Repetitive, Excessive and Compulsive Grooming in Cats: Causes, Vet Workup and What Helps

Overgrooming in cats is a sign, not a diagnosis. Learn the medical causes to rule out, what the vet checks, and what helps if stress is involved.

A cat that licks hair off its belly or grooms until bald patches appear is showing a sign, not a diagnosis. Itch, pain, parasites, allergy, infection and other skin disease are all common reasons for self-trauma, so a vet visit comes first. A compulsive or stress-related explanation is considered once medical causes have been investigated and addressed.

Normal grooming versus a problem

Cats groom a lot. Cornell University College of Veterinary Medicine, citing Pamela Perry, D.V.M., puts typical grooming at roughly 30% to 50% of a cat’s day (the page does not state a date). That figure describes healthy cats. Time spent licking alone does not establish a disorder, so judge by what the grooming is doing to the cat, not by the clock.

Look for these signs, which deserve attention:

  • Broken or missing hair, thinning coat or bald patches (alopecia, or hair loss; when the cat causes it, self-inflicted alopecia).
  • Irritated, red or wounded skin, or visible lesions.
  • A change in how, where or how often your cat grooms.

Cats can hide itch. Merck’s cat-owner guidance notes that a cat can be itchy without obvious scratching, so excessive grooming can be subtle. Much of the licking may happen when you are not watching.

Why overgrooming is a sign, not a verdict

Merck explains that hair loss can come from disease affecting hair follicles or shafts, slowed hair growth, or discomfort that leads a cat to lick, chew and otherwise injure its own skin. Itch and pain often go with inflammatory causes. Possibilities include:

  • Fleas and other parasites. Flea allergy can be present even if you never see a flea.
  • Allergy, including to food or the environment.
  • Infection and other skin disease.
  • Pain. VCA notes that pain, neurologic disease and skin disease can all produce similar signs.
  • Stress or a compulsive disorder, considered after the above.

Hair loss in a symmetrical pattern, in places the cat can reach, suggests self-inflicted loss. It does not tell you the reason. Don’t diagnose from the pattern alone.

What “psychogenic alopecia” means

The term is not a shortcut label for any bald cat. VCA’s article, written by Debra Horwitz, DVM, DACVB, and Gary Landsberg, DVM, DACVB, DECAWBM, says: “The diagnosis of psychogenic alopecia as a compulsive disorder is reserved for those cases in which no underlying medical problem is evident.”

Cornell’s Dr. Perry puts the same sequence in practical terms: “If all medical problems have been ruled out, then we normally treat over grooming as the result of some form of stress in a cat’s life.”

What the veterinary workup involves

Evaluation starts with the history and a physical exam. Which tests follow depends on what the vet suspects. Options described by Merck and VCA include:

  • Parasite checks.
  • Skin tests and cultures, and examination of broken hair shafts.
  • Blood and urine tests.
  • A diet trial when food allergy is a concern.
  • Skin biopsy in some cases.

Merck says behavior diagnosis needs the clinical history and exam plus whatever testing is indicated to exclude medical conditions. Useful things to bring: when the licking began, where on the body it happens, any changes at home around that time, current flea control, diet, and photos or video of the grooming.

Managing it when stress is part of the picture

Treat any identified medical cause under your vet’s direction first. If medical issues have been excluded or addressed and stress seems relevant, these steps come from Cornell and VCA guidance.

Change the environment gradually

  • Make changes and introductions slowly, and keep familiar bedding during a move (Cornell).
  • Provide cat-friendly high places and a rest area where the cat can retreat.
  • Keep a predictable routine.

Add play and enrichment

  • Schedule daily interactive play. Cornell suggests 10 to 15 minutes a day; that is its practical recommendation, not a universal rule.
  • VCA recommends scheduled social interaction and stimulating toys that use food and textures. A cat food puzzle toy fits here as an optional aid. It does nothing for parasites, pain, allergy or infection.
  • Use reward-based training and never punishment.

Medication

Some seriously stressed cats may be given temporary anti-anxiety medication by a veterinarian. The choice, dose and monitoring are the vet’s decision. Repetitive licking can persist as a learned or self-rewarding habit even after the original trigger is addressed, so expect improvement to take time.

What is not established

The sources reviewed give no dated figures on how common compulsive grooming is, no comparison of treatment success rates, and no controlled evaluation of enrichment products. Merck’s page was last updated in May 2025 (reviewed by Karen A. Moriello, DVM, DACVD); the Cornell and VCA pages do not display publication dates.
