Broad, full cheeks can be part of a mature male cat’s normal facial appearance and are associated with testosterone. They do not prove that a cat is intact or indicate a particular testosterone level. A new, localized, painful, or growing swelling is different from a stable broad face and should be assessed by a veterinarian.
What are tomcat jowls?
“Tomcat jowls,” “stud cheeks,” and “shields” are informal terms for the broad, full cheeks that some mature male cats develop. A full face can be a normal feature, particularly in an intact male, but cats vary in how prominent their cheeks look.
Face shape is only one feature that may be considered when assessing a cat’s neuter status. A cat’s appearance alone cannot confirm whether he has been neutered or diagnose a health problem.
Why do intact male cats develop big cheeks?
Prominent cheeks are associated with male sexual maturity and testosterone. The testicles produce testosterone, and neutering reduces testosterone-driven effects. However, cheek size is not a validated measure of an individual cat’s hormone level.

Cheek rubbing is a separate behavior: cats deposit scent from glands in their cheeks when they rub their faces on people or objects. This scent-marking behavior does not explain whether a cat’s cheeks are unusually large.
Do big cheeks prove a male cat is not neutered?
No. A broad face is one clue that may be considered when assessing neuter status, not proof. Facial appearance varies between cats, so a cat’s cheeks alone cannot establish whether he is intact or neutered.
If you are unsure whether a cat has been neutered, ask a veterinarian to assess him. Physical examination and, when appropriate, further testing can help clarify his status.
When are big cheeks probably normal?
A stable, broadly full face—particularly in an intact male—is compatible with normal male facial development. Consider the history of the appearance: whether it has been present for a while, whether it is broad or localized, whether it is changing, and whether the cat seems uncomfortable or has difficulty eating. These observations can help you decide whether to contact a veterinarian, but they are not a home diagnosis.

When a large cheek may be a medical problem
A new or changing lump should not automatically be assumed to be a normal facial feature. Oral swellings are not normal and may result from trauma, infection, fluid accumulation, or tumors. Contact a veterinarian if you notice a new swelling, especially if it is increasing, painful, or affecting eating.
VCA Animal Hospitals advises: “If you see a swelling in your cat’s mouth, book an appointment to have your veterinarian examine your cat.” Do not press on a swelling or try to treat it at home.
Routine preventive cat dental care is separate from investigating a new swelling. Dental care products cannot determine the cause of a lump or replace a veterinary examination.
When is facial swelling an emergency?
Contact a veterinarian promptly about a new or increasing swelling, pain, or difficulty eating. If your cat appears seriously unwell or has difficulty breathing or swallowing, seek urgent veterinary care.

What should you do if you notice a big cheek?
- Consider the timing. Note whether the broad facial appearance has been stable or whether you have noticed a new change.
- Look at the pattern. Note whether the face looks broadly full or whether there is a localized swelling, and whether it is changing.
- Observe your cat. Watch for signs of pain, difficulty eating, or other changes that concern you.
- Do not press or treat the area. A veterinarian should examine a suspected swelling; do not try to diagnose or treat it at home.
- Ask a veterinarian about neuter status. Cheek size alone cannot confirm whether a cat is intact or neutered.
Does neutering make tomcat cheeks go away?
Neutering removes the testicles, which produce testosterone, and reduces testosterone-driven effects. The research sources do not establish how much or how quickly a cat’s cheeks may change after neutering, so a specific change should not be promised.
VCA Animal Hospitals reports that male cats typically reach puberty between four and twelve months. Neutering is generally done before puberty, but timing should be discussed with a veterinarian, who can consider the individual cat’s health, behavior, and environment.
Common misunderstandings about male cat cheeks
“Big cheeks prove a cat has high testosterone.”
No. Prominent cheeks are associated with male sexual maturity, but cheek size does not measure an individual cat’s testosterone level.
“Big cheeks prove a cat is intact.”
No. Face shape is one feature that may be considered when assessing neuter status, but appearance alone does not prove it.

“Cheek rubbing means a cat has unusually large cheeks.”
No. Cheek rubbing is scent marking: cats deposit scent from cheek glands when they rub their faces on people or objects. It is separate from cheek size.
“A new lump is just a normal jowl.”
Do not assume that. A new or changing swelling should be assessed by a veterinarian, particularly if it is painful or affects eating.

Why does my cat have one swollen cheek?
A new, localized swelling may have a health-related cause. Oral swellings are not normal; contact a veterinarian to have it examined, especially if it is growing, painful, or affecting eating.
When do male cats reach puberty?
VCA Animal Hospitals reports that male cats typically reach puberty between four and twelve months. Discuss neutering timing with your veterinarian, who can consider your cat’s individual circumstances.
The Bottom Line
Broad, stable cheeks can be a normal, testosterone-associated feature of a mature male cat, but they do not prove neuter status or measure testosterone. A new, localized, painful, or growing swelling is different from a stable broad face and should be examined by a veterinarian.
