Omeprazole is a proton pump inhibitor (PPI) that lowers stomach acid. Veterinarians sometimes use it in cats with certain stomach or upper small-intestine ulcers and gastric erosions, including erosions linked to some medicines. Its use in cats is off-label, so the decision to use it, the dose, the formulation, and the length of treatment belong to your veterinarian. A published dose range is useful background, not a dosing instruction for your cat.
This guide explains what the drug does, when it is considered, how it is given, what side effects and interactions to watch for, and when a vomiting cat needs an examination rather than a medication.
What omeprazole does in a cat
Omeprazole belongs to the PPI class. It binds to and inhibits the proton pumps of gastric parietal cells, which are the cells that secrete acid into the stomach. The Merck Veterinary Manual’s small-animal ulcer reference puts it this way in its Key Points section:
“Proton pump inhibitors are superior to H2-receptor antagonists to decrease gastric acidity.”

That article is titled Gastrointestinal Ulcers in Small Animals, and it is authored by Shauna Blois, DVM, DVSc, DACVIM-SAIM.
The Veterinary Companion Animal Hospital (VCA) describes omeprazole for ulcers of the stomach and upper small intestine in dogs and cats, and for treating or preventing gastric erosions caused by drugs such as NSAIDs. VCA also notes that feline use is off-label.
Lowering acid helps heal damage that acid is causing, but it does not explain why the damage happened. Merck states that the underlying cause of GI ulceration should also be addressed. A cat that vomits once or twice has not, by that fact alone, been shown to need omeprazole.
When a veterinarian may choose it
Acid suppression is a treatment for a specific problem, not a general response to vomiting. In the Merck entry, the situations where it fits are:

- Gastric ulceration that has been confirmed or is strongly suspected by the veterinarian.
- Gastric erosions linked to a drug, such as an NSAID, where the veterinarian decides acid suppression is warranted.
Merck also states that acid suppression is not routinely needed for uncomplicated acute gastritis. Acute gastritis is the sudden inflammation of the stomach lining that most often shows up as vomiting, and it is commonly managed without routine acid suppression unless ulceration is found or suspected.
Dosage: what the published range means
Merck’s Gastrointestinal Ulcers in Small Animals reference lists omeprazole at 0.5 to 1.5 mg/kg by mouth every 12 hours for treatment of GI ulcers. VCA notes that different conditions may call for different doses.
This is a clinical reference range for a specific indication. It is not a recommendation for your cat, and it does not cover every reason a cat may vomit. A veterinarian chooses the dose after examining the cat, confirming the diagnosis, and considering factors such as:
- The cat’s current body weight, measured on a veterinary scale.
- Liver or kidney disease, which VCA identifies as a reason for caution.
- Pregnancy or nursing status.
- Other medicines the cat already takes.
- The exact product and strength being dispensed, including any compounded form.
Do not calculate a dose from your cat’s weight, and do not use the directions on a human package label. VCA says human label directions may differ from veterinary instructions, and that is the reason the prescribing veterinarian’s directions govern.

How to give it
VCA describes omeprazole as available in tablet or capsule form, with other forms possible through a compounding pharmacy. Merck notes that human formulations are used in cats. Confirm with the clinic what your cat was dispensed, then follow these steps:
- Check the label. Confirm the drug name, strength, and directions match the prescription you were given.
- Give it on an empty stomach. VCA advises giving it by mouth before the first meal of the day.
- Do not crush it or let the cat chew it. VCA specifically advises against crushing tablets or capsules and against allowing chewing.
- If the cat vomits after empty-stomach doses, VCA notes that future doses can be given with food. Ask your veterinarian before changing the timing, because the prescriber decides the schedule.
- If a dose is missed, skip it and resume the regular schedule. Do not double the next dose.
Give the drug for the length of time the veterinarian recommends. The stopping rules are covered below.
Side effects and cautions
VCA lists the following as possible side effects of omeprazole in pets:
- Vomiting
- Decreased appetite
- Gas
- Diarrhea
Contact your veterinarian if any of these worsen, become severe, or keep going. Be especially careful and talk to the vet first if your cat has:

- Liver or kidney disease.
- Pregnancy or is nursing.
- A known allergy to omeprazole or a similar drug.
VCA advises against use in pets with an allergy to omeprazole or similar drugs.
Medicines that need the veterinarian’s review
VCA lists these medicines as requiring caution when given with omeprazole:
- Benzodiazepines
- Certain antibiotics
- Clopidogrel
- Cyclosporine
- Diuretics
- Levothyroxine
- Phenobarbital
Give your veterinarian a complete list of everything the cat receives, including prescription drugs, supplements, vitamins, and herbal products, before the first dose.

If your veterinarian is weighing acid-reducing options, the useful comparison points are the veterinary indication, how strongly acid is suppressed, the interaction profile, and how often the drug must be given. The table below summarizes what the Merck reference states about each option.

None of these is a do-it-yourself substitute for another, and combining them without direction creates the interaction problems described in the table.
How long treatment lasts and how to stop
VCA says to give omeprazole for the duration the veterinarian recommends and not to stop without first speaking with the veterinary team.
The reason is rebound. Merck says rebound hyperacidity is likely after stopping long-term treatment, and it describes gradual tapering for patients treated for at least a month. A veterinary pharmacology reference recommends tapering after use beyond about three to four weeks. Ask your veterinarian for a written taper plan before the course ends, so that the cat is not stopped abruptly.

- Vomit contains blood or dark, digested-looking blood.
- Stool contains blood, which Merck describes as melena.
- The cat appears systemically unwell.
- There may have been exposure to a toxin or a foreign object.
- Signs do not resolve within two days of treatment.
Sudden vomiting is a common feature of acute gastritis. Merck lists possible causes as dietary indiscretion, drug or toxin ingestion, systemic illness, parasites, and infection. Because the same symptom can come from several causes, an examination is the reliable route to a diagnosis, and it should come before any acid-suppressing medicine is started.
Based on the sources reviewed, omeprazole is a reasonable option for cats with confirmed or strongly suspected ulceration, and it should be used only under a veterinarian’s directions for dose, duration, and tapering.
