Ivermectin is an approved prescription ingredient for cats only in specific products, most notably HEARTGARD for Cats, which is labeled to prevent feline heartworm disease and to remove and control certain hookworms. It is not a general-purpose cat dewormer, and no cat should receive it based on a label figure, a human or livestock product, or a guess at weight. Any dose should come from a veterinarian who knows your cat, and any suspected overdose or ingestion of a non-cat product is an emergency.
What ivermectin is approved to do in cats
The FDA’s Freedom of Information Summary for HEARTGARD for Cats (ivermectin) describes a prescription chewable with two defined indications. The first is prevention of feline heartworm disease: the drug eliminates the tissue-stage larvae of Dirofilaria immitis for one month after infection. The second is removal and control of adult and immature (L4) Ancylostoma tubaeforme and A. braziliense hookworms. Those two claims define the approved use. They do not make ivermectin a universal cat wormer, and they do not cover every intestinal parasite a cat can carry.
Two points about the product matter for owners. It is given by mouth once a month during mosquito season, and the chewable is beef-based and palatable. It is labeled for use from six weeks of age. Heartworm prevention is a different job from treating an established infection. FDA consumer guidance notes that several prescription products are approved to prevent heartworm in cats, and that no FDA-approved drug treats heartworm disease in cats. If your cat already has heartworm disease, the question is a veterinary treatment plan, not which preventive to buy.
Dosage: what the label says and what it does not mean
Dosing is the part of this topic most likely to cause harm when read out of context. The label for HEARTGARD for Cats uses weight classes, and the FDA summary describes a minimum of 24 mcg/kg (10.9 mcg/lb) ivermectin when the product is used according to its listed weight schedule. These are label details for one named product. They are not a dosing calculator, and they do not tell you what your cat should receive. Weight changes, age, other medications, and the product’s current package insert all affect the correct choice, and your veterinarian is the person who sets it. A Merck reference table lists ivermectin at 0.024 mg/kg orally, monthly, against the same two hookworm species. That figure is the same 24 mcg/kg expressed in milligrams and is included only as reference context for clinicians. It is not a basis for dosing a cat at home or for substituting another ivermectin formulation.
Never use an ivermectin product made for horses, cattle, or other livestock on a cat. Those products are formulated at concentrations and with excipients that were never designed for feline dosing, and small volume errors are easy to make.

Uses veterinarians may direct beyond the label
Veterinary references describe extra-label ivermectin use for some parasite conditions. Merck notes that ivermectin has been used for notoedric mange (feline mange) in cats, while stating plainly that the drug is not FDA-approved for that indication and that caution and client education about its inherent risks are important. VCA’s medication guide also describes extra-label use for ear mites and cat scabies. Extra-label use means a licensed veterinarian has decided the risk-benefit balance for a specific animal. It is not a green light for owners to choose a regimen, copy a dose from a forum, or treat a skin problem they have not had examined.
Side effects and toxicity
Macrocyclic lactones, the drug class that includes ivermectin, can cause poisoning when a cat receives too high a dose or ingests a product not meant for its species or size. VCA’s toxicology material explains that toxic exposure can affect brain function. As Renee Schmid, DVM, DABT, DABVT, a veterinary toxicologist writing for Pet Poison Helpline, puts it in VCA Animal Hospitals’ article “Macrocyclic Lactone Poisoning in Cats”: “Macrocyclic lactones include ivermectin, eprinomectin, milbemycin, and moxidectin.” That classification matters because a cat exposed to a different macrocyclic lactone product can show the same toxicity.

The VCA medication guide lists serious neurologic signs, including stumbling, trembling, dilated pupils, drooling, and head pressing. It also lists gastrointestinal upset. Treat any of these as urgent. The list is not exhaustive, so a cat that seems unwell after any ivermectin exposure needs a veterinary assessment even if its symptoms are not on this list.
Signs that mean call a veterinarian now
- Stumbling, unsteady walking, or sudden weakness
- Tremors or trembling
- Dilated pupils
- Excessive drooling
- Head pressing or pacing
- Vomiting, diarrhea, or other gastrointestinal upset
What to do if you suspect an ingestion or overdose
- Stop giving the medication and keep the packaging or the bottle of the product your cat may have eaten.
- Call your veterinarian or the nearest 24-hour emergency clinic right away, and state the product name, the amount you think was ingested, and the time it happened.
- Call a pet poison control service if your veterinarian cannot be reached quickly, and follow its instructions.
- Do not induce vomiting or give any home remedy unless a veterinarian or poison service directs you to do so.
- Bring the packaging to the clinic so the treatment team can confirm the active ingredient and concentration.
Questions to bring to your veterinarian
- Is my cat’s problem a heartworm prevention need, a hookworm problem, or something that needs a different diagnosis?
- Which product, weight class, and schedule are correct for my cat today, and when should I weigh it again?
- If you are considering an extra-label use, what are the specific risks for my cat?
- Are there other medications or conditions that change the safety picture?
Product labels can change. Read the current package insert for the exact product your veterinarian prescribes, and confirm the directions with that clinician before the first dose.

For cat owners, the safe way to use ivermectin is simple: treat the approved product as a prescription item, follow the veterinary plan rather than the label figures alone, and treat any unexpected exposure as an emergency.
