Most cats that eat a single earthworm will likely be fine: earthworms are not established as poisonous to cats. The main concern is possible parasite exposure, not immediate poisoning. If your cat seems unwell or you are worried, contact your veterinarian; one earthworm does not mean your cat is infected, and the risk from a single ingestion has not been quantified.
What should I do if my cat ate an earthworm?
If your cat ate one and is acting normally, monitor them rather than assuming they are ill. The available veterinary guidance does not establish a universal need for testing or treatment after a single earthworm. Contact your veterinarian if your cat becomes unwell or if you have concerns. Do not give a dewormer unless your veterinarian advises it.

Can earthworms give cats parasites?
Yes, earthworms can be part of parasite transmission, but that possibility does not mean every earthworm carries parasites or that a cat will become infected after eating one.

Some Capillaria species use earthworms as an intermediate host: eggs must first be ingested by an earthworm, and a new host can then become infected by eating that worm. VCA Animal Hospitals notes that many infected cats show no visible signs and that limiting a cat’s ability to eat earthworms can reduce the likelihood of infection. VCA: Capillaria in Cats

Ascarid larvae
The Companion Animal Parasite Council says earthworms, and possibly other invertebrates, may harbor ascarid larvae originating from eggs in soil. Cats may be exposed by ingesting those hosts. This is a possible route, not evidence that earthworms routinely infect cats. CAPC: Ascarid
What signs should prompt a call to the vet?
Many Capillaria infections have no obvious signs. When signs occur, they vary with the species involved and can overlap with other illnesses, so symptoms cannot confirm a parasite on their own.

- Urinary signs: blood in the urine, accidents, or straining to urinate.
- Breathing signs: coughing, wheezing, shortness of breath, or labored breathing.
- Nasal signs: sneezing or nasal discharge.
Contact a veterinarian for advice if you notice these signs. Labored breathing or respiratory distress warrants prompt veterinary attention. Merck Veterinary Manual describes feline lungworm signs that can range from coughing and increased breathing rates to respiratory distress, but its overview does not establish earthworm ingestion as a common route for every feline lungworm species. Will my cat need a test or treatment?
That depends on your cat’s signs, history, and what parasite a veterinarian suspects. Diagnosis may involve examining feces or urine, depending on the suspected Capillaria species. The sources do not set a single testing protocol for every cat that eats an earthworm, and treatment decisions should come from a veterinarian. How can I reduce the chance of exposure?
The most direct prevention step is to limit your cat’s opportunity to eat earthworms, as VCA recommends. For a cat that spends time outdoors, a secure enclosure such as a catio is one possible way to limit access to roaming areas; it is an option, not a guarantee against parasites. Choose an arrangement that suits your cat and that you can maintain.
