For most dogs with pyometra, the recommended definitive treatment is urgent veterinary stabilization followed by surgery to remove the ovaries and infected uterus. Medical treatment is a narrower option for selected, stable dogs when preserving breeding potential matters; it can take days to work, may fail, and does not prevent recurrence. Suspected pyometra needs prompt veterinary assessment, even if there is no vaginal discharge.
Why suspected pyometra is urgent
Pyometra is an infection of the uterus that can progress to sepsis and death without treatment. It often occurs in unspayed dogs, and signs can worsen rapidly. The American College of Veterinary Surgeons calls it a medical emergency requiring rapid intervention: ACVS: Pyometra.
Do not wait for discharge before seeking care. With a closed cervix, infected material may be trapped in the uterus, so discharge can be absent; abdominal enlargement may occur instead. A closed cervix also raises concern about rupture during medical management. If your dog may have pyometra, contact a veterinarian or emergency clinic promptly.

What surgery involves
The standard operation is an ovariohysterectomy: the veterinarian removes the ovaries and infected uterus. Before surgery, the team may stabilize the dog with intravenous fluids and antimicrobial treatment. Pyometra surgery can be more complex than a routine spay, especially when the dog is systemically ill; the clinical team may need to manage dehydration, shock, anemia, or sepsis.

ACVS reports survival of 80–100% after ovariohysterectomy when abdominal contamination is avoided and systemic infection is treated and responds appropriately. This is a conditional estimate, not a guarantee or an individual prognosis; severe sepsis and organ failure can make the outlook less favorable. Your veterinarian can discuss your dog’s specific risks and findings.
When medical management may be considered
Medical treatment is generally considered only for a stable dog when preserving breeding potential is an important goal. Veterinary protocols described in the sources use prostaglandin-based medication alongside antibiotics and fluids. This is not a home-treatment option or a substitute for prompt veterinary evaluation.

There are important limits: improvement may take several days, treatment may not work, and adverse effects or uterine rupture are possible. Merck advises emergency ovariohysterectomy if medical management does not produce major improvement within 48 hours or if the dog worsens: Merck Veterinary Manual: Pyometra in Small Animals. The veterinarian should set the reassessment schedule and explain in advance when to switch to surgery.
A closed cervix and critical illness weigh strongly against attempting medical management. UC Davis guidance describes surgery as the only reasonable treatment for a critically ill dog: UC Davis: Pyometra. The veterinarian must assess the cervix, overall condition, and ability to monitor the dog before deciding whether a medical approach is appropriate.

Signs and how veterinarians diagnose pyometra
Possible signs include lethargy, reduced appetite, vomiting, increased thirst or urination, and vaginal discharge. Open-cervix cases may have pus or blood-tinged discharge. With a closed cervix, discharge may be absent and the abdomen may enlarge. These signs are not a diagnosis on their own, but suspected pyometra warrants prompt care.
Diagnosis combines the dog’s history and physical examination with tests selected by the veterinarian. Ultrasound or radiographs can help identify an enlarged, fluid-filled uterus. Blood or urine tests can assess infection and metabolic status, including factors relevant to anesthesia. The findings help the team judge urgency, stabilization needs, and treatment options.

What recovery after surgery requires
Follow the treating clinic’s discharge instructions for your dog. ACVS describes postoperative care as generally similar to routine spay recovery, with prescribed antibiotics as directed, restricted activity for two weeks, and protection of the incision from licking or other self-trauma: ACVS postoperative guidance.

- Limit play, stairs, and off-leash activity for the period specified by the clinic.
- Keep the incision protected from licking or chewing; a recovery collar is one possible option if the veterinarian recommends it.
