Three very different parasites fit the question “What bugs can live under human skin?”: the scabies mite, fly larvae that cause myiasis, and animal hookworm larvae that cause cutaneous larva migrans. They are not three kinds of insects, and “under” means something different in each condition.
Scabies mites stay in the uppermost skin layer. Some fly larvae develop in a subdermal cavity, while hookworm larvae usually migrate within the skin for a limited time rather than establishing a permanent deep infection. Symptoms can overlap, so an itchy rash or lump cannot identify the cause by itself.

First, what “under the skin” means in each infection
The phrase sounds as though insects routinely burrow deep into the body. The medical reality is more specific:
| Condition | Organism | Where it is found | What it does | Typical time described by CDC sources |
|---|---|---|---|---|
| Scabies | Sarcoptes scabiei var. hominis, a mite | Upper layer of skin, no deeper than the stratum corneum | Female mites burrow and lay eggs; itching is an immune reaction to mites, eggs and feces | Mites generally survive only two to three days away from human skin |
| Myiasis | Fly larvae (maggots), including the human botfly Dermatobia hominis | Often a wound or a subdermal cavity; some species can move into other tissues | Larvae feed in tissue and breathe through an opening in the skin | D. hominis larvae feed in a subdermal cavity for 5–10 weeks |
| Cutaneous larva migrans | Larvae of animal hookworms, such as Ancylostoma braziliense | Usually the epidermis and dermis | Larvae migrate through skin in an accidental human host and then die | Lesions can persist for several weeks before resolving |
This distinction matters: a parasite that remains in the outer skin is not doing the same thing as a larva occupying a cavity beneath it, and neither description means the organism is normally traveling throughout the body.

1. Scabies: a mite that burrows only in the outer skin
What the organism is
Scabies is caused by Sarcoptes scabiei var. hominis, the human itch mite. A mite is an arachnid, not an insect. CDC DPDx describes the mites as burrowing into the upper layer of skin but never below the stratum corneum, the outermost layer of the epidermis.
The life cycle includes egg, larva, nymph and adult stages. Female mites create short burrows and lay eggs in them. The rash and itching are largely the body’s reaction to the mite, its eggs and its feces rather than evidence that an insect is moving through deep tissue.
Typical signs
- Intense itching, often worse at night.
- An itchy, papular rash.
- Occasionally visible burrows in the outer skin.
Those findings are suggestive, not diagnostic. Eczema, allergic reactions and other skin problems can also itch, so a clinician may need to examine the distribution and appearance of the rash.
Where it occurs and how long mites persist off a person
Scabies occurs worldwide. CDC says mites generally do not survive more than two to three days away from human skin. That survival fact does not mean every itchy household rash is scabies; it explains why the mite is adapted to human skin and why diagnosis should be based on the clinical picture rather than on finding an insect in the home.

Treatment requires a prescribed scabicide
CDC states that scabies is treated with prescribed scabicides. Because treatment choice and application depend on the patient and the clinical diagnosis, do not substitute household chemicals or unverified skin products. A clinician or public-health service can tell you which medicine to use and how to manage close contacts or the living environment when necessary.
2. Myiasis: fly larvae developing in human tissue
What myiasis means
Myiasis is a parasitic infection with fly larvae in human tissue. In plain language, the “bug” is a maggot, not an adult fly living under the skin. CDC describes the human botfly, Dermatobia hominis, as a primary human botfly. Its newly emerged larva penetrates tissue and feeds in a subdermal cavity.
How a botfly larva lives under the skin
A D. hominis larva can remain in a subdermal cavity for 5–10 weeks. It breathes through a hole in the skin, so a person may notice a small opening associated with a moving or painful lump. The duration is a life-cycle observation for this botfly, not a prediction for every case of myiasis.

Geography and exposure
CDC’s public overview says myiasis typically occurs in tropical and subtropical areas and is uncommon in the United States. The DPDx entry specifically places D. hominis in Mexico and Central and South America; that distribution applies to this species, not to every fly that can cause myiasis.
CDC also says myiasis is not spread person to person. Open or untreated wounds increase risk in affected areas. A travel history, a persistent wound or a new tissue lump can therefore be important information for a healthcare professional.
Why suspected myiasis needs medical assessment
Some larvae stay localized, while others can damage tissue or move beyond the subdermis. Do not try to cut, squeeze or pull a larva out yourself. Removal and any additional treatment should be planned by a clinician who can identify the type and make sure all larvae are addressed.
3. Cutaneous larva migrans: hookworm larvae moving through skin
Humans are accidental hosts
Cutaneous larva migrans is a skin-migration syndrome caused by larvae of animal hookworms. The larvae can penetrate exposed human skin, but humans are not their normal host. CDC says larvae of most animal hookworm species cannot penetrate beyond the dermis in people.

What the migration looks like
Ancylostoma braziliense is one example. Its larvae migrate in the epidermis, producing lesions that can advance over several weeks before the larvae die. This is not the same process as a scabies mite making a burrow, and it is not a botfly larva occupying a subdermal cavity.
CDC notes that most cases resolve without treatment, although severe cases may require medication. The path of a spreading, itchy lesion can be a useful clue for a clinician, but appearance alone cannot confirm the cause.

When a skin parasite warrants prompt or urgent care
Arrange medical care for an unexplained persistent lesion
- Intense nighttime itching or a new papular rash that does not have an obvious cause.
- A lump with a central opening, a sensation of movement, or a wound that does not heal.
- An itchy track or lesion that appears to migrate over days or weeks.
- Symptoms after travel or residence in a region where myiasis is known to occur.
Seek urgent attention for a rapidly worsening wound
CDC’s clinical information on New World screwworm describes rapidly worsening, foul-smelling wounds and significant pain as common features. Suspected cases require physical removal of all larvae and immediate reporting to local public-health authorities. Those warnings concern a potentially destructive form of myiasis, not every ordinary itchy rash.
What diagnosis and treatment usually involve
History and skin examination
A clinician will usually start with the appearance and distribution of the lesion, timing, itching or pain, travel and exposure history, and the presence of wounds. The distinction between an outer-skin mite, a cavity-dwelling fly larva and a migrating hookworm larva often comes from this combination rather than from one symptom.
Condition-specific management
- Scabies: Use the prescribed scabicide exactly as directed. The diagnosis and treatment plan should account for the patient’s age, health and close-contact situation.
- Myiasis: Clinical removal is required when larvae are present. The clinician may also assess tissue damage and whether the species could have moved beyond the subdermis.
- Cutaneous larva migrans: Many cases resolve spontaneously; medication may be used when disease is severe or persistent.
Do not apply fuel, pesticides, caustic chemicals or improvised instruments to a suspected infestation. Such actions can injure skin and make a wound harder to assess.

Reducing exposure without confusing prevention with treatment
For scabies concerns
Because scabies mites are adapted to human skin and generally survive only two to three days away from it, a suspected case should be handled through a clinician’s treatment and contact-management instructions rather than by attempting to disinfect the body with harsh products.
For myiasis concerns
In tropical or subtropical areas, protect open wounds and seek care for wounds that develop a new lump, opening, odor or increasing pain. Myiasis is not spread person to person, but untreated wounds can provide an entry point for certain flies.
For hookworm-related skin migration
Cutaneous larva migrans occurs when animal hookworm larvae penetrate exposed skin. If you develop a progressive itchy track after an environmental exposure, have it evaluated rather than waiting for the lesion to become extensive.
Common misconceptions to avoid
- “All three are insects.” Scabies is caused by a mite; myiasis involves fly larvae; cutaneous larva migrans involves animal hookworm larvae.
- “Anything under the skin is deep in the body.” Scabies remains above the stratum corneum, and most animal hookworm larvae cannot pass beyond the dermis in humans.
- “A moving rash proves a worm.” Symptoms overlap, and diagnosis requires clinical assessment.
- “Parasites routinely spread throughout the body.” The CDC descriptions distinguish localized skin burrows, subdermal cavities and skin-limited migration.
- “A home remedy can remove a larva safely.” Suspected larvae should be managed medically, especially when a wound is painful, foul-smelling or worsening quickly.
What pet owners should take from the evidence
The hookworm larvae involved in cutaneous larva migrans normally infect animals, while scabies and myiasis involve different organisms and transmission patterns. That taxonomy matters: a skin problem in a person cannot be diagnosed simply by assuming it came from a household pet or by using an animal parasite product on human skin. If a person has a persistent rash, lump or migrating lesion, the safest next step is a human medical evaluation and a clear account of travel, wounds and possible environmental exposure.
