Signs of Pain in Cats: How to Tell If Your Cat Is Hurting

Cats hide pain well, so the clues are usually changes from normal routine: reluctance to jump, withdrawal, grooming or litter-box shifts, and posture changes. Here is how to recognize them and what to do next.

A cat in pain often looks like a cat that is simply a little off. Cats tend to hide discomfort, so the useful clues are usually changes from that cat’s own normal routine: a reluctance to jump, a new hiding spot, less play, or a shift in litter-box habits. A change like this is a reason to call a veterinarian. It is not a diagnosis you can make at home.

Why pain in cats is easy to miss

Veterinary guidance from VCA Animal Hospitals notes that cats are well adapted to concealing illness and pain, so a cat may not cry out, limp visibly, or stop eating before an owner notices something is wrong. That is why owners are often the first to spot a problem. Because you know what your cat normally does, you are in a good position to notice when a habit, a posture, or a way of moving has changed, even when the change seems small.

The main signs, grouped by what changes

The signs below are grouped by the kind of change you might see. Any one of them can have causes other than pain, so treat each as a prompt to observe more closely and, if it persists, to ask a veterinarian.

Movement and activity

  • Limping, stiffness, or trembling in the back legs
  • Reluctance to jump onto furniture or to use stairs
  • Taking several small jumps to reach a familiar resting spot instead of one
  • Slower getting up, lying down, or changing position
  • Reduced endurance during play, or less interest in play they used to enjoy
  • Difficulty reaching the litter box or other favorite places

Older cats are often assumed to be “just slowing down.” Veterinary guidance advises discussing these mobility changes with a vet rather than accepting them as normal aging.

Social behavior, handling, and voice

  • Hiding more often, or withdrawing from people and other pets
  • Interacting less than usual, including less greeting or following you around
  • Resisting being picked up, or objecting to brushing
  • A new hiss, growl, swat, or bite when approached or touched in a sensitive area
  • More meowing than usual, or vocalizing at unusual times or in the litter box

Purring is not a reliable sign of comfort. A cat can purr while in pain, so look at purring alongside the other signs rather than as proof that all is well.

Grooming, appetite, sleep, and litter-box habits

  • Less grooming, leading to a matted or untidy coat
  • Overgrooming focused on one area, sometimes causing irritation or bald patches
  • Sleeping noticeably more or less, or choosing an unusual place or position to rest
  • Eating less, or a change in appetite or drinking
  • Struggling to step into a high-sided litter box, squatting awkwardly, or walking to the box more slowly
  • Going outside the litter box

Litter-box changes are among the most important things to report to a veterinarian, because they can have several different causes. Describe exactly what you see rather than guessing at the reason.

Posture and facial expression

  • A hunched or guarded posture, or legs tucked under the body
  • A tense, grimace-like look, squinting, or a vacant stare
  • Ears or whiskers held in a position that is unusual for that cat

Facial signs are easy to over-read. They are most useful when they appear alongside changes in movement or behavior, and they should never be used alone as a test for pain.

Read the pattern, not a single cue

A single odd moment usually means little. A pattern that persists for days, or a cluster of changes that appear together, is what deserves attention. The most reliable comparison is your cat’s own baseline: how it moved, ate, slept, groomed, and greeted you last month. When you describe the change to a veterinarian, the difference from that baseline is often more informative than any single sign on the list above.

What to do if you suspect your cat is hurting

  1. Write down the baseline and the change. Note when you first noticed the difference, which activities have changed, whether your cat can still reach its favorite places, and any changes in eating, drinking, grooming, sleeping, handling, or litter-box use.
  2. Record short clips if it is practical. A brief video of your cat walking, jumping, or getting up can show details that are hard to describe and easy for a veterinarian to miss in a clinic visit.
  3. Call your veterinary clinic. International Cat Care’s carer guidance states: “If you have any concerns that your cat may be in pain, contact your veterinary clinic.” Ask for an appointment if you suspect pain or if the changes persist.
  4. Do not give human pain relievers or leftover medication. Many common human medicines are toxic to cats. Any pain treatment should be prescribed for your specific cat by a veterinarian.
  5. Bring your notes to the visit. The veterinarian’s job is to look for the cause and decide what to do about it. Your written observations make that assessment faster and more accurate.

When to seek emergency care

VCA’s guidance for young cats specifically identifies yowling or straining in the litter box as a reason for emergency veterinary attention. Do not wait for a routine appointment if you see either of these. The sources behind this article do not provide a complete triage list for every acute symptom, so if you are unsure whether a change is urgent, call your veterinary clinic or an emergency clinic and describe what you are seeing.

Optional aids for access difficulties

Some changes are about access rather than pain itself. A cat that struggles with a barrier may benefit from equipment that removes that barrier while you and your veterinarian work out the underlying cause. These aids address the physical obstacle only. They do not diagnose or treat pain, and their suitability should be confirmed with your veterinarian.

What the available guidance does and does not establish

The guidance cited here comes mainly from VCA Animal Hospitals, International Cat Care’s carer guide, and an acute pain scale excerpt attributed to the Colorado State University Veterinary Medical Center. Publication dates were not consistently shown in the material reviewed, so check the date on any guide you read.
