Reverse sneezing is a sudden, brief series of noisy, rapid inward breaths, often with the cat stretching its neck out. It is usually thought to be a reflex that tries to clear irritation around the nose, mouth, or throat. The episode can sound alarming, and it is easy to mistake for choking, coughing, vomiting, or trouble breathing. In most cases a single brief episode in an otherwise healthy cat is not an emergency, but new, frequent, or persistent episodes deserve a veterinary check.

What reverse sneezing looks and sounds like

During an episode the cat usually stands still, extends its neck, and pulls air in quickly through the nose, producing a loud snorting or gasping sound. The episode is brief, and most cats appear to return to normal afterward. Because the sound resembles several other problems, the description alone cannot tell you what is causing it. Some owners find it helpful to compare the sound against a short video, which is one reason veterinarians often ask for one.

Reverse sneezing or something else?

Several other events can look similar. Before assuming the cause, consider whether the sound may actually be:

  • Coughing, which is usually a forceful expulsion of air from the chest and often follows a different pattern of breathing.
  • Gagging or retching, which may come before vomiting or hairball expulsion.
  • Choking, where something may be stuck in the airway. This is an emergency.
  • Difficulty breathing, which can involve the chest and belly working hard, open-mouth breathing, or a bluish tinge to the gums.

If you are not certain which of these you are seeing, treat the episode as something to check with a veterinarian rather than something to diagnose at home.

What can trigger it

Reverse sneezing is a description of an episode, not a single known cause. Veterinary education material and pet health websites list several possible triggers and associated factors. The reviewed sources do not say how often each one is responsible, so none should be assumed to be the usual explanation for a particular cat.

Irritants in the air

Pollen, dust, grass or other plant material, smoke, perfume, and other strong odors are commonly cited as things that may irritate the nasal passages and set off an episode.

Foreign material in the nose or throat

A small object such as a piece of grass can irritate the airway, and an inhaled foreign body is listed among the possible underlying problems.

Infection and inflammation

Upper respiratory infection, rhinitis (inflammation of the nasal lining), and nasal mites are all reported as possible causes. Nasal polyps, which are growths in the nasal passage, are also listed. Cats.com notes that nasal tumors are a rare cause.

Airway anatomy

Cats with short-faced, or brachycephalic, anatomy are listed as having a possible predisposition, because the shape of the nose and airway can make irritation more likely to provoke a reflex.

Other associated factors

  • Intranasal vaccination, which is listed as a possible factor in some reports.
  • Eating or drinking very quickly.
  • Overexcitement, such as during play.
  • Dental disease, which Cats.com includes among possible underlying problems.

When to worry: choosing the right level of care

Rather than sorting episodes by how alarming they sound, it helps to sort them by how the cat behaves before, during, and after. The table below sets out three situations and the response each one calls for.

Situation What it may mean Suggested next step
A single brief episode in a cat that otherwise seems well, with normal breathing and eating afterward Often a self-limiting reflex to irritation Stay calm, note what happened just before, and record a video. Monitor the cat and watch for any change.
Episodes that are new, recurring, more frequent, more severe, or persistent, or that appear with nasal discharge, coughing, changes in breathing, or other signs of illness Possible underlying nasal, airway, dental, or infectious problem that needs assessment Contact a veterinarian for an appointment. Bring your notes and videos.
Apparent breathing distress, blue gums, collapse, or possible choking A possible emergency, including an obstructed airway Seek immediate veterinary care. Do not wait for the episode to pass.

What to do during an episode

  1. Stay calm and keep the cat in a quiet space. Excitement can prolong or worsen the episode.
  2. Check whether the cat is breathing comfortably. If it is in distress, follow the urgent row in the table above and go to a veterinarian without delay.
  3. If the episode is brief and the cat looks normal, note what happened just before it, such as eating or drinking quickly, play, a spray or candle, or exposure to smoke or dust.
  4. Record a short video on your phone, with the date and time if you can. Try to capture the neck position and sound, since a recording is often more useful to a veterinarian than a verbal description.
  5. Once the episode ends, watch how the cat behaves over the next hour or so. Keep a simple log of episodes, including how long each lasted and how often they occur.
  6. Do not give human medications or home remedies in place of an assessment.

What the veterinary visit involves

A veterinarian will usually start with a history of the episodes and a physical examination, including the nose, mouth, and breathing. Many cats with occasional episodes need no further testing. For frequent or chronic episodes that do not respond to treatment, PetMD notes that a veterinarian may examine the nasal cavity and mouth more closely, which can involve sedation and a procedure called rhinoscopy, in which a small camera is used to look inside the nose. Treatment depends on what is found. No single treatment is established for reverse sneezing in general, so the plan should come from your own veterinarian.

Prevention: reducing likely irritants

Since many of the possible triggers are airborne, the most practical step is to reduce exposure where it is feasible. This includes limiting cigarette and other smoke, keeping dust down, and cutting back on aerosol sprays, air fresheners, scented candles, and strong household chemicals. Cats.com suggests low-dust cat litter as one way to reduce airway irritation. Low-dust litter is an optional measure rather than a guaranteed fix, and no particular brand should be expected to prevent episodes. Exposure reduction is also not a promise that episodes will stop, particularly if the cause is an infection, polyps, or anatomy.
