Vaginal discharge in a cat is a sign that something is happening in the reproductive tract, not a diagnosis in itself. In an unspayed female, the most important possibility to rule out is pyometra, a bacterial infection of the uterus that can become life-threatening. Discharge can also come from vaginitis or from the discharge that follows giving birth. Because the cause cannot be reliably identified by how the discharge looks, any discharge in an intact female that comes with illness should prompt a call to your veterinarian promptly.
Is vaginal discharge normal in cats?
Discharge is not something to expect from a healthy, spayed adult cat, and the veterinary references used for this article do not describe ongoing discharge as a normal finding in a non-pregnant adult female. The one context where discharge is part of normal recovery is after giving birth. Merck Veterinary Manual’s guidance on metritis describes normal postpartum discharge (lochia) and notes that it should resolve, while an increasing amount or a foul odor needs further examination. That passage is written for small animals in general, and it does not provide a cat-specific timeline, so owners should not treat a particular number of days as a confirmed feline standard. If a queen has recently given birth and her discharge is heavy, changing, or smells bad, that is a reason to call the veterinarian.
Common causes of discharge in cats
Three conditions account for most of the situations in which a veterinarian will evaluate vaginal discharge. They differ in spay status, timing, and how sick the cat is, which is why the same discharge can mean very different things.
| Feature | Pyometra | Vaginitis | Postpartum discharge or postpartum metritis |
|---|---|---|---|
| Cat’s reproductive status | Unspayed (intact) female | Described for small animals; adult vaginitis is described as rare in queens | Recently given birth |
| Typical discharge | Pus, sometimes with blood, when the cervix is open; none visible when the cervix is closed | Vulvar discharge, licking, or spotting | Normal lochia can occur; increasing amount or foul odor needs examination |
| Systemic illness | Can be severe, including lethargy, poor appetite, vomiting, and an enlarged abdomen | Not stated in the reference material as a typical feature | Not stated in the reference material for cats specifically |
| Main concern | Bacterial infection of the uterus that can progress rapidly | Possible underlying urinary, structural, or irritation problems | Distinguishing normal recovery from metritis |
Pyometra
Pyometra is a bacterial infection of the uterus that develops in unspayed cats as a result of hormonal changes in the reproductive cycle. VCA Animal Hospitals notes that the illness can occur after a heat cycle. Merck Veterinary Manual states that pyometra is less common in cats than in dogs, though the cat-owner guidance it cites does not give a percentage, so no prevalence figure should be assumed.

- SCOPE OF APPLICATION FOR CAT CONE M size (neck circumference 6.5" - 10.5" diameter 11.5") is suitable for pets weighing less than 6 - 11 lbs. Measure the pet's neck circumference before ordering. The cone features an adjustable elastic for a secure fit
- NO IMPACT ON DAILY LIFE If your pet struggles to eat or drink with standard plastic cones, this e-collar is an ideal solution. Your pet can eat, drink, and play normally. For foot wounds, consider a larger size if neck circumference allows
- NOT JUST RECOVERY AFTER SURGERY Pets often lick their wounds. We increased the diameter of the cat cone to prevent this, even for lower limb wounds, promoting better recovery. It is also suitable for in vitro deworming, nail trimming, and grooming
- SOFT WATERPROOF MATERIAL This e-collar is made of waterproof PP cotton, keeping your pet's wound dry for better recovery. The soft material prevents scratches to pets and furniture. The smooth outer fabric ensures comfort, even in summer
- FESTIVAL DECORATION This cat cone can be used as a soft pillow or Christmas or Halloween decoration. The neck rope has been designed with an open knot, and if the rope is too long or there is a risk of the pet breaking free, it can be knotted
How it appears depends on whether the cervix is open. With an open cervix, pus, often with blood, drains through the vagina, and the owner may see it on the cat’s bedding or fur. With a closed cervix, there may be no discharge at all. In that case the uterus can enlarge with fluid and infection, and toxins can make the cat seriously ill. This is why the absence of discharge does not rule out pyometra. The VCA guidance lists increased drinking and urination, vomiting, lethargy, poor appetite, and abdominal enlargement among its possible signs.
Vaginitis
Vaginitis, inflammation of the vagina, is described by Merck Veterinary Manual as rare in adult queens. The reference material lists the predisposing factors it describes for small animals in general: congenital or structural abnormalities, urine or fecal contamination, irritation, trauma, a foreign body, or neoplasia. Signs can include vulvar discharge, licking at the vulva, spotting, and frequent urination. Because several of these factors involve the urinary tract or the surrounding anatomy, a veterinarian will look beyond the vagina itself rather than treating the discharge alone.
Discharge after giving birth
Discharge after parturition can be normal lochia, but postpartum metritis and other conditions must be distinguished from it. The distinction rests on how much discharge there is, how it changes over time, what it smells like, and how the mother is doing overall. A queen that is weak, not eating, or running a fever needs veterinary attention regardless of how the discharge looks.

What your veterinarian will check
A veterinarian cannot identify the cause from appearance alone. The workup depends on what is suspected.

For suspected pyometra
- History and physical examination, including the abdomen
- Assessment of the discharge, if present
- Blood tests and urinalysis
- Abdominal radiographs (X-rays) or ultrasound
Ultrasound can show an enlarged, fluid-filled uterus and helps distinguish it from pregnancy, which is important because the two can both cause abdominal enlargement.
For suspected vaginitis
- Urinalysis
- Vaginal examination
- Microscopic examination of the discharge
- Culture and susceptibility testing of vaginal and urine samples
The veterinarian will also look for any underlying urinary or structural problem, since treating the infection alone may not resolve the cause.

Surgery for most pyometra cases
For most cats with pyometra, the preferred treatment is ovariohysterectomy, surgical removal of the ovaries and uterus. Before surgery, a cat that is dehydrated or unwell may need stabilization with intravenous fluids and antibiotics. The specific timing and plan are decided by the veterinarian based on the cat’s condition.
Medical management in selected breeding cats
Medical treatment may be considered for some breeding cats under veterinary supervision. The reference material describes it as less reliable than surgery and as carrying risks, so it is not a substitute for surgery in a cat that is not being bred and should only be discussed with a veterinarian.
Treating vaginitis
Vaginitis treatment focuses on the underlying cause. Antimicrobials are selected based on culture and susceptibility results when they are indicated, rather than chosen by guesswork. Any urinary or structural problem the veterinarian finds is addressed alongside the infection.

Prevention
Spaying is the usual way to prevent pyometra in cats that are not intended for breeding. Spaying removes the hormonal cycles that underlie the condition. Merck Veterinary Manual states that prevention of estrus is typically accomplished by spaying.
Hormonal medication to suppress estrus is not a risk-free alternative. Merck cautions that medical suppression can have adverse effects, including uterine inflammation or infection. Any breeding decision or hormonal medication should be discussed with your veterinarian before it is used.
Keeping the cat clean or using hygiene products does not prevent uterine infection, because pyometra arises from hormonal changes inside the uterus rather than from the outside of the body.
What to note before the appointment
Writing down a few details helps your veterinarian narrow the cause. These notes support the examination and do not replace it.
