Before leaving with an adopted dog or cat, ask the shelter or rescue for copies of the complete veterinary and behavioral file it has available—not just a vaccination card. Collect identification and microchip details, vaccine and rabies documentation, spay/neuter information, exam findings, test and procedure reports, medication and parasite-treatment history, known conditions, behavior notes, and follow-up instructions. Keep the adoption and ownership papers with the records, make a digital backup, and take the packet to your new veterinarian.
Why the full record packet matters
A vaccine card alone may not tell your new veterinarian what tests have already been done, whether a treatment is ongoing, or what follow-up the shelter recommended. The Association of Shelter Veterinarians’ standards describe a medical record as including identifying information and dated findings, vaccinations, diagnostic-test results, procedures, and treatments. In practical terms, the aim is to give the next veterinarian enough history to understand what is known, what is uncertain, and what may need attention.

Ask for the records that exist, including behavioral notes, and ask whether anything is still pending. A shelter may have incomplete history, especially for an animal whose earlier life is unknown. A blank or missing record is not proof that a vaccine, test, or surgery never happened; it means the information was not provided in the packet. Make that distinction clear to the clinic rather than guessing.

Records to request before you leave
| Record area | What to collect | Why it helps |
|---|---|---|
| Identity | Shelter or animal ID, species, age or estimated age, sex, reproductive status, and microchip number | Helps match documents and results to the correct pet. |
| Vaccines and rabies | Vaccine names, administration dates, product or certificate details when available, rabies tag or certificate details, and any listed next-due date | Lets the veterinarian assess documented status and plan follow-up. |
| Microchip | Chip number, implantation or scan information, registry name, and transfer instructions | Knowing the number is not enough if the registry still has someone else’s contact details. |
| Surgery and procedures | Spay/neuter status and date, operative or discharge notes, dental or other procedure records, and any pending-surgery voucher | Clarifies what has been done and what remains to be arranged. |
| Exams and conditions | Dated exam findings, weight or body-condition notes, diagnoses, injuries, allergies, and restrictions | Provides context for changes noticed after adoption and for care planning. |
| Tests | Available laboratory, parasite, heartworm, FeLV/FIV, imaging, dental, and other diagnostic reports | Results and their dates can affect whether further evaluation is appropriate. |
| Medicines and parasite control | Medication name, dose, route, schedule, start and stop dates, refills, plus deworming, flea, tick, or heartworm-treatment details | Prevents uncertainty about what the pet has received or may still be taking. |
| Behavior and background | Available behavior observations, bite history, known triggers, and prior-home or foster notes | Helps the household and veterinarian discuss safety and support needs. |
| Adoption and ownership | Signed adoption contract, proof of ownership, animal ID, and shelter contact information | Keeps identity and ownership documentation together with medical records. |
Vaccination and rabies documentation
Request a dated history for each recorded vaccine, including the product or certificate information when available and any next-due date shown by the shelter. Ask separately for rabies documentation: a certificate or tag details, tag number if applicable, administration date, expiration or next-due information, and the administering veterinarian or clinic. The UK government’s guidance names vaccination certificates among medical records to provide; the American Veterinary Medical Association (AVMA) also identifies rabies certificates as records to retain when applicable.

Do not treat a general note such as “vaccinated” as interchangeable with a certificate or dated record. If the packet is missing details, tell the new veterinarian exactly what is absent and ask how to reconcile the history with the pet’s circumstances and applicable local rules. Rabies requirements differ by jurisdiction, so a shelter record does not by itself establish what a licensing authority requires where you live.
Microchip, surgery, and pending care
Ask staff to confirm whether the pet was scanned or implanted with a microchip and to give you the number and registry-transfer instructions. If the shelter has registered the chip for the adopter, confirm what contact details were entered and whether any further step is required. Seattle Animal Shelter says it registers a new pet’s microchip when applicable; AVMA guidance emphasizes keeping the registry information current.

For spay or neuter status, request the surgery date and any operative or discharge note available. If the procedure is not yet done, collect the voucher, its deadline, and the participating clinic information rather than relying on a verbal assurance. Fairfax County’s adoption information illustrates that records may include completed surgery details or a later voucher. Ask about other procedures, such as dental work, and obtain the reports if they exist.
Exams, diagnostic results, medications, and parasite treatment
Request dated physical-exam findings and any notes about weight, condition, illness, injury, allergy, or care restriction. Then ask for reports from tests or procedures the animal received—such as laboratory work, parasite or heartworm testing, FeLV/FIV testing, imaging, dental care, or surgery. The Association of Shelter Veterinarians includes test results, procedures, and treatments in its description of medical records, so these are part of the history to pass on, not optional background.

For every current or recent medication, capture the name, dose, route, schedule, start and stop dates, and refill information if recorded. Ask what parasite prevention or treatment was administered and when, including deworming and flea or tick care. The UK government guidance specifically names worming and flea-treatment evidence, while shelter-veterinary standards include treatments and medications more broadly. If a label, dose, or end date is unclear, do not infer it; have the veterinarian review the documentation.
Behavior notes and adoption papers
Ask for available behavior observations, bite history, known triggers, and any useful foster or prior-home information. Such notes can help you describe the pet’s history accurately and can inform safe handling and care discussions. Seattle Animal Shelter says adopters receive copies of available medical and behavioral records; New York law also lists available health and bite history among shelter records. The precise records a shelter must provide vary by jurisdiction, so these examples should not be read as a universal legal checklist.

Keep the signed adoption contract, proof of ownership, animal ID, and shelter contact information in the same file. AVMA owner guidance includes adoption papers and proof of ownership among records to retain. These documents can help connect the medical packet to the animal and provide a route back to the shelter if a date, test, or procedure needs clarification.
How to check whether the packet is complete
Use the following six categories to audit what you received. The purpose is to spot unanswered questions—not to assume that every record exists or that a gap proves care was not performed.

- Identity: Do the animal ID, species, age, sex, reproductive status, and microchip details consistently identify this pet?
- Preventive care: Are vaccine and rabies details dated and verifiable, and is parasite-control history recorded?
- Procedures: Is spay/neuter status clear? Are surgery, dental, or other procedure notes available, or is a voucher still pending?
- Medical history: Are diagnoses, exam findings, test results, and medication details included where known?
- Behavior and safety: Are available bite history, behavior observations, or known triggers included?
- Next actions: Does the packet identify what is due, pending, or subject to follow-up—and when, if a date is provided?
If something is missing, ask the shelter whether the item was never recorded, is held by a clinic, or is still pending. Request the name of the clinic or laboratory when one is listed so your veterinarian can determine whether further records can be obtained. Do not fill gaps by assigning dates, doses, or test results that the documents do not establish.
What to do after pickup
- Make a durable copy. Scan or photograph each page, including certificates, vouchers, and the adoption contract. Store the files somewhere you can retrieve them and keep the original papers together. AVMA recommends retaining copies of adoption, vaccination, medical-history, and microchip information.
- Transfer the microchip registration promptly. Use the registry and instructions provided by the shelter. Confirm that your phone number, email, and address are current, and retain the chip number with the medical file.
- Arrange the first veterinary visit using the shelter’s timing instructions. Houston BARC recommends veterinary follow-up within 72 hours, but that is a recommendation from one shelter, not a universal deadline. Follow the instructions you were given and account for the animal’s condition; contact a veterinarian sooner if the shelter directed you to do so or you have an urgent concern.
- Bring the full packet, not only the vaccine card. Give the clinic copies of the available exam notes, tests, procedures, medication and parasite-treatment history, microchip information, and adoption details. Mention which records are missing or unclear.
- Ask the veterinarian to reconcile the record. The clinician can review the documented history against the pet’s age, species, lifestyle, current condition, and local rabies rules, then explain what follow-up is appropriate. Local legal requirements vary; New York’s shelter-record statute is one example of jurisdiction-specific rules, not a nationwide standard.
How to prepare for the first veterinary visit
Before the appointment, gather the digital and paper copies, write down any dates or instructions that are unclear, and note which care is pending. Bring medication packaging or labels if the shelter provided them. If a voucher has a deadline or names a participating clinic, include those details so the veterinarian can account for the arrangement. This preparation makes it easier to distinguish documented care from information that still needs verification.
At the visit, start with the pet’s identity and adoption date, then point out any known diagnosis, injury, allergy, medication, procedure, or behavior concern. Ask the veterinarian to review vaccine and rabies records rather than assuming that a card or verbal summary settles every question. If the packet lacks a test result or procedure report, explain that the shelter did not provide it; let the clinic decide whether clarification or additional evaluation is warranted.

The records establish what the shelter has documented and can provide; they do not necessarily establish everything that happened before intake.
