Spay vs. Neuter: What’s the Difference?

Spaying usually sterilizes female dogs and cats; neutering usually sterilizes males. Learn how the procedures differ, when they are recommended, and what health and behavior changes to expect.

Spay and neuter both mean surgically sterilizing a pet, but the terms usually refer to different procedures. A spay is performed on a female dog or cat and removes the ovaries, with or without the uterus. A neuter is performed on a male and removes the testicles.

The difference matters because the operations, hormone changes, health benefits, possible risks, and ideal timing are not identical. Your veterinarian should recommend timing based on the pet’s species, sex, breed, adult size, health, and lifestyle—not a one-size-fits-all age.

Both procedures are performed under general anesthesia. They prevent accidental litters, but they are not interchangeable surgeries: a spay involves female reproductive organs, while a neuter involves the testicles.

What happens during a spay?

The traditional spay is an ovariohysterectomy. The veterinarian removes the ovaries, fallopian tubes, and uterus through an abdominal incision. Removing the ovaries stops ovarian hormone production, preventing heat cycles and pregnancy. Removing the uterus also eliminates the possibility of pyometra in the remaining uterus, a potentially life-threatening uterine infection.

Some practices perform an ovariectomy instead. This removes the ovaries but leaves the uterus in place. It still prevents reproduction and heat cycles because the ovaries—and therefore the main source of ovarian hormones—are gone. Merck Veterinary Manual notes that ovariectomy is becoming more common in dogs and may involve a smaller incision, less abdominal trauma, and less surgical or anesthesia time. Ask your veterinarian which technique they recommend and why.

What happens during a neuter?

A standard neuter is an orchiectomy, meaning removal of both testicles. The operation makes the male infertile and substantially reduces testosterone-driven reproductive behavior.

Neutering does not normally mean removing the scrotum. The testicles are removed, but the scrotal skin generally remains. The incision may be made in or just in front of the scrotum, depending on the animal and the veterinarian’s technique.

Neutering can reduce roaming, urine marking, mounting, and some male-to-male conflict. It is not a guaranteed cure for aggression or any other behavior problem. A behavior that has been practiced for years may continue after testosterone levels fall, and fear, anxiety, pain, poor socialization, and learned habits require their own treatment.

Benefits of spaying

  • No pregnancy or unwanted litters. A spayed female cannot become pregnant.
  • No heat cycles when the ovaries are removed. This can eliminate bleeding, calling or vocalizing, restlessness, and attempts to escape in search of a mate.
  • Protection from pyometra if the uterus is removed. Pyometra can progress rapidly and requires urgent veterinary treatment.
  • Lower mammary-tumor risk. The protective effect is greatest when spaying occurs before the first heat cycle, although the timing decision must be balanced against other considerations.
  • No ovarian or uterine reproductive diseases involving the removed organs.

Spaying before a first heat can offer the strongest mammary-tumor protection, but that does not mean every dog should automatically be spayed at the same age. Breed, expected adult size, orthopedic concerns, urinary incontinence risk, and the dog’s individual circumstances may affect the recommendation.

Benefits of neutering

  • Prevents testicular cancer. The testicles, where this cancer develops, are removed.
  • Reduces the risk of benign prostatic hyperplasia. This age-related enlargement of the prostate is common in intact male dogs.
  • Prevents accidental litters.
  • May reduce roaming, marking, mounting, and mating-related fighting. The earlier these behaviors are prevented, the less opportunity they have to become established.

Neutering does not automatically make a dog calm, friendly, obedient, or non-aggressive. It can reduce behaviors driven by testosterone, but it does not change every aspect of temperament.

When should a cat be spayed or neutered?

AAHA endorses sterilizing cats by five months of age. Female cats can enter their first heat as early as four months, so waiting until six months—or waiting for a first litter—is not necessary and creates a risk of pregnancy. Sterilizing before five months may also reduce urine spraying or marking in male cats.

Healthy kittens may be sterilized as young as eight weeks when they meet the clinic’s size and health requirements. A female cat can be spayed while in heat, but the procedure may require individualized planning because the reproductive tissues can be more vascular during that period.

When should a dog be spayed or neutered?

There is no universally correct age for every dog. Current AAHA guidance considers projected adult size, breed, sex, health risks, and lifestyle.

Dog Typical guidance
Small-breed male Neuter at about six months
Small-breed female Spay before the first heat, often around five to six months
Large-breed male Often neuter after growth stops, commonly around 9–15 months
Large-breed female Individualize timing, commonly within a 5–15-month window depending on disease and lifestyle risks

These are starting points, not prescriptions. A large-breed dog’s growth pattern may make the trade-off different from that of a toy breed. Your veterinarian may discuss the benefits of earlier sterilization—such as preventing pregnancy or lowering mammary-tumor risk—against possible longer-term associations with orthopedic disease, some cancers, urinary incontinence, endocrine disorders, obesity, and behavior. Research findings vary by breed, sex, size, and age; they do not predict the same outcome for every dog.

Risks and trade-offs

Spay and neuter are major surgeries performed under general anesthesia. Serious complications are uncommon overall, but risk depends on the pet’s age, body condition, size, existing illness, and the operation itself. Before scheduling surgery, ask what pre-anesthetic examination and testing the clinic recommends, what pain control will be used, and which technique will be performed.

One concern discussed most often in dogs is urinary incontinence. Merck Veterinary Manual estimates an incidence of approximately 11%–20% in large-breed spayed female dogs, while noting that incontinence can also occur in intact females, male dogs, and cats. This is one reason timing should be discussed rather than selected from a generic internet rule.

After sterilization, pets can gain weight if calories are not adjusted to their new needs. The operation does not make a pet fat by itself. Measure meals, limit treats, encourage age-appropriate activity once fully recovered, and monitor body condition with your veterinarian.

Claims that need correcting

“Every dog should be fixed at six months.”

Not necessarily. Six months may be reasonable for some small dogs, but large-breed dogs often need a different plan. Timing should be based on the individual dog.

“Neutering always calms a dog down.”

Neutering may reduce roaming, marking, mounting, and other testosterone-related behaviors. It does not reliably eliminate fear, anxiety, learned habits, or every form of aggression.

“Spaying or neutering automatically causes obesity.”

Weight gain follows excess calorie intake relative to activity. Reassess portions after surgery instead of assuming the pet must become overweight.

“A spay always removes the uterus.”

Not always. An ovariectomy removes the ovaries only. It prevents reproduction and heat cycles, but it is different from an ovariohysterectomy.

“A vasectomy or hysterectomy is the same as a neuter or spay.”

No. These alternatives may prevent reproduction while leaving hormone-producing organs intact. They may not stop heat cycles or mating-related behaviors and may not provide the same disease prevention.

“A spayed female showing heat definitely was not spayed.”

Not necessarily. Ovarian-remnant syndrome occurs when functional ovarian tissue remains after surgery. Months or years later, a pet may develop vulvar swelling, urine marking, restlessness, vocalization, or other heat-like signs. Contact your veterinarian if these signs appear.

Preparing for surgery and recovery

Follow the operating veterinarian’s instructions rather than applying a universal fasting rule. Puppies and kittens may need food withheld for a shorter period—or not at all—depending on the clinic’s protocol. Tell the veterinary team about all medications, supplements, illness, vomiting, diarrhea, or recent changes in appetite.

After surgery:

  1. Keep the pet quiet and prevent running, jumping, and rough play. Activity restriction may last up to two weeks, depending on the operation and the veterinarian’s instructions.
  2. Stop licking or chewing. A properly fitted Elizabethan collar may be necessary, even if the pet seems comfortable.
  3. Check the incision once or twice daily without applying creams or cleaning products unless instructed.
  4. Give prescribed medication exactly as directed. Never substitute human painkillers unless a veterinarian specifically tells you to.

Call the veterinary clinic promptly for increasing redness or swelling, discharge, an open incision, marked lethargy, repeated vomiting, diarrhea, or loss of appetite. Heavy bleeding, collapse, trouble breathing, or severe pain warrants emergency care.

Which procedure is right for your pet?

For most dogs and cats not intended for responsible breeding, sterilization prevents accidental litters and offers important health benefits. The practical decision is usually not whether spay and neuter are the same—they are not—but when and how the procedure should be performed.

Before booking, ask your veterinarian:

  • What age and timing do you recommend for this pet, and what factors influenced that recommendation?
  • Will you perform an ovariohysterectomy or ovariectomy?
  • What health benefits and long-term risks are most relevant to this breed and sex?
  • What should I expect for pain control, activity restriction, and follow-up?

Is spaying for females and neutering for males?

Usually, yes. Spaying generally refers to sterilizing a female dog or cat, while neutering generally refers to sterilizing a male. A spay removes the ovaries, with or without the uterus; a neuter removes both testicles.

Does neutering stop a dog from being aggressive?

Not reliably. It may reduce testosterone-related behaviors such as roaming, urine marking, mounting, and some male-to-male conflict. Aggression caused by fear, anxiety, pain, poor socialization, or learned behavior needs separate veterinary and behavioral assessment.
