Why Your Cat’s Anus Looks Red: 8 Possible Causes and When to Call the Vet

A cat’s red-looking anus is a symptom, not a diagnosis. Learn eight possible causes and which signs call for prompt veterinary attention.

A red-looking anus is a sign of irritation or another problem, not a diagnosis. Diarrhea, anal sac disease, straining, injury, and skin conditions can look similar, and a veterinarian may need to examine your cat to identify the cause. If tissue is protruding, or there is painful swelling or bloody or pus-like drainage, seek veterinary care promptly.

Eight possible reasons a cat’s anus looks red

These are plausible explanations, not a ranked list: available veterinary references do not establish which causes are most frequent specifically among cats with anal redness. More than one problem can be involved—for example, intestinal disease may cause diarrhea, which can irritate the skin.

1. Diarrhea and irritation

Loose or frequent stool can irritate the skin around the anus. Inflammation of the rectum may occur with diarrhea, and repeated licking can further aggravate the area. Diarrhea itself can have many causes, including parasites, infection, a diet change, food intolerance, inflammatory bowel disease, or another illness. A red area alone cannot identify which cause is involved. PetMD’s overview of rectal and anal inflammation discusses these overlapping signs.

2. An impacted anal sac

Cats have anal sacs beside the anus. If a sac’s duct does not drain normally, material can build up and cause irritation or swelling. Clues may include scooting, licking, an unpleasant odor, discomfort, or reluctance to defecate. Anal sac disease is much less common in cats than in dogs, so redness by itself is not a reason to assume this is the cause. 3. An infected anal sac or abscess

An impacted sac can become infected and develop an abscess. A painful swelling beside the anus or bloody, sticky, or pus-like drainage needs veterinary attention. Treatment depends on the findings and may include veterinary expression or flushing, medication, or surgery in selected cases; it is not a condition to treat by attempting to empty the sacs at home.

4. Rectal prolapse

A pink or red tubular mass, or other tissue protruding through the anal opening, may be rectal tissue that has prolapsed. Severe diarrhea or repeated straining can be associated with prolapse. Merck Veterinary Manual advises: “No matter what type of prolapse is present, any tissue mass protruding from the anal opening should be promptly examined by a veterinarian.” Merck Veterinary Manual’s article on rectal and anal disorders in cats was last updated in September 2024.

5. Parasites causing intestinal signs

Some parasites can contribute to diarrhea or straining, which may in turn irritate the anal area or be associated with prolapse. Redness does not show that parasites are present. A veterinarian may recommend stool testing to investigate the cause.

6. Constipation or repeated straining

Straining or painful defecation can accompany anal or rectal disease and may be associated with prolapse. Repeated trips to the litter box or straining need assessment: urinary problems can also look like difficulty using the litter box, and appearance alone is not a reliable way to tell the difference at home.

7. Injury or a rectal tear

A sharp foreign object or another injury can tear tissue in or around the rectum. Possible signs include bleeding, straining, constipation, or reluctance to defecate. These signs warrant prompt veterinary advice rather than home treatment.

8. Skin inflammation, infection, or a mass

Skin irritation or infection can cause redness and itching. Less commonly, a tumor or another rectal or anal disorder may be involved. An examination is needed to distinguish these possibilities from more common sources of irritation. When a red anus needs veterinary attention

  • Seek prompt assessment for protruding tissue. Do not wait to see whether a mass retracts on its own.
  • Contact a veterinarian promptly for painful swelling beside the anus or bloody, sticky, or pus-like drainage. These signs can occur with an infected anal sac or abscess.
  • Get prompt advice for severe bloody diarrhea, or diarrhea accompanied by weakness, fever, vomiting, abdominal pain, dehydration, or loss of appetite. VCA’s cat diarrhea guidance covers warning signs and diagnostic approaches.
  • Arrange veterinary advice for persistent or recurring symptoms: ongoing redness, repeated licking or scooting, painful defecation, recurring diarrhea, blood, odor, or discharge.

What the veterinarian may check

The assessment depends on your cat’s signs and history. A veterinarian may inspect the area and perform a rectal examination. Depending on what they find, they may recommend a fecal or rectal sample, blood tests, imaging, or another procedure. The findings guide treatment; redness alone does not point to one universal medicine or product.

What to do while arranging care

  • Note when the redness began and whether you have seen diarrhea, straining, scooting, licking, blood, odor, swelling, or discharge.
  • Tell the clinic if tissue is protruding or your cat seems unwell, so they can advise how urgently your cat should be seen.
  • Do not try to express anal sacs or apply human medicines to an undiagnosed area. Veterinary references describe cause-specific care, not a general home treatment for anal redness.
