Vitamin B12 (cobalamin) is given to cats when a veterinarian has measured a low level or strongly suspects a deficiency, especially in cats with chronic intestinal disease or exocrine pancreatic insufficiency (EPI). The doses published in veterinary references describe defined clinical situations. They are not a home dose, and a cat should not receive B12 from a human product or an injection without veterinary instruction. The safe sequence is a blood test, a search for the underlying disease, and scheduled rechecks while the cat is supplemented.
What vitamin B12 does in cats
Cobalamin is the form of vitamin B12 that cats use. It comes from food and must be absorbed through the gut. Cornell’s diagnostic center describes serum cobalamin as a reflection of absorptive function in the distal small intestine, the lower segment of the small bowel. When that segment is inflamed or diseased, or when the pancreas does not supply enough digestive enzymes, serum cobalamin can fall. A low result therefore usually points to a problem with absorption or digestion rather than a simple shortfall in the diet.
Why a low level matters
Low cobalamin is most often discussed alongside the following conditions in veterinary references:
- Chronic enteropathy (including inflammatory bowel disease). Cornell’s IBD material explains that intestinal disease can hinder B12 absorption, and that supplementation is appropriate when an affected cat is deficient.
- Exocrine pancreatic insufficiency. Merck’s veterinary reference lists hypocobalaminemia as a feature of feline EPI.
- Feline cholangitis. Merck notes that low cobalamin in this setting raises concern for severe small-bowel malabsorption or severe pancreatic disease.
- Hepatic lipidosis. Merck describes B12 deficiency as common in this condition, in a context that may include reduced appetite and concurrent intestinal or pancreatic disease.
A low B12 result is a clue that the gut or pancreas needs investigation. It is not a diagnosis on its own, and the presence of a deficiency does not explain every sign a cat is showing.

How a veterinarian confirms and monitors low B12
Serum cobalamin is a routine blood test that a veterinarian can order. Cornell identifies serum B12 and folate measurement as useful for characterizing some small-intestinal malabsorptive disorders. Because B12 has a short half-life in cats, Cornell’s diagnostic center recommends regular measurement while a cat is being supplemented. The usual sequence looks like this:
- The veterinarian measures serum cobalamin, often together with folate.
- If the level is low, the cat is investigated for the cause, such as chronic enteropathy, EPI, or cholangitis, rather than treated with B12 alone.
- The veterinarian chooses a route, product, dose, and duration that fit the diagnosis.
- Serum cobalamin is rechecked during and after supplementation to judge response and decide how long treatment continues.
Reference dosing schedules
The table below lists schedules published in Merck Veterinary Manual entries. They illustrate what veterinary references describe for specific conditions. They are not a universal dose, and they do not replace the veterinarian’s plan for a particular cat.

The chronic-enteropathy entry was last reviewed in August 2025. Clinical references are revised over time, so ask the veterinarian whether a published schedule matches current guidance before accepting it for your cat. Dose, route, and length of treatment are decisions for the veterinarian, who can adjust them to the cat’s weight, diagnosis, and recheck results.
Oral or injection: how the options compare
Both routes appear in the same veterinary references for the same conditions, and none of the cited references ranks one as superior. The practical differences are mostly about who gives the dose and how the plan fits daily life.

If a cat is a poor candidate for one route because of stress, handling difficulty, or an existing health problem, that is a conversation to have with the veterinarian, who can weigh it against the underlying condition.

VCA Animal Hospitals describes cyanocobalamin as generally well tolerated in animals. The most common complaint it lists is a brief sting at the injection site. Its cyanocobalamin medication guidance states: “The injection may sting but typically resolves quickly.”

Allergic reactions are rare but possible. VCA lists the following signs and advises stopping the medication and contacting the veterinarian promptly if they appear:
- Irregular or difficult breathing
- Rash
- Fever
- Facial puffiness or swelling
Merck’s hematinics overview reports no major toxicity for B12 at the listed veterinary regimens. That finding applies to those regimens and should not be read as a guarantee that every product or dose is risk-free.

Medications and supplements to disclose
VCA advises telling the veterinarian about all medications, vitamins, and supplements a cat receives, before starting B12. It specifically lists chloramphenicol, and omeprazole or pantoprazole, as drugs that call for caution when combined with cyanocobalamin. A cat that is already on several gut or pancreas medications needs the veterinarian to review the full list before any B12 plan is set.
What to do when a cat may be low in B12
B12 supplementation should follow a diagnosis, not replace one. Contact a veterinarian rather than starting supplements at home if your cat shows any of these:
- Ongoing weight loss or poor appetite
- Chronic vomiting or diarrhea
- A known diagnosis of inflammatory bowel disease, EPI, or liver disease
- Any sign of an allergic reaction after a B12 product has been given
When the veterinarian recommends a vitamin B12 supplement for cats, a veterinarian-selected preparation and dose are the safe choice. Retail human B12 products are not interchangeable with a formulation chosen for a specific cat.
