A cutaneous horn is a hard projection of keratin—the same structural protein found in claws and hair—that often grows from a cat’s paw pad. It can look like an extra claw, but it is not necessarily one, and its appearance alone cannot establish what caused it or rule out another lesion. A veterinarian can examine the growth and decide whether monitoring, testing, or biopsy is appropriate.

What a cutaneous horn looks like

Cutaneous horns are typically hard, brownish-yellow growths. They most often appear on the side of a paw pad or near a nail bed, and may affect one or several pads. They can also occur elsewhere, including occasionally on the nose. Cats.com’s veterinary explainer and the 2023 American Association of Feline Practitioners proceedings describe these locations and appearances.

A horn on a part of the paw that bears weight may press into the pad and contribute to discomfort or lameness. Watch for limping, frequent licking, changes in activity, redness, swelling, or discharge. These signs warrant veterinary attention; they do not reveal the cause by themselves.

Why cats develop them

The visible projection consists of excess keratin. In many cases, the cause is unknown. Friction or pressure has been suggested as a possible contributor, but it is not an established explanation for every horn. Some lesions can be associated with underlying skin conditions or other disease.

A horn is a description of a growth, not a diagnosis. Cutaneous horns have been reported in cats with feline leukemia virus (FeLV), often when multiple pads are involved. Similar keratin growths can occur with papillomavirus-associated lesions, actinic keratosis, squamous cell carcinoma in situ, squamous cell carcinoma, or keratinizing acanthoma. The horn alone does not prove that a cat has FeLV, a viral infection, or cancer. The Merck Veterinary Manual overview of epidermal and hair follicle tumors discusses these associations and differentials.

Papillomavirus also occurs in normal feline skin, so finding the virus does not by itself establish that it caused a particular lesion. The European Advisory Board on Cat Diseases’ feline viral papillomatosis guidance explains this limitation.

How a veterinarian evaluates a horn

A veterinarian may recognize a typical-looking horn during an examination. They will consider its location, whether it is solitary or multiple, whether it is changing, and whether it is painful or accompanied by other skin lesions. If the appearance or history raises concern, they may recommend a biopsy: a tissue sample is examined under a microscope to identify what is beneath the keratin projection.

The 2023 proceedings recommend considering FeLV screening in cats with cutaneous horns. They also discuss confirming FeLV-related horns through positive FeLV status and skin biopsy. Whether screening or biopsy is appropriate depends on the individual cat and the veterinarian’s assessment.

The proceedings advise that solitary horns should be biopsied for underlying neoplastic or pre-neoplastic causes, especially when a horn arises from a proliferative lesion. A horn’s presence is not itself a cancer diagnosis; biopsy is one way a veterinarian can investigate a concerning lesion.

Monitoring, removal, and what not to do at home

Care depends on the cause, location, and symptoms. The 2023 proceedings say idiopathic lesions that are not causing lameness may be monitored or left alone. If a horn is uncomfortable or its underlying lesion concerns the veterinarian, removal may be considered. Removing the base may reduce regrowth, but excision must be weighed against the risk of creating a large defect in the paw pad. These are clinical decisions, not home-care procedures.

Do not cut or trim a horn yourself. It can be difficult to tell where the keratin ends and healthy tissue begins; cutting into the pad can cause pain, bleeding, or infection. Cats.com notes that emollients are rarely effective or necessary for these growths, so they are not a substitute for veterinary assessment.
