It is not one disease with one cause. Straining in the litter box can come from several different problems, including feline idiopathic cystitis (FIC), urinary stones, infection, or a urethral blockage, and each one needs a different response. Only a veterinary examination and testing can sort out which is happening in a particular cat.
Why FLUTD is a group of problems rather than a single diagnosis
The term describes what goes wrong with the lower urinary tract, not why. The 2025 iCatCare consensus guideline, as reflected in its published abstract, stresses that these signs can arise from varied underlying causes and need investigation rather than assumption. Because the outward signs look alike from one cat to the next, a cat with a sore bladder and a cat with a stone can behave almost identically at home.
Signs caregivers commonly notice
The American Veterinary Medical Association (AVMA) lists the following as typical signs of lower urinary tract trouble:
- Difficulty or pain when urinating, including straining
- Passing small amounts of urine more often than usual
- Vocalizing, such as crying, while in the litter box
- Blood in the urine
- Excessive licking of the genital area
- Urinating outside the litter box
None of these signs points to a specific cause on its own. A cat that urinates in the bathtub may have FIC, a stone, an infection, or a blockage, and only testing will distinguish them.

Recognizing an emergency
The most important distinction is between a cat that is uncomfortable and a cat that may be blocked. A cat that strains repeatedly, produces little or no urine, and becomes increasingly distressed may have a urethral obstruction. The AVMA states: “Urethral obstruction is an emergency and requires immediate veterinary care.” This is a situation to act on immediately, not to monitor at home. Do not wait to see whether it resolves, and do not try home remedies first. Contact a veterinarian or an emergency animal hospital right away.
Common causes
Several conditions produce the same lower urinary signs. The most frequently named are the following, along with infection and other disorders that can also cause them.
Feline idiopathic cystitis (FIC)
FIC is a likely consideration in young adult and mature cats with lower urinary signs, but it is a diagnosis of exclusion. The veterinarian must first rule out stones, infection, and obstruction. The AAHA/AAFP guideline notes that perceived stressors can worsen FIC, which is why stress management is a central part of its care.

Urinary stones (uroliths)
Stones can irritate the bladder and urethra and may cause a blockage. Their type determines treatment. Struvite stones may be dissolved with a special diet prescribed by a veterinarian. Calcium oxalate stones cannot be dissolved with diet and may need to be removed.
Urinary tract infection
Bacterial infection can produce urinary signs, and it is one of the conditions a veterinarian checks for with urine testing and culture. Infection and FIC can look similar, so an infection should not be assumed or ruled out from symptoms alone.
Urethral obstruction
A blockage, most often from crystals, stones, or debris, prevents urine from leaving the body. It is the most time-sensitive of the causes described here and is treated as an emergency whatever its underlying trigger.

How the diagnosis is made
A typical evaluation follows a sequence, though the exact steps depend on the cat and the findings:
- History. The veterinarian asks when the signs started, how often the cat is using the litter box, what the urine looks like, and whether the cat’s routine, household, or diet has recently changed.
- Physical examination. The vet checks the bladder and abdomen and assesses how distressed the cat is.
- Urinalysis. Testing the urine is a core part of the initial workup.
- Additional tests as needed. Depending on the results, these may include a urine culture, bloodwork, and imaging such as X-rays or ultrasound to look for stones.
Causes can differ between episodes, and a cat can have more than one problem at a time. Recurring signs therefore still warrant a new veterinary assessment rather than a repeat of the last treatment.
Treatment depends on the cause
Treatment is set by what the workup finds. Urethral obstruction requires urgent veterinary treatment before anything else. For confirmed struvite stones, a veterinarian may prescribe a stone-dissolving diet; this is a cause-specific treatment, not a general treatment for FLUTD. For calcium oxalate stones, the veterinarian will discuss removal rather than dietary dissolution. Confirmed infection is treated according to the culture results.

Supportive care for FIC and everyday management
For FIC, the AVMA describes environmental modification as part of management. The aim is to reduce stressors and help the cat feel secure. Practical measures include the following:
- Keep fresh water available at all times
- Keep litter boxes clean and place them in suitable, quiet locations
- Maintain a stable routine and environment
- Provide enrichment, such as play, climbing spaces, and hiding places
These measures support the cat but do not replace diagnosis. A cat that may be obstructed needs emergency care, not a change to its environment.
What to avoid
Do not start supplements or urinary acidifiers without veterinary direction. Cornell University’s educational material cautions that excessive acidification can cause metabolic and mineral problems. Marketed urinary-health foods are also not a substitute for diagnosis. Cornell notes that such foods have not been shown to reduce the incidence of feline lower urinary tract signs themselves, so a cat with symptoms should be examined rather than simply switched to a product.

The guidance above draws on several sources with different publication dates. The AVMA client brochure is dated 2024. The 2025 iCatCare consensus guideline was reviewed only through its published abstract, so the statements attributed to it here are limited to that summary. The AAHA/AAFP guideline page is dated February 18, 2021. Cornell’s educational page was last updated in October 2016, so where it overlaps with newer guidance, the AVMA and iCatCare statements are the more current framing. No prevalence figure for FLUTD is given here, because the sources reviewed did not identify a named, dated prevalence study.
Use this article as an orientation to what FLUTD means. Any cat showing straining, blood in the urine, or reduced urine output should be seen by a veterinarian promptly, and a cat that is straining without producing urine should be treated as an emergency.
