Prazosin is not a proven way to prevent another urinary blockage in cats. The 2025 iCatCare consensus guideline says that, on current evidence, it is not generally recommended for this purpose. A veterinarian may still consider it for an individual cat, but the dose, the duration, and whether to use it at all are decisions for the treating veterinarian.
If your cat is straining in the litter box or passing little or no urine, that is an emergency. The signs that need immediate veterinary care come first below.
Signs that need emergency veterinary care before any medication discussion
Male cats are the cats most at risk for urethral obstruction, a blockage of the tube that carries urine out of the bladder. Merck Veterinary Manual’s entry “Urethral Obstruction in Small Animals” (last updated May 2025) identifies male cats as uniquely predisposed to it as a complication of lower urinary tract disease. It also warns that owners may mistake frequent, unproductive attempts to urinate for constipation. That matters because a complete blockage can cause severe systemic illness and death quickly.
Treat these signs as an emergency:
- Repeated trips to the litter box with little or no urine produced
- Straining, crying, or crouching without passing urine
- Only a few drops of urine, or urine that appears bloody
- Weakness or collapse
Do not wait at home to see whether a medication helps. Merck states: “UO is an emergency condition, and stabilization should be prioritized in severely affected patients.” (UO refers to urethral obstruction; the sentence is from Laura Van Vertloo, DVM, MS, DACVIM, with peer review by Joyce Carnevale, DVM, DABVP.) The same entry says obstruction ultimately must be relieved. For male cats, relief should be performed under general anesthesia whenever possible, with adequate pain management. Oral prazosin does not replace that treatment.

What prazosin is and why veterinarians tried it
Prazosin is an alpha-1 adrenergic antagonist, a drug that relaxes smooth muscle. After a urethral obstruction, some veterinarians have used it on the rationale that smooth-muscle relaxation may address a component of urethral spasm. That reasoning explains why the drug was studied. It does not, by itself, show that the drug works.
Does prazosin prevent urinary blockage in cats?
On current evidence, no benefit has been shown, and prazosin is not generally recommended to prevent recurrence. The 2025 iCatCare consensus guideline reports that more recent studies did not show prazosin reduced recurrent urethral obstruction. It also says further work is needed to determine whether subsets of cats might benefit.

A 2023 knowledge summary from Veterinary Evidence, written by Rebecca Hearne, assessed three studies and judged the overall evidence weak. The table below shows each study as that summary reports it.

The two randomized trials
Both trials were randomized and double-blind, which are the higher-quality designs in the summary. Neither found a statistically significant difference in recurrent obstruction. The trials were small, however. Forty-seven cats finished the 2017 trial and 65 finished the 2021 trial, which is too few for statistically reliable conclusions. A trial that does not detect a difference has not proven that no cat could benefit, which is why the guideline still calls for more work.

The retrospective cohort
Conway et al. enrolled 485 cats, and 388 remained in the relevant analysis after exclusions. Within 14 days after discharge, 73 of 302 prazosin-treated cats (24%) had recurrence, compared with 11 of 86 untreated cats (13%). These are associations in an observational study. Treatment was not standardized, and selection bias, owner adherence, hospital care, and home environment all complicate comparison. The figures do not show that prazosin caused recurrence, and they should not be read as the chance that any particular cat will block again. The same design also cannot show whether prazosin lowers risk or whether phenoxybenzamine, a related alpha-blocking drug, raises it.
Dosage: why a study dose cannot be copied
The studies used different doses and durations, and none of them is a prescription for your cat. No single standard dose has been established for a cat after urethral obstruction. The appropriate dose depends on the individual cat and the veterinarian’s plan, including:

- The diagnosis, including whether the blockage was in the urethra or the ureters
- Hydration and kidney status
- Other medications the cat takes
- The formulation and strength available
- How the cat will be monitored during treatment
Merck’s Veterinary Manual gives an example of 0.25–0.5 mg/kg by mouth every 12–24 hours as needed. It appears in the entry “Obstructive Uropathy in Dogs and Cats” (full review March 2025, last updated June 2025) in the context of feline ureteral obstruction, a blockage of the ureters, which carry urine from the kidneys to the bladder. That is a different condition, and the dose is weight-based. It should not be used as the dose for a cat recovering from urethral obstruction.
Never split tablets, skip doses, stop treatment, or change the amount at home without speaking to the prescribing veterinarian.

Side effects and safety
Reported side effects
Veterinary Evidence’s 2023 summary reports lethargy, diarrhea, and anorexia (loss of appetite) among adverse effects noted in the 2017 trial. These effects can contribute to dehydration and complicate recovery, particularly if the cat is passing large volumes of urine after the obstruction (post-obstruction diuresis).
Low blood pressure
The 2025 iCatCare guideline identifies hypotension, or low blood pressure, as a potential adverse effect of alpha-1 antagonists. It advises delaying their start until fluid deficits and azotemia (a buildup of waste products in the blood, linked to kidney strain) have been corrected.
When to call your veterinarian
Contact the prescribing veterinarian promptly for marked weakness, worsening lethargy, poor appetite, or diarrhea. A phone call is not an examination. If your cat cannot urinate, has collapsed, or seems to be in pain, go to an emergency clinic rather than waiting for a callback.
