Its active ingredient, bismuth subsalicylate, contains salicylate, which is absorbed systemically in cats and can cause toxicity. If your cat has already swallowed any, contact a veterinarian or animal poison-control service promptly; do not wait for symptoms.
Why Pepto-Bismol is a concern for cats
Pepto-Bismol contains bismuth subsalicylate. The Merck Veterinary Manual says practically all of the salicylate from this compound is absorbed systemically in dogs and cats. It cautions that salicylate toxicosis is possible, especially in cats. The Pet Poison Helpline also identifies Pepto-Bismol as a possible salicylate source and says cats are more sensitive to aspirin poisoning than dogs.

This is a serious reason not to self-dose, but it does not mean every exposure will cause poisoning. The risk depends on factors including the amount ingested and the cat’s size and individual circumstances. A veterinarian or poison-control professional needs the details to assess the situation.

Call your veterinarian or an animal poison-control service as soon as you realize your cat may have ingested it. VCA advises immediate contact for suspected overdose or adverse reaction, and its guidance on cat toxins recommends calling promptly after a pet consumes a toxin.

Have the following information ready:
- The exact product and formulation, such as liquid, tablet, or paste
- Your best estimate of how much was swallowed
- When the exposure happened
- Your cat’s weight
- Any signs you have noticed
Do not induce vomiting or give another medicine at home unless a veterinary professional specifically tells you to. The appropriate next step depends on the exposure and your cat’s condition.
Signs to report to a veterinarian
Possible salicylate-related signs include:
- Reduced appetite or refusing food
- Vomiting, which may contain blood, or diarrhea
- Blood in the stool or black, tarry stool
- Abdominal pain or swelling
- Increased thirst or urination
- Lethargy, weakness, or pale gums
- Incoordination, seizures, or coma
- Difficulty breathing
Serious salicylate poisoning can also involve respiratory changes, elevated body temperature, kidney failure, bone marrow suppression, or effects on the central nervous system, according to the Pet Poison Helpline. These signs are not a way to diagnose poisoning or judge that a cat is safe; seek professional advice even if your cat seems well.

Black stool needs particular care. Bismuth subsalicylate can turn feces black, which Merck says can interfere with evaluating stool for bleeding. Stool color alone cannot establish whether your cat is safe or whether bleeding is present.
Why veterinary references mention bismuth for cats
Some veterinary references discuss bismuth compounds for animal patients. VCA describes their use for diarrhea and certain Helicobacter infections, and Merck lists bismuth subsalicylate in a professional drug table. That does not make a human Pepto-Bismol product appropriate for owners to give a cat: Merck says there is no precise dose for animals and warns of possible salicylate toxicosis, especially in cats. Any use must be specifically directed by a veterinarian.

VCA lists cats among the groups requiring caution with bismuth compounds. It also advises against use in animals with allergies to the ingredients or similar medicines, including aspirin or other NSAIDs, and in those with stomach or intestinal ulcers. Caution is also advised for pregnant or nursing animals, animals with bleeding disorders, and young, debilitated animals.
Bismuth compounds may interact with antacids, aspirin, some heart and blood-pressure medicines, corticosteroids, insulin, NSAIDs, opioids, and tetracycline or other antibiotics. If a veterinarian prescribes a bismuth product, disclose all medicines and supplements your cat receives and follow the prescribed directions. For diarrhea without a prescription, ask a veterinarian to determine the cause and appropriate care rather than trying an over-the-counter human medicine.
