Feline atopic skin syndrome (FASS) is an itchy, inflammatory skin disease associated with environmental allergens. It can cause hair loss, crusted bumps, head-and-neck scratching, or other skin lesions—but those signs can also come from fleas, infections, food reactions, parasites, and other conditions. A veterinarian must assess the cat and rule out competing causes; allergy tests alone cannot diagnose FASS.
What is atopic dermatitis in cats?
Merck Veterinary Manual uses the term feline atopic skin syndrome (FASS) for allergic skin disease associated with environmental allergens. Its review, last updated May 2026, notes that this condition was previously called non-flea-induced, non-food-induced hypersensitivity dermatitis. The broader proposed term feline atopic syndrome (FAS) refers to allergic disease that may affect the skin, gastrointestinal tract, and respiratory system.

The condition is thought to involve genetic predisposition, impaired skin-barrier function, and changes in the immune system. A FASS diagnosis does not, by itself, establish the precise cause in an individual cat. Potential allergens may be inhaled or contact the skin; avoiding a suspected allergen can be considered, but may be difficult even when it has been identified. What signs might an itchy cat show?
Itching, or pruritus, is the main feature. Cats may develop one or more recognizable skin-reaction patterns. These are clues for a veterinary assessment, not proof of atopy.

Miliary dermatitis
Small crusted bumps may appear in one area or across the body. They can be difficult to spot beneath long fur, though a person may feel them while petting the cat.
Hair loss from overgrooming
A cat may lick or chew away fur, often on areas it can reach, such as the abdomen, flanks, thighs, or tail area. Hair loss can therefore be a sign of itch even if the cat is not seen scratching.
Head and neck itching
Repeated scratching or skin lesions may affect the head, face, ears, or neck.

Eosinophilic skin lesions
FASS can be associated with patterns such as plaques, indolent ulcers, or granulomas. Similar-looking lesions can have other causes, so appearance alone is not enough to identify the disease.
How do veterinarians diagnose FASS?
Diagnosis is a clinical process: the veterinarian considers the cat’s history and examination findings, then investigates or rules out other causes of itch. The work-up may include a physical and dermatologic examination, flea combing, skin cytology or scrapings, and fungal culture. The Cornell University College of Veterinary Medicine also notes that assessment may involve seeing how a cat responds to medical treatment or dietary changes; treatment depends on the cause identified. Fleas and infection need attention
Flea allergy dermatitis is the most common allergic dermatitis in cats, according to Merck. The veterinarian may review or start appropriate flea control as part of the assessment. Flea combing can help check for an important alternative cause, but it neither treats FASS nor establishes an atopy diagnosis. If an infection is suspected, the veterinarian can use tests such as skin cytology to assess it.

Food reactions may need an elimination trial
In a nonseasonal case, after flea allergy and infectious skin disease have been addressed, a veterinarian may use a food-elimination trial to assess an adverse food reaction. This helps investigate another possible cause of itch rather than confirming environmental atopy by itself.
Allergy tests are not a stand-alone diagnosis
Blood or intradermal allergy tests may show allergen-specific IgE or exposure patterns, but a positive result alone does not diagnose FASS. Merck describes their use as an aid, including to help select allergens for allergen-specific immunotherapy after a clinical diagnosis has been made.
How to interpret the published diagnostic criteria
Merck summarizes criteria for non-flea-induced hypersensitivity dermatitis from 2021 guidelines by Santoro and colleagues: meeting six of ten criteria had reported sensitivity of 90% and specificity of 83%. How is atopic skin disease managed?
There is no single treatment that suits every cat. Merck describes an individualized, multimodal approach: manage flare factors, treat confirmed secondary infection, reduce itch, consider allergen avoidance where practical, and assess whether allergen-specific immunotherapy is appropriate.
Address confirmed infection and flare factors
When cytology confirms bacterial infection, treating it may reduce lesion severity. Topical antimicrobials are preferred when feasible, according to Merck. The veterinarian may also consider whether a relevant environmental allergen can realistically be avoided.

Veterinarian-directed options discussed by Merck include topical or systemic glucocorticoids, modified cyclosporine, antihistamines, essential fatty acids or palmitoylethanolamide, and allergen-specific immunotherapy. These options differ in route, likely role, response, adverse effects, and monitoring needs; suitability depends on the cat. They are not owner dosing instructions, and supplements should not be used as a substitute for veterinary care.

For longer-term antipruritic treatment, Merck advises tapering to the lowest dose and frequency that maintain control. Long-term glucocorticoid use requires monitoring, and cyclosporine can have adverse effects. Ask the veterinarian what follow-up is needed for the specific treatment and the cat’s health.
Understand the status of oclacitinib
Merck states that oclacitinib is approved for canine atopic dermatitis but not for FASS; its use in cats described in the review is extralabel, and long-term safety data are unavailable. Approval and prescribing rules vary by jurisdiction, so the veterinarian should verify local labeling and current guidance.
Set expectations for ongoing care
FASS management may require adjustments as itch, lesions, treatment response, and adverse effects change. Keep the veterinarian informed about new symptoms and how the cat responds; do not start, stop, or change medication without veterinary direction.
When should you contact a veterinarian?
Arrange a veterinary assessment if your cat has persistent itching, repeated overgrooming or hair loss, crusted bumps, or recurring skin lesions. Because several conditions can look alike and some may coexist, guessing at the cause or treating the skin alone can delay appropriate care. Seek veterinary advice before giving medication or supplements intended for people or other animals.
