If your pug is struggling to breathe, breathing with marked effort, has pale or blue gums or tongue, faints, or collapses, go to an emergency veterinarian immediately. Persistent noisy breathing, reduced exercise tolerance, or difficulty coping with heat also deserves a veterinary appointment—not dismissal as a normal pug trait.
Pugs can develop brachycephalic obstructive airway syndrome (BOAS), a group of upper-airway abnormalities that can restrict airflow. Here’s how to recognise possible signs and decide how urgently to act.
What breathing problems can BOAS cause in pugs?
Pugs have a short skull shape. In brachycephalic dogs, the skull bones shorten while soft tissues may not shorten to the same degree, leaving structures that can obstruct airflow. Pugs are among the breeds frequently affected by BOAS, but the specific abnormalities and signs vary from dog to dog, and problems can worsen with age. Cornell University College of Veterinary Medicine explains BOAS and its signs.
Listen and look for changes in your dog’s breathing and daily activity. Signs that merit a veterinary discussion include:

- Snoring, snorting, wheezing, or other persistent or worsening airway noise.
- Tiring quickly or doing less exercise than before.
- Difficulty coping with heat or humidity.
- Labored breathing or breathing with the mouth open.
- Gagging, retching, vomiting, or regurgitation.
- Disrupted sleep, increased abdominal effort, or visible movement of the skin at the front of the chest or neck while breathing.
Some pugs make respiratory noises habitually, but sound alone cannot establish how severe an obstruction is—or show that the noise is harmless. Ask your veterinarian about persistent or worsening noise, changes in stamina, heat intolerance, or sleep disruption. When is breathing trouble an emergency?
Seek emergency veterinary care immediately if your pug is struggling to breathe or breathing very laboriously, has blue or pale gums or tongue, faints, or collapses. These signs can indicate respiratory distress. Cornell University College of Veterinary Medicine states: “If your dog is experiencing a respiratory crisis, immediate transport to your veterinarian or nearest emergency hospital is essential.” See its guidance on recognising and responding to canine respiratory distress. Do not wait to see whether a breathing crisis passes, and do not try home remedies before seeking care. Call the emergency clinic while you are on the way if practical, but do not let the call delay transport.
During the trip to the emergency clinic
- Keep your pug as calm as possible and avoid exertion.
- Use air conditioning to keep the car cool if available; heat can increase panting and worsen breathing effort.
- Travel directly to a veterinarian or emergency animal hospital.
How quickly should you arrange a routine veterinary visit?
Book a veterinary discussion for persistent or worsening airway noise, reduced exercise tolerance, heat intolerance, or disrupted sleep, even if your pug is not in immediate distress. Changes in how your dog breathes, rests, or manages ordinary activity are useful details to report. If signs become labored breathing, gum or tongue colour changes, fainting, or collapse, treat the situation as an emergency instead.

How do veterinarians assess possible BOAS?
A veterinarian considers the dog’s breed and reported signs alongside a physical examination. Depending on the case, further assessment of airway structures may involve examination under light anesthesia, imaging, endoscopy, or CT. Owners should not try to judge the severity of airway obstruction from noise alone.
Management depends on the individual dog’s signs and examination findings. A veterinarian may recommend conservative management, additional diagnostics, or surgery; severe cases may need surgical correction, and a veterinary surgical specialist may be appropriate. The Cornell BOAS guidance describes these assessment and treatment considerations.
What can help reduce strain in routine care?
For routine management of mild BOAS, Cornell recommends maintaining a healthy weight, avoiding excessive heat and humidity, controlling activity, minimising stress, and using a harness instead of a neck collar. These steps may reduce avoidable strain; they do not cure airway obstruction or replace veterinary assessment.
