Pacing is a behavior, not a diagnosis. In cats it can come from stress or unmet needs, pain, illness such as hyperthyroidism, age-related cognitive or sensory change, or neurologic disease, and more than one cause can be present at once. If the pacing is new, persistent or unusual, especially in a senior cat, call your veterinarian rather than assuming the cat is bored or anxious.
Five common reasons cats pace
These are practical groupings, not a ranked or exhaustive list. They overlap, and your veterinarian interprets them using the cat’s age, history, other signs, examination and test results. Don’t try to match one symptom to one disease.
1. Stress or environmental frustration
A change in routine or surroundings can affect behavior. Keeping life predictable and looking at the cat’s environmental needs is the sensible response, as AAHA’s senior cat guidance frames it. Don’t settle on stress as the explanation until health problems have been considered.
2. Pain or mobility problems
Pain can change activity, sleep, interaction and litter-box behavior. Arthritis and dental disease are easy to miss, particularly in older cats, so a restless cat may be uncomfortable rather than anxious (Cornell Feline Health Center; AAHA).

3. Hyperthyroidism or another systemic illness
Hyperthyroidism can cause increased activity or hyperactivity. Clues that may accompany it include weight loss despite a good appetite, increased thirst or urination, vomiting, diarrhea and a poor coat (Cornell). These are reasons to book an exam, not a home diagnosis. The FDA notes that “almost 99 percent” of feline hyperthyroidism cases are caused by a benign thyroid tumor. That describes what causes the disease, not how many pacing cats have it.
4. Cognitive or sensory changes with age
Wandering, disorientation, altered sleep, nighttime vocalization and pacing can occur in aging cats. Cognitive dysfunction should be considered only alongside evaluation for look-alike medical causes, including pain, hypertension, hyperthyroidism and kidney disease. Penn Vet’s Carlo Siracusa, professor of clinical animal behavior and welfare, says: “It is therefore important that cat owners prioritize weight control, predictable routines, stress reduction, and control of chronic inflammatory disease, such as oral and gastrointestinal disease.”

5. Neurologic or other serious disease
Circling, getting stuck in corners, altered awareness or other abrupt behavior changes can signal a neurologic problem or another illness. Cornell attributes this to Alexander de Lahunta, DVM: “when examining a cat, if it is acting as if it’s in a world of its own, doesn’t respond to me, paces in a circle, or gets itself in a corner and just stays there, I’ll consider that to be a prosencephalon problem.” That is a clinician’s interpretation in context, not something to apply at home. If you see these signs, contact a veterinarian for guidance. The sources reviewed don’t give a complete pacing-specific emergency checklist.
When to call the veterinarian
Any new or unusual behavior change merits an evaluation, especially in a senior cat. Call promptly if pacing is persistent or comes with:

- changes in eating or drinking
- weight loss, vomiting or diarrhea
- altered litter-box habits
- weakness or other new signs
This is not a complete triage list. If you’re unsure, phone the clinic.
What the vet visit may involve
Expect a history and physical exam first. Depending on the signs, the vet may check blood pressure, blood chemistry, thyroid hormone levels, urine or neurologic function. AAHA lists physical and neurologic examination, biochemical evaluation and urinalysis in assessing suspected cognitive dysfunction, and Cornell describes thyroid testing when hyperthyroidism is suspected. Testing is chosen for the individual cat.

How to help at home
- Keep a log. Note when pacing happens, how long it lasts, and any link to meals, nighttime, visitors, household changes or other signs. Share it with your vet. It’s record-keeping, not a diagnostic test.
- Keep things predictable and accessible. Make food, water, resting places and litter boxes easy to reach. Older cats with mobility problems may benefit from low-sided litter boxes or ramps.
- Add enrichment once health is being addressed. Play, new toys or cognitive games can suit an interested cat, and AAHA includes enrichment in its management guidance. A toy or puzzle feeder won’t treat pacing or cognitive dysfunction by itself.
- Don’t self-medicate. Treatment depends on the cause, and Cornell notes medication for cognitive problems is considered only after other causes are ruled out. Don’t start any drug or supplement without veterinary guidance.
If pacing persists after medical causes have been evaluated, a qualified feline behavior professional can help, alongside your vet rather than instead of one.
