2026 AAHA Diabetes Management Guidelines for Dogs: What Owners Need to Know

AAHA’s 2026 dog-specific diabetes guideline sets controlling clinical signs as the goal of insulin therapy and explains how glucose curves, CGM and fructosamine fit into care.

The American Animal Hospital Association’s 2026 diabetes guideline for dogs, published August 31, 2026 and announced in AAHA’s September 1, 2026 release, treats canine diabetes as a disease of absolute insulin deficiency that requires insulin. Its first treatment goal is control of the dog’s clinical signs and quality of life, not a single perfect blood glucose number. Glucose readings, glucose curves, continuous glucose monitor (CGM) data and fructosamine all help direct care, but the guideline says each must be read alongside the dog’s signs, body weight and daily routine.

What the 2026 guideline is and who it is for

The guideline is a dog-specific reference written for veterinary teams. It covers diagnosis, insulin treatment, monitoring, troubleshooting, diabetic ketoacidosis (DKA), diet and communication with clients. For the first time, AAHA has split its diabetes guidance into separate canine and feline documents. AAHA Chief Medical Officer Jessica Vogelsang, DVM, said the division was made “in recognition of the unique nature of the disease process in each species.” If you have seen older combined dog-and-cat advice, the canine guideline is now AAHA’s current reference for dogs.

Task force member Audrey K. Cook, BVM&S, MSc Vet Ed, FRCVS, DACVIM, DECVIM, DABVP (Feline Practice), linked the update to practice changes: care of diabetic dogs “has changed dramatically over the last few years, with the advent of inexpensive continuous glucose monitors and a new understanding of how to leverage long-acting insulins.” Both quotations come from AAHA’s September 1, 2026 release.

Use the guideline as the framework your veterinarian works within, not as a home dosing protocol. Insulin choices, dose changes and interpretation of monitoring results belong to the dog’s veterinary team.

How diabetes is diagnosed in dogs

Common signs that prompt a diabetes workup are increased urination and thirst, increased appetite and weight loss. The guideline notes that the average canine renal threshold for glucose is 160–200 mg/dL. Once blood glucose rises above that range, glucose appears in the urine, which is why glucosuria matters in the diagnostic picture.

AAHA’s summary of the Project ALIVE criteria describes diabetes as present when a dog shows all of the following:

  • Blood glucose of at least 200 mg/dL, measured fasted or nonfasted.
  • Glucosuria (glucose in the urine) together with characteristic signs.
  • No other plausible cause for the findings.

A hyperglycemic crisis can also meet the diagnostic picture. These are criteria for veterinary diagnosis, not a home test.

Two distinctions matter for owners. Ketones may appear in the urine, but ketonuria does not by itself mean diabetic ketoacidosis. The guideline also separates clinical diabetes from transient insulin resistance and other conditions that can cause sustained insulin resistance. Because absolute insulin deficiency can lead to ketosis within several days, a dog may show overt signs for a relatively short time before it is examined.

Treatment goal: control the signs first

The guideline describes diabetic dogs as absolutely insulin deficient and dependent on insulin therapy. Most dogs tolerate lifelong treatment well, and prognosis is good with appropriate therapy and monitoring.

Why the goal is clinical control, not a single number

AAHA states that the primary goal of treating canine diabetes mellitus is to control clinical signs (Section 7, Troubleshooting in Diabetic Dogs). The guideline says tight glycemic control is rarely possible with twice-daily intermediate-acting insulin, which is why the benchmark is the dog’s condition rather than a narrow glucose range.

Brief high readings in a controlled dog

AAHA says occasional brief blood glucose elevations above 300 mg/dL may occur in clinically controlled dogs receiving twice-daily intermediate-acting insulin. Such readings do not necessarily require additional workup. The 300 mg/dL figure is not a stand-alone treatment trigger.

When signs persist

Persistent polyuria, polydipsia, polyphagia or weight loss warrants review with the veterinary team. The review covers insulin administration, dose, diet, the dog’s insulin response and any concurrent disease.

Blood glucose curves: what they show

A blood glucose curve is a serial test that shows how insulin behaves over a day in one dog. As the guideline describes it:

  1. A baseline blood glucose measurement is taken.
  2. The dog eats a normal meal and receives the full insulin dose.
  3. Readings are taken, typically every two hours, for up to 10–12 hours.
  4. The results identify the nadir (lowest point), the duration of insulin effect and the degree of fluctuation.

AAHA’s stated ideal nadir is 80–150 mg/dL. The guideline cautions against prolonged subclinical hypoglycemia below 60 mg/dL, meaning low glucose that continues without obvious signs. These are veterinary interpretation parameters, not instructions for owners to change a dose.

Comparing monitoring methods

Monitoring approaches differ in what they measure, how long they cover and how reliable the device is for a dog. The guideline does not recommend choosing a method based on device convenience alone.

Which glucose monitor can I use for my dog?

The answer depends on the device’s calibration for veterinary patients and on who interprets the readings. Ask your veterinary team which meter the clinic uses for serial testing and whether a home device is appropriate for your dog.

Veterinary-calibrated meters

AAHA’s discussion of practical serial monitoring names the AlphaTrak 3 as an example of a veterinary-calibrated meter. It is an example, not an endorsement, and availability should be confirmed with your clinic.

People-calibrated meters

The task force does not recommend people-calibrated glucose meters for veterinary use. Calibration differences can make the results inaccurate, so a reading from one of these meters should not be used to make treatment decisions without veterinary guidance.

CGM sensors such as FreeStyle Libre

AAHA discusses FreeStyle Libre as a CGM used in veterinary patients. CGM shows trends in the dog’s normal environment, which can reveal patterns a single clinic reading misses. Its readings can differ from a veterinary-calibrated meter. Before changing therapy, confirm a low reading with a veterinary-calibrated point-of-care meter, particularly when the dog shows no signs of hypoglycemia. Compatibility and availability should be checked for the individual dog.

When glucose should be measured

AAHA describes monitoring in these situations:

  • Five to 14 days after starting insulin or changing a dose.
  • When signs recur in a dog that was previously well controlled.
  • When hypoglycemia is suspected.

Reading the numbers alongside signs and weight

The guideline is clear that clinical signs and weight remain essential. No single glycemic assessment consistently separates well-controlled from poorly controlled dogs.

AAHA cites a Briggs et al. case review from 2000. In a review of 53 canine diabetes cases, all glycemic assessments and glycated protein concentrations agreed with good control in only 60% of dogs classified as clinically well controlled. The finding is a reason to read measurements together with the dog’s condition. It is not a population-wide estimate of how accurate monitoring is.

A simple home log helps the veterinary team see the whole picture. Record appetite, drinking, urination, weight, insulin doses and times, and any episodes of weakness, and bring the log to each visit.

Home routine and when to call the veterinary team

The guideline advises owners to keep a consistent care routine, including feeding and insulin timing, as directed by the veterinary team. It also advises contacting the team when signs return or when hypoglycemia is suspected.

For insulin technique, AAHA’s resource center offers a client handout titled “How to administer insulin to your dog,” along with videos. Syringes must match the insulin concentration on the vial, such as U-40 or U-100 insulin. Ask your clinic to confirm which syringe type goes with your prescription.

Call the veterinary team promptly if a dog with diabetes stops eating, vomits, becomes lethargic or shows signs of low blood sugar such as weakness, unsteadiness, trembling or seizures. Ask for a written plan for what to do at home. The guideline’s DKA and troubleshooting sections are written for clinicians, so the veterinary team is the right source for emergency steps.
