The best allergy treatment for a dog depends on the cause of the itching, its severity, and the dog’s age, health, and other medicines. For diagnosed allergic or atopic dermatitis, a veterinarian may discuss prescription options such as lokivetmab, oclacitinib, ilunocitinib, corticosteroids, or cyclosporine; flea prevention, food trials, immunotherapy, and skin care address different needs. There is no single best medicine for every itchy dog.

Why the cause of itching matters more than the brand
Atopic dermatitis is a diagnosis of exclusion. Fleas, mites, bacterial or yeast infection, food allergy, ear disease, and other skin conditions can cause or worsen itching, and more than one problem may be present. The 2023 American Animal Hospital Association (AAHA) guidance recommends a diagnostic workup before assuming atopy.

Depending on the dog’s signs, the veterinarian may consider:
- Skin cytology to check for yeast, bacteria, and inflammation
- Flea combing and assessment of parasite prevention
- Skin scrapings when mites or other parasites are possible

AAHA lists lokivetmab as an option for both acute flares and long-term management and notes that onset can vary. A veterinarian can assess whether it fits the dog’s diagnosis, health, and treatment needs.
An in-clinic injection may be practical when daily oral dosing is difficult. It does not treat every cause of itching, so parasites, infection, and other conditions still need appropriate attention.
Ask your veterinarian: What findings support allergic dermatitis, and have infection, fleas, mites, and ear disease been considered?
2. Oclacitinib (Apoquel): an oral targeted option
Oclacitinib, sold as Apoquel, is a prescription oral medicine used for itch associated with allergic dermatitis and atopic dermatitis. AAHA describes it as having rapid onset and a role in acute flares and long-term management. Its official label includes a twice-daily initial schedule for up to 14 days followed by once-daily maintenance; only a veterinarian should set or change a dog’s regimen.

It is not for dogs under one year. Infection and neoplasia precautions, other illnesses, concurrent medicines, and the current official label should be reviewed with the veterinarian. Do not use leftover medicine for a new episode of itching: the cause and appropriate treatment may have changed.
3. Ilunocitinib (Zenrelia): review current safety information
Ilunocitinib, sold as Zenrelia, is a prescription oral option for allergic itch. Its suitability depends on the dog’s diagnosis and health, as well as the current prescribing information. The FDA has issued a veterinarian safety communication highlighting vaccination and infection information for Zenrelia.
Before starting or changing treatment, tell the veterinarian about recent or upcoming vaccinations, infections, other illnesses, and all medicines or supplements the dog receives. Do not rely on an older online summary in place of current label guidance.
4. Corticosteroids: rapid control for some acute flares
Corticosteroids can act rapidly and may be useful for some severe acute flares when a veterinarian determines that the benefits outweigh the risks. They can also cause common side effects, and the risks depend on the dog and treatment plan.
Use them only as prescribed. Ask how to monitor the dog and whether the medicine must be tapered; do not stop or adjust a prescribed course on your own. Infection and other underlying problems should be considered as part of the treatment plan.
5. Cyclosporine: a slower-onset option for selected dogs
Cyclosporine is an immunomodulator that may be considered for longer-term management of atopic dermatitis in selected dogs. It is not a quick substitute for a veterinarian’s assessment of a new episode of itching.

AAHA notes its slower onset and potential gastrointestinal and other effects, and that periodic monitoring may be needed. The veterinarian should review the dog’s medical history, other medicines, and the specific product information before treatment.
6. Allergen-specific immunotherapy: a long-term strategy
For confirmed environmental allergy, allergen-specific immunotherapy may be discussed as a long-term approach that changes the immune response. Allergy testing is used to identify relevant allergens for this plan, not to diagnose atopy on its own.
Immunotherapy may take months to a year to work, so the veterinarian may also recommend symptom-control medicines and trigger management while it takes effect. Ask what monitoring and follow-up the plan requires.

Flea allergy can cause intense itching, even when few or no fleas are seen.
