Orbax for Cats: Overview, Dosage, and Side Effects

Orbax (orbifloxacin) is a prescription fluoroquinolone. The U.S. cat suspension label covers skin infections at 3.4 mg/lb (7.5 mg/kg) once daily, with retinal and nervous system warnings.

Orbax is the brand name for orbifloxacin, a prescription fluoroquinolone antibiotic. In the United States, the Orbax oral suspension is labeled for cats for skin infections such as wounds and abscesses caused by susceptible strains of Staphylococcus aureus, Escherichia coli, and Pasteurella multocida. The label dose is 3.4 mg/lb (7.5 mg/kg) once daily. The most important cat-specific warning concerns the eyes and nervous system: fluoroquinolones can affect the feline retina, so vision changes or dilated pupils need a prompt call to your veterinarian.

What Orbax is and what it is labeled to treat

Orbifloxacin is a synthetic, broad-spectrum fluoroquinolone antibacterial. Orbax is its brand name and is restricted in the United States to use by or on the order of a veterinarian. Because it is a fluoroquinolone, it should be reserved for infections a veterinarian has diagnosed as likely bacterial and susceptible to the drug; it does not treat viral illness, fungal infection, or parasites.

The cat oral-suspension label is narrower than many owners expect. Its cat indication covers skin infections, including wounds and abscesses, caused by susceptible strains of S. aureus, E. coli, and P. multocida. That is not the same as approval for every infection or every organism, so a label-based discussion should stay within that scope.

The Orbax tablets are described differently. The FDA Freedom of Information Summary for tablet application NADA 141-081 lists the tablet use as management of diseases in dogs and cats associated with bacteria susceptible to orbifloxacin. The tablets come in 5.7 mg, 22.7 mg, and 68 mg strengths and are prescription-only. Because the two products are labeled differently, always confirm which formulation your cat was prescribed before comparing any instructions.

Cat oral suspension dosage

The labeled dose for cats is 3.4 mg/lb (7.5 mg/kg) body weight once daily. The ORBAX oral-suspension prescribing label states: “DO NOT EXCEED 3.4 mg/lb (7.5 mg/kg) BODY WEIGHT PER DAY IN CATS.” The label directs treatment to continue for two to three days beyond the cessation of clinical signs.

These figures describe the labeled product. They are not a calculation for your cat. Your veterinarian sets the dose and duration based on your cat’s weight, the infection, and the response to treatment, and the bottle label on your prescription should match what the clinic told you.

How to measure the liquid

  1. Shake the bottle before use, as the label directs.
  2. Insert the supplied syringe adapter into the bottle before first use.
  3. Withdraw the prescribed amount with the calibrated syringe. The carton includes a 3 mL syringe.
  4. If the syringe is lost or damaged, ask the dispensing clinic or veterinarian which calibrated device to use. Do not estimate the dose with a household spoon.

What to do if there is no improvement

The label says susceptibility testing should be determined before use. Treatment may begin before results are back, and the veterinarian should adjust therapy when results arrive. If there is no improvement within three to four days, the label says to reevaluate the diagnosis and consider a different course. Do not stop or extend the course on your own; call the clinic with your cat’s progress.

Suspension and tablets are not interchangeable

The label states that the cat suspension and Orbax tablets are not bioequivalent. Per mg/kg, the suspension produces lower and more variable plasma concentrations than tablets. The manufacturer’s package insert also notes that tablets may be used when higher blood levels are needed, under veterinary direction. Never swap one form for the other using the milligram amounts from the other product’s directions.

Your veterinarian chooses the form based on the type of infection and the blood levels needed. Neither form is generally better; the right one depends on your cat’s diagnosis.

Side effects reported in cats

The current ORBAX suspension label lists the following cat adverse events from post-approval reporting, in decreasing order of reported frequency: blindness, mydriasis (dilated pupils), anorexia, ataxia, depression or lethargy, vomiting, convulsions, abnormal retina, and hypersalivation. The label says blindness was temporary in some cases.

The label is explicit that post-approval reports cannot reliably estimate how often an event occurs or establish that the drug caused it. Do not read the list as a list of common effects, and do not treat the ordering as a rate. Some events may have been reported in cats with other illnesses or other medicines.

Retinal and nervous system warnings

  • The label states that fluoroquinolone use in cats has been reported to adversely affect the retina, and advises caution.
  • Use caution in animals with known or suspected central nervous system disorders. The label notes that rare CNS stimulation may lead to convulsive seizures.
  • Hypersensitivity to quinolones is listed as a contraindication for cats.

Higher-dose study results

In a 30-day animal safety study described in the label, the higher suspension doses tested (45 and 75 mg/kg/day) caused mild gastrointestinal effects, including more emesis, salivation, lacrimation, and soft, mucoid, or watery feces. Decreased food consumption was also noted at those doses. These doses were several times the labeled dose and are not estimates of what happens at the prescribed amount.

Palatability

The label cites a 101-cat field palatability study by Intervet Inc. (the manufacturer) in which 95% of cats accepted the oral suspension. The label does not give the study year, so treat this figure as a manufacturer-reported result from an unspecified date rather than a current general statistic.

Drug interactions to report to your veterinarian

Oral products or supplements containing iron, aluminum, calcium, magnesium, or zinc may reduce how well quinolones are absorbed. The label also identifies potential interactions with theophylline, cimetidine, cyclosporine, and oral anticoagulants. Give your veterinarian a complete list of every medicine, supplement, and antacid your cat receives, including ones bought without a prescription, and do not change the timing or dose yourself.

When to contact the veterinarian

  • Promptly, the same day: any change in vision, bumping into objects, dilated pupils, loss of appetite, vomiting, drooling, or unusual lethargy.
  • Urgently: seizures, collapse, loss of coordination, or severe behavior changes. Go to an emergency clinic if your regular veterinarian is not reachable.
  • Also call if: the skin infection is not improving within three to four days, or if your cat stops eating during the course.

The list of reported events does not determine whether Orbax caused a particular symptom. The clinician must evaluate each case, which is why any new symptom should be reported rather than assumed to be unrelated.
