These steps can reduce avoidable stress, but they are not a formal Fear Free® heartworm protocol, and they do not replace your veterinarian’s instructions.
What heartworm treatment may involve
The American Heartworm Society (AHS) describes melarsomine as the only FDA-approved drug for treating canine heartworm infection. It is given by injection at a veterinary hospital. The AHS owner guide outlines a typical sequence of veterinarian-directed preventive medication, 30 days of doxycycline or minocycline, three melarsomine injections, and follow-up testing. Some dogs need stabilization first, and the plan can vary with disease severity, symptoms, test results, and other health conditions. A typical AHS timeline is shown below; your clinic may adjust it for your dog.

Do not substitute preventive-only “slow kill” treatment or choose drugs or doses yourself. If you are unsure why a particular plan is recommended, ask your veterinarian to explain the expected benefits, risks, monitoring, and alternatives for your dog.

How to make strict rest practical
Restriction begins at diagnosis, not just after the injections. Excitement and exertion can increase risk, so plan the home environment before treatment gets underway. The AHS owner guide says restriction continues for 6–8 weeks after the last melarsomine injection; the 2024 guideline advises a gradual return to activity after that period. Get your veterinarian’s specific clearance before increasing activity. AHS owner guide; 2024 AHS canine guidelines.
Set up a calm resting area
- Choose a quiet room or, if suitable for your dog and recommended by your veterinarian, a crate or other small, secure area. A crate is a practical aid to restriction, not a treatment.
- Use gates or close off access to stairs, furniture, and other places where your dog might run or jump.
- Keep greetings low-key. Tell household members and visitors not to encourage chasing, jumping, or rough play.
- Take your dog outside on leash for brief bathroom trips only, following the clinic’s directions.
Offer low-key enrichment
Quiet mental stimulation and social contact can help make confinement less frustrating, provided they do not provoke exertion. AHS shelter guidance gives examples such as appropriate chew toys, reading to dogs, and in-kennel enrichment. Adapt activities to your dog, supervise as needed, and stop anything that leads to excitement, running, or jumping. AHS shelter heartworm management plan.

If your dog is difficult to keep calm, ask the veterinary team what options are safe. Do not start sedatives, supplements, or other medicines—or change prescribed doses—without veterinary direction.
Comfort after injections and when to call
Pain, swelling, and tenderness at the injection site are common after melarsomine. The AHS guideline calls for appropriate pain control, and the veterinary team can tell you what soreness to expect, how to give any prescribed medication, and whom to contact after hours. The AHS owner guide advises monitoring for coughing, gagging, vomiting, diarrhea, and loss of appetite. Contact your clinic for guidance if these occur. Seek immediate veterinary attention for excessive sluggishness, respiratory distress, or coughing up blood. Do not wait to see whether breathing difficulty or blood in a cough improves. Follow-up tests and activity clearance
Follow-up testing checks how treatment is progressing; it also informs the veterinary team’s next steps. AHS describes an interim assessment around day 120 and an efficacy assessment around day 365, with antigen and microfilaria testing specified at day 365 in the 2024 guideline. An earlier positive test does not by itself prove treatment failure: AHS notes that antigen can take longer to clear, so the veterinarian may recommend rechecking. If you are weighing treatment approaches
AHS recommends a three-dose melarsomine protocol. Its shelter FAQ says a labeled two-treatment option for dogs with asymptomatic, mild, or moderate disease may clear approximately 90% of worms, compared with 99% for three-dose adulticidal therapy. These are AHS-published estimates, not a guarantee for an individual dog. Discuss any proposed approach with your veterinarian, including efficacy, disease severity, safety, duration, monitoring, and the implications of prolonged worm survival. AHS shelter treatment FAQs.
