Why Cats Pace: Possible Causes, Warning Signs, and What to Do

Pacing is a behavior, not a diagnosis. Learn what may be behind it, which warning signs need urgent attention, and how to prepare for a veterinary visit.

Cat pacing can reflect illness, pain, disorientation, sensory changes, stress, or another change in your cat’s environment or health. The behavior alone cannot identify the cause. If pacing is new, persistent, or accompanied by other changes—especially in an older cat—contact your veterinarian rather than assuming it is simply a habit or a normal part of aging.

What pacing can—and cannot—tell you

Walking back and forth, circling, or repeatedly moving around the home is an observable behavior, not a diagnosis. The same pattern can accompany different health problems, and a cat may have more than one factor affecting its behavior. Age, when the pacing happens, and changes in appetite, drinking, urination, sleep, vocalizing, coat, or mobility can help your veterinarian decide what to investigate. They cannot establish a cause on their own.

The possibilities below are areas to discuss with a veterinarian, not eight conditions that can be diagnosed from pacing.

Eight possible explanations to consider

1. Hyperthyroidism or another illness

Hyperthyroidism is a common condition in middle-aged and older cats. Restlessness or increased activity may occur along with weight loss despite a good appetite, increased thirst or urination, vomiting, diarrhea, or a poor coat. These signs are not unique to hyperthyroidism; a veterinarian assesses the cat and may use blood tests, including thyroid hormone testing, to investigate it.

If hyperthyroidism is diagnosed, treatment options can include medication, radioactive iodine, surgery, or dietary therapy. The appropriate choice depends on the cat’s circumstances and should be discussed with the veterinarian.

2. Arthritis or another painful condition

Pain and reduced mobility can change a cat’s behavior. An older cat may find it harder to reach a litter box, food, water, or a favorite resting spot, even if there is no obvious limp. Watch for changes in how easily your cat moves around the home or gets to essential resources, and share them with the veterinarian.

3. Cognitive dysfunction or disorientation

In an older cat, pacing may occur alongside altered sleep, nighttime vocalizing, seeming disoriented, or getting stuck in a corner. Cognitive dysfunction is one possible explanation, but it should not be presumed before medical causes are considered.

Cornell Feline Health Center’s cognitive dysfunction guidance describes circling or getting stuck as patterns that may also arise from neurologic or physical problems. A concerning pattern warrants assessment, not a home diagnosis based on one behavior.

4. Sensory changes or hypertension

Nighttime vocalizing or restlessness in a senior cat may have several explanations, including changes in hearing or vision, pain, hyperthyroidism, hypertension, or cognitive dysfunction. The timing can be useful information for a veterinarian, but it does not distinguish among these possibilities by itself.

5. Kidney, urinary, or other age-associated disease

Illness affecting different organ systems can alter behavior or litter habits. For example, changes that increase urine production may affect how often a cat needs the litter box, while arthritis may make it harder to reach one promptly. Note changes in drinking and urination as well as pacing, and tell the veterinarian when they began.

6. Stress or anxiety

Anxiety and nighttime restlessness can be among behavior changes seen in older cats. A change in the home or routine may be relevant context, but it does not prove that stress is the cause. Discuss new or persistent pacing with a veterinarian before treating it as an emotional or environmental problem alone.

7. Neurologic or other brain-related problems

Circling, seeming unaware of surroundings, or getting stuck can be concerning patterns with neurologic or physical explanations. Cornell’s guidance quotes Alexander de Lahunta, DVM, describing a cat that acts as if “it’s in a world of its own,” does not respond, circles, or remains stuck in a corner as a pattern that can prompt consideration of a forebrain problem. This is not a diagnosis of ordinary pacing; a veterinarian must assess the individual cat.

8. A change whose context matters

Sometimes the useful next step is not choosing a label but describing the change precisely. When did pacing begin? Does it happen at night, or at particular times? Did appetite, thirst, urination, sleep, vocalizing, coat condition, or mobility change around the same time? Did anything in the home or routine change? These details help the veterinarian interpret an overlapping set of signs.

When pacing needs urgent or prompt veterinary attention

  • Emergency: Inability to urinate is life-threatening and requires urgent veterinary care. Sudden inability to use the back legs also needs immediate veterinary attention.
  • Contact your veterinarian: Arrange an assessment for new or persistent pacing, particularly if your cat is older or has other behavior or physical changes. Do not dismiss a change as untreatable aging.

What you can do while arranging an assessment

Keep a useful record

Write down when the pacing happens and note changes in appetite, thirst, urination, sleep, vocalizing, coat, and mobility. A clear timeline can help your veterinarian understand what changed and what else is happening.

Make essentials easier to reach if mobility is difficult

Keep food, water, and the litter box accessible. A low-sided litter box or ramp may help some older cats with access, but these are accommodations—not a diagnosis or treatment for the cause of pacing. Ask your veterinarian what changes suit your cat.

Ask about behavior support when appropriate

If behavior problems persist after medical causes have been assessed, a veterinarian may help determine whether referral to a qualified animal-behavior professional is appropriate. The ASPCA identifies certified applied animal behaviorists and veterinary behaviorists as possible resources.
