Refer an eye case when vision or comfort is at meaningful risk, disease is progressing, treatment is failing, or a specialist procedure could change the outcome. A referral is not an admission that primary care has failed. Many ocular problems can be diagnosed and treated in general practice, while glaucoma, worsening or deepening corneal ulcers, retinal disease, severe injury, ocular cancer and selected cataracts may need prompt specialist input.

This guide translates current advice from the American College of Veterinary Ophthalmologists (ACVO), the 2025 AAHA Referral Guidelines and ophthalmologist Ron Ofri’s 2025 AAHA interview into a practical workflow. The interview reflects expert clinical opinion, not a universal protocol or outcomes study; apply it alongside examination findings, local emergency arrangements and the patient’s response to treatment.

When an eye case should leave general practice

ACVO’s referral guidance starts with a useful balance: many eye conditions can be managed by a general practitioner, but specialist expertise benefits patients when the diagnosis is uncertain, the disease is time-sensitive or advanced diagnostics and procedures are likely to matter. ACVO’s examples include perceived cataracts, glaucoma, retinal disease, severe ocular injury and ocular cancer. It also advises prompt consultation when a condition is not responding to recommended therapy, vision is declining despite treatment, or a corneal ulcer is not responding.

Use the following questions during triage rather than treating any one sign as an automatic referral rule:

  • Is vision already reduced, or could delay cause irreversible loss?
  • Is the eye becoming more painful, inflamed, cloudy or structurally unstable?
  • Is the problem stable and responding, or progressing despite appropriate treatment?
  • Can the practice perform the diagnostics and monitoring needed to manage it safely?
  • Would a specialist-only procedure, imaging modality or surgical decision change the prognosis?
  • Can the client realistically provide the medication, surgery, transport and repeated rechecks required?

ACVO notes that clients in the United States may contact an ophthalmologist directly, although a coordinated relationship with the primary-care veterinarian remains valuable. Referral requirements vary by clinic and geography, so confirm whether a formal referral, records transfer or emergency phone discussion is required.

In the United States, a Diplomate of the American College of Veterinary Ophthalmologists (DACVO) has completed veterinary school, at least 12 months of full-time clinical practice, a minimum three-year ophthalmology residency and the American Board of Veterinary Ophthalmology certifying examination. Outside the United States, check the relevant national or regional specialty college.

Ofri summarized the central judgment this way: “The biggest mistake he sees general practitioners making is just that: not knowing when it’s time to refer.” His comment, published in the AAHA interview on March 27, 2025, is practical advice rather than a formal referral threshold.

Urgent patterns: glaucoma and corneal ulcers

Glaucoma

Ofri recommends immediate referral for glaucoma because vision may be lost and the eye can become painfully blind. The urgency is tied to the risk of permanent damage, not simply to an elevated pressure reading in isolation. If glaucoma is suspected, begin the examination and stabilization that are appropriate to your setting, contact the receiving ophthalmologist or emergency service, and arrange transfer without waiting to see whether routine treatment eventually works.

Tell the owner why time matters: the referral is intended to protect both comfort and vision, and the specialist may need to make decisions that cannot be reproduced by watchful waiting. Document the time of onset, measured intraocular pressures, menace and dazzle responses where interpretable, pupil findings, medications already given and any contraindications or concurrent disease.

Corneal ulcer

Ofri says most corneal ulcers can be treated successfully by general practitioners. The risk profile changes when an ulcer is contaminated, deepening, enlarging or otherwise progressing. He advises referral as an emergency for worsening or deepening disease and consultation when an ulcer has not begun to heal within a few days. “A few days” is interview language, not a universal deadline: reassess the individual eye, the depth and location of the defect, the patient’s behavior, and the reliability of home care.

Arrange urgent specialist care when you see signs such as:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Increasing stromal depth, malacia or concern for impending perforation.
  • Progression despite the prescribed treatment plan.
  • Heavy contamination, a foreign body or severe injury.
  • Marked pain, worsening vision or inability of the owner to administer treatment.
  • No convincing healing trend at the planned recheck.

Send the fluorescein findings, ulcer dimensions and location, depth estimate, eyelid and tear-film findings, suspected cause, analgesia, topical and systemic drugs, protective measures and the exact timing of each dose. Do not delay transfer while repeatedly changing medications in a rapidly worsening eye.

Cataracts: deciding when to consult and how to counsel

Opacity versus a progressing cataract

A small, focal lens opacity is not automatically an immediate surgical case. Ofri distinguishes an early, limited opacity from a cataract that is progressing. When the opacity is enlarging, affecting vision or creating concern for lens-related inflammation, earlier referral can make assessment and surgery easier and may reduce postoperative complications. The appropriate timing depends on the patient’s ocular examination, visual function, systemic health and the specialist’s assessment of surgical candidacy.

Explain that referral answers several questions at once: Is the opacity truly a cataract? Is useful vision still present? Is there inflammation, glaucoma or retinal disease? Would surgery provide a reasonable benefit, and can the family complete the aftercare? Online supplements marketed as cataract cures or reversals should not be presented as proven therapy; the AAHA interview provides no evidence that such products work.

Surgery is a treatment pathway, not a single event

Cataract surgery requires honest discussion of the financial commitment, anesthesia and diagnostics, intensive postoperative medication, repeated examinations and the possibility that complications or later disease may affect vision. A specialist cannot always provide a complete cost estimate before examination. You can explain the known referral fee and likely categories of testing, while clearly identifying what remains uncertain.

Ofri reported his own estimates in the AAHA interview as “coming out of surgery, 95% of dogs see after cataract surgery” and “a year out? We may be down to 70% and even fewer further down the road.” These are his interview estimates, not a published study rate or a promise for an individual dog. He attributed part of the decline to missed rechecks and postoperative medication. Use the figures only with that attribution and qualification; do not turn them into a guaranteed benchmark.

Uveitis: a sign that needs a cause

Ofri’s key warning is: “The most important thing that you should keep in mind for uveitis is that really, it’s clinical sign. It’s not a diagnosis.” In practice, that means treating pain and inflammation appropriately while asking why the eye is inflamed. Consider the history, examination, species and breed context, travel and infectious exposure, medications, trauma, neoplasia and other systemic findings when deciding which systemic evaluation is justified.

Some cases can be managed in general practice with close monitoring. Refer when the inflammation cannot be controlled, the diagnosis remains uncertain, vision is threatened, the patient has recurrent disease or systemic illness requires coordinated specialty care. Include the timeline of signs, both-eye examination, intraocular pressure measurements, fundic findings if visible, laboratory results, imaging, medications and response or adverse effects.

What modern veterinary ophthalmology can do

Ofri describes substantial progress in cataract care over the preceding 20–30 years: “Our instrumentation has improved, our pre-operative examination and screening of patients has improved, and our techniques have improved.” For a general practice, the practical implication is not to promise a procedure but to recognize when a referral could offer options that were previously less predictable or unavailable.

  • Glaucoma management: Ofri reports that patients can maintain vision longer than in the past, although urgency and long-term monitoring remain essential.
  • Retinal detachment: Ofri describes retinal reattachment surgery as an option that was previously unavailable for dogs in his account.

Retinal degeneration remains an area where promising science should not be confused with routine care. Ofri identifies gene therapy, stem-cell therapy, artificial retinas and retinal prostheses as research directions and future challenges. The AAHA interview does not establish a specific product, trial, indication or broad availability for these approaches, so they should not be presented to owners as established cures or generally accessible treatments.

Choosing consultation versus hands-on transfer

The 2025 AAHA Referral Guidelines describe a continuum rather than a single model. You may need a professional-to-professional consultation while retaining the case, or a hands-on transfer in which the specialist assumes examination and treatment. Decide explicitly which model fits the risk, the practice’s capabilities and the client’s circumstances.

Situation Reasonable starting point Escalate when
Stable problem responding to treatment and within practice capability Continue primary-care management with a defined recheck plan; seek advice if uncertainty remains. Progression, treatment failure, vision decline or new pain develops.
Suspected glaucoma or rapidly worsening vision Immediate specialist or emergency referral. Any delay, worsening comfort or inability to provide required monitoring.
Corneal ulcer without deepening and showing an early healing trend Manage with close rechecks and clear owner instructions. Contamination, increasing depth, progression or no healing within a few days.
Progressing cataract Early ophthalmology consultation to assess candidacy and timing. Vision loss, lens-related inflammation, glaucoma or uncertainty about the diagnosis.
Uveitis that is controlled and explained Continue treatment while investigating an appropriate systemic cause. Uncontrolled, recurrent, vision-threatening or diagnostically uncertain disease.

Build a referral that the specialist can use

A complete transfer reduces duplicated work and helps the client understand what will happen next. AAHA recommends communicating completed and pending tests, sharing records, setting expectations about possible repeat diagnostics and costs, agreeing on contacts and follow-up, and keeping the primary-care team involved. A secure web-based shared portal can support coordination when available.

  1. Stabilize and document. Record visual responses, pain, ocular examination findings, intraocular pressure, fluorescein or other tests performed, photographs when useful, treatments given and the patient’s response. Note the time and dose of the last medication.
  2. Send the clinical question. State whether you need a diagnosis, an urgency decision, a surgical candidacy assessment, advice on medical management or complete hands-on care. Include the owner’s goals and constraints.
  3. Transmit records before arrival. Forward the history, examination, laboratory and imaging reports, medication list, allergies or adverse reactions, and pending results. Identify tests that were not performed and why.
  4. Prepare the client for uncertainty. Explain the known referral fee, likely categories of examination or testing, possible repeat diagnostics and the fact that a definitive treatment estimate may not be possible before specialist assessment.
  5. Agree on communication. Name the primary contact at each practice, decide how urgent changes will be reported, and clarify who will prescribe, monitor and answer questions after the visit.
  6. Plan follow-up before discharge. Obtain the specialist’s written plan, medication schedule, recheck timing and warning signs. Schedule the primary-care follow-up when appropriate rather than leaving responsibility undefined.

Remain involved even when the specialist performs the procedure. The primary-care team often knows the patient’s systemic history, can monitor medication tolerance and can help the owner complete long-term treatment. AAHA’s 2025 guidance reports a Collaborative Care Coalition finding that client perceptions were six times more likely to improve after a referral; the page does not provide the study’s sample size or detailed methods, so treat this as a reported association, not a guaranteed result.

Practical owner conversations

Explain urgency without alarmism

Use a specific risk statement: “This eye may be losing vision or becoming more painful, and an ophthalmologist has equipment and procedures that could change the outcome. I recommend contacting the referral service today.” Avoid implying that referral guarantees sight or surgery.

Set expectations for aftercare

For cataract surgery and many inflammatory or corneal conditions, the outcome depends on medication administration and rechecks as well as the procedure itself. Ask who will give each dose, whether an Elizabethan collar or other protection is feasible, how transport will be arranged and what the family will do if the eye worsens overnight.

Address access barriers early

If travel, cost or appointment availability is limiting, ask the specialist about a professional consultation, teleconsultation where legally and clinically appropriate, emergency triage or a staged plan. Document the alternatives and the safety-net instructions. Do not let a cost discussion obscure an urgent risk to vision or comfort.

Useful preparation and continuing education

Ofri recommends textbooks, pictures and descriptions as useful starting points for general practitioners. A current small-animal veterinary ophthalmology textbook or atlas can support pattern recognition, but it does not replace examination, specialist advice or local emergency protocols. Keep the practice’s referral contacts, transfer template, medication list and after-hours arrangements readily accessible.
