-purple color is a warning sign, not a cute variation of a protruding tongue.
Signs that make this an emergency
The National Kitten Coalition lists the following as signs of respiratory distress in kittens. If any are present, contact an emergency veterinarian promptly; severe respiratory distress requires immediate veterinary care.
| What you see | Why it matters | What to do |
|---|---|---|
| Open-mouth breathing | A kitten normally should not be treated as simply posing when it is breathing with the mouth open. | Seek urgent veterinary care. |
| Gasping | Gasping indicates serious difficulty moving air. | Go to an emergency veterinarian immediately. |
| Unusually noisy or rapid breathing | Abnormal sound or speed can signal respiratory distress. | Call an emergency clinic while preparing to travel. |
| Abdominal movement greater than chest movement | Marked abdominal effort is a listed sign that breathing is taking excessive work. | Do not wait to see whether the tongue retracts; obtain urgent care. |
| Gray or blue gums, or a blue or purple tongue | This abnormal color can accompany inadequate oxygenation. | Treat it as an emergency and leave for veterinary care. |
Keep handling to a minimum while arranging transport. Do not put food, water, or medication into a kitten that is struggling to breathe, because attempting to feed during respiratory distress can make the situation more dangerous. The priority is professional assessment of the airway and breathing.

Using breathing rate as context—not a diagnosis
The National Kitten Coalition gives a general reference of up to 40 breaths per minute at rest for kittens. The publication year is not displayed on the retrieved page. This figure is a reference point, not a test that explains a tongue sticking out and not a substitute for looking at breathing effort, sound, mouth position, and gum or tongue color.

If the kitten appears comfortable right now
If the kitten has a closed mouth, quiet breathing, normal responsiveness, and no gray, blue, or purple discoloration, the behavior still does not identify a cause. Observe without repeatedly opening the mouth or manipulating the tongue. Note whether the tongue retracts, whether the kitten can nurse, and whether the behavior recurs.
Contact a veterinarian for advice if the tongue remains out, the mouth cannot close, feeding becomes difficult, or the kitten’s energy changes. A newborn can deteriorate quickly, and a remote description cannot assess the airway, hydration, temperature, or underlying illness. If your regular clinic is closed and the kitten develops breathing difficulty, use an emergency clinic rather than waiting for a routine appointment.

When bottle-feeding is part of the picture
Bottle-feeding guidance applies to an orphaned kitten, a kitten unable to nurse, or one that is not receiving enough nutrition. It is not a treatment for tongue protrusion and does not apply automatically to every kitten shown with its tongue out.
Use the right supplies
| Item | What the guidance says |
|---|---|
| Milk | Use a commercial milk replacer made for kittens. Veterinary sources recommend it over cow’s milk or homemade mixtures. |
| Bottle and nipple | Use a small kitten nursing bottle with an appropriately sized nipple. The nipple should release only a single drop at a time when tested. |
| Weight monitoring | PetMD notes that a small gram scale can help track an orphaned kitten’s progress; use weight information with veterinary guidance rather than as a replacement for an examination. |
Follow the specific milk-replacer instructions and your veterinarian’s schedule. Formula concentration, volume, and frequency depend on the product and the kitten’s condition; guessing can cause harm.

Feed in a safe nursing position
- Place the kitten belly-down in a natural nursing position. VCA describes a horizontal, head-neutral posture rather than an upright or reclined one.
- Offer the nipple and allow a slow, controlled flow. A single-drop flow is the target; rapid flow can allow milk to enter the airway and may lead to aspiration pneumonia or death.
- Never cradle the kitten on its back. The back-lying position allows liquid to run toward the airway instead of following a normal nursing pattern.
- Do not squeeze the bottle and do not force milk or water into the mouth. ASPCA guidance warns that flooding the mouth can cause aspiration.
- Stop if the kitten cannot latch, becomes weak, or develops any breathing concern. A cold or hypothermic kitten may show no interest in a bottle, and a weak or cold kitten needs veterinary direction rather than repeated attempts.
Today’s Veterinary Nurse also emphasizes aspiration risk during assisted feeding. A nursing bottle is therefore a tool that must be used with controlled flow and correct positioning, not a way to test whether the tongue behavior will disappear.
If milk bubbles from the nose
Milk appearing at the nostrils can mean that liquid has entered the nasal passages or airway. Stop feeding immediately. Check whether the kitten is breathing normally and quietly. If breathing is abnormal, if the kitten remains distressed, or if the concern persists, contact a veterinarian promptly.
Do not resume feeding simply because the tongue retracts or the kitten appears briefly calmer. A veterinarian can decide whether the kitten needs examination or treatment for aspiration and can demonstrate a safer feeding technique.
Practical decision guide
| Situation | Safest next step |
|---|---|
| Tongue is briefly visible, mouth is closed, breathing is quiet, and the kitten responds | Observe the whole kitten rather than diagnosing the tongue position. Arrange veterinary advice if it persists, recurs with feeding trouble, or the kitten’s behavior changes. |
| Tongue is out with open-mouth breathing, gasping, abnormal noise, marked effort, or gray/blue/purple color | Treat the listed respiratory signs as an emergency and seek immediate veterinary care. |
| Kitten will not latch or repeatedly refuses a bottle | Do not force-feed. A cold, weak, or hypothermic kitten may not feed normally; contact a veterinarian for an examination and feeding plan. |
| Milk bubbles from the nose during a bottle attempt | Stop, verify normal breathing, and call a veterinarian if breathing is affected or the problem continues. |
Common mistakes to avoid
- Calling it harmless from a picture: The available evidence does not provide a diagnosis for this exact behavior.
- Waiting for the tongue to go back in during breathing distress: Respiratory effort and color matter more than whether the tongue retracts.
- Using cow’s milk or a homemade substitute: Kitten-care guidance recommends a commercial kitten milk replacer.
- Feeding on the back: Use the belly-down, horizontal nursing posture instead.
- Squeezing the bottle or forcing milk: Rapid flow and force-feeding increase aspiration risk.
- Continuing after nasal milk appears: Stop and check breathing before considering another attempt.
What to tell the veterinarian
When you call, describe when the tongue first appeared, whether it comes and goes, whether the mouth is open, and any sound or effort during breathing. Report the kitten’s responsiveness, ability to latch, recent feeding attempts, and any milk seen at the nose. Mention the color of the gums and tongue and whether the kitten feels cold or seems unusually weak. A short video recorded without delaying emergency transport can help a clinic understand an intermittent episode, but it should never replace urgent care when breathing is difficult.
The takeaway
A newborn kitten with its tongue out needs an assessment of breathing, response, feeding, and color—not a diagnosis based on appearance. A comfortable kitten may be observed and discussed with a veterinarian if the behavior persists, while any open-mouth breathing, gasping, excessive effort, or blue-purple coloration calls for immediate emergency care. If bottle-feeding is necessary, use kitten milk replacer, a slow single-drop nipple, and a belly-down position; stop at once if milk reaches the nose or breathing changes.
