If your puppy’s demodectic mange returns, arrange a veterinary recheck rather than restarting treatment on your own. A relapse can mean mites are still present, a secondary bacterial infection has developed, the disease is more widespread than it first appeared, or another illness is affecting the immune response. Skin that looks smoother is not proof that demodex has cleared: generalized disease is monitored with repeat deep skin scrapings and a veterinarian’s defined stopping point.
Why recurring demodectic mange needs a fresh examination
Canine demodicosis is caused by Demodex mites living in hair follicles and sebaceous glands. Small numbers can be present without illness; disease develops when mites overgrow and produce clinical signs. When hair loss, redness, crusting or swelling returns, the appearance alone cannot tell you whether mites remain.

Merck Veterinary Manual notes that recurrence within the first year of treatment is not uncommon and that generalized cases may take several months to treat. That statement describes a pattern, not a diagnosis for an individual puppy. The useful next step is a recheck in which the veterinarian reviews the original diagnosis, treatment course, scraping results and the puppy’s current skin.

- Have the skin checked for secondary bacterial infection, which is common in generalized disease and can intensify inflammation.
- Review every treatment, dose, timing and missed or delayed follow-up visit with the veterinarian.
- Discuss whether the age, distribution of lesions or treatment response warrants evaluation for an underlying problem.
First determine whether the pattern is localized or generalized
The distinction changes both urgency and expectations. A few circumscribed patches in a young dog behave differently from widespread, inflamed disease.

| Pattern | Typical findings | Why it matters when signs return |
|---|---|---|
| Localized juvenile demodicosis | A small number of limited hairless patches | Often resolves spontaneously, but a new or expanding lesion still needs a veterinary assessment to confirm that it remains limited. |
| Generalized demodicosis | Widespread hair loss, inflamed or swollen skin, papules, crusts or affected footpads | More serious, commonly complicated by bacterial infection and usually requiring prolonged, closely monitored treatment. |
Do not apply advice for mild localized patches to a puppy with rapidly expanding areas, multiple body regions involved, marked swelling, crusts or footpad disease. Those findings fit the more serious generalized pattern described by Merck Veterinary Manual and the Companion Animal Parasite Council (CAPC).
How a veterinarian confirms that demodex is still involved
Deep skin scrapings are the central test
CAPC describes microscopic examination of deep skin scrapings from affected, hairless areas as the diagnostic approach for canine demodicosis. The sample is examined for mites and their developmental stages. Because mites live down in follicles, a superficial look at the skin or a photograph cannot replace this test.
A recurrence may have more than one cause
If mites are not found, the veterinarian may investigate other explanations for the lesions. Merck’s owner guidance notes that testing for other infections or diseases that suppress immunity may be appropriate. A puppy can also have demodex plus a bacterial infection; a negative mite sample does not make an infected, painful skin problem harmless.
Bring the treatment history
Take the medication names, dates given, product packaging or photos of labels, bathing or dipping instructions, missed doses, and every previous scraping result. This record helps the veterinarian distinguish persistent infection from a genuine relapse and prevents accidental duplication or unsafe changes.

Why the skin can look better before the disease is cleared
Hair regrowth and reduced redness are encouraging clinical changes, but they are not the endpoint for generalized disease. Merck recommends continuing therapy until clinical signs have abated and at least two consecutive skin scrapings are negative, with those negative scrapings obtained one month apart. CAPC describes continuing selected treatment for one to two months after mites are no longer detected. These are different stated endpoints; your veterinarian should specify which monitoring schedule applies to your puppy and when treatment can safely stop.
CAPC describes a Demodex life cycle of approximately 20 to 35 days. That biology helps explain why a single negative scraping or a short period of visible improvement may not be enough to establish cure. Some dogs need several months of therapy and repeated examinations.
Secondary bacterial infection can make a relapse look dramatic
Generalized demodicosis frequently has a secondary bacterial component. Infection can increase redness, swelling, crusting, discharge, discomfort and odor, making the skin appear to worsen even when the mite burden is changing. Merck states: “Secondary bacterial infections must be treated aggressively with an appropriate antimicrobial.” The choice, route and duration belong to the veterinarian after examining the puppy; do not use leftover antibiotics or human skin products.
At the recheck, ask whether cytology, culture or another test is needed for the particular lesions. Treating infection and controlling mites are related but separate decisions, so improvement in one does not prove the other is resolved.

What treatment may involve—and why you should not dose at home
For generalized canine demodicosis, Merck describes systemic oral or topical isoxazolines as a preferred approach. In the United States, however, Merck and CAPC state that demodicosis is not an FDA-approved indication for the relevant isoxazoline products and that discussed therapies are extra-label. The veterinarian must select a product, calculate a dose for the puppy’s weight and health, check interactions and determine follow-up.
Other antiparasitic approaches, including macrocyclic-lactone protocols, also require veterinary supervision. Merck warns that ivermectin protocols need close monitoring, are contraindicated in Collies and Collie crosses, and can cause idiosyncratic toxicity in any breed. Never convert a flea-and-tick label into a home demodex regimen, split another animal’s medication, or increase frequency because lesions look worse.
Merck states that topical and systemic corticosteroids are contraindicated in dogs with demodicosis. A steroid can suppress inflammation while worsening the underlying mite-associated disease. If your puppy has received a steroid from any source, tell the veterinarian exactly what, how much and when.
What recurrence can reveal about the puppy’s health
Juvenile generalized disease
Generalized disease beginning in puppyhood is associated with an inherited immune defect. That association does not identify a specific genetic problem in an individual dog, but it is one reason veterinary guidance advises against breeding dogs that develop juvenile-onset generalized demodicosis.

Adult-onset generalized disease
When generalized disease begins in adulthood, references advise looking for an underlying immunosuppressive or metabolic illness. This is less likely to explain a typical young puppy’s first episode, but it matters if signs persist, recur later or fail to respond as expected. Your veterinarian can decide whether blood tests or other evaluation are justified from the age, history and examination.
Heat cycles and intact females
CAPC recommends spaying intact females with generalized demodicosis because relapse may occur during subsequent heat cycles. This is preventive veterinary guidance, not proof that every recurrence is hormone-related. Discuss timing with the treating veterinarian, especially if the puppy is still undergoing active treatment.
A practical recheck plan for a returning case
- Call the treating clinic. Describe where lesions are appearing, how quickly they changed and whether the puppy has swelling, crusts, footpad involvement or other signs of generalized disease.
- Pause self-directed changes. Do not add, stop, double or combine antiparasitic, antibiotic or steroid products unless the veterinarian instructs you to do so.
- Attend the examination and sampling. The veterinarian may take deep scrapings from active areas and assess for bacterial infection or other disease.
- Clarify the endpoint. Ask how often scrapings will be repeated, what counts as a negative result and how long treatment continues after mites are no longer seen.
- Record every administration. Use a calendar for doses, baths or other prescribed care, and bring the record to each follow-up.
- Return for the scheduled controls. Stopping when the coat looks normal can leave undetected mites behind and make interpretation of a later relapse harder.
Questions to ask at the appointment
- Is this still localized disease, or does the distribution now meet a generalized pattern?
- Were mites seen on a deep scraping, and from which areas?
- Is there evidence of secondary bacterial infection, and what treatment is appropriate?
- Which medicines are being used extra-label in this country, and what monitoring is required?
- What exact scraping schedule will determine that treatment can end?
- Does this age, severity or response justify testing for an immune, metabolic or other underlying condition?
- If the puppy is an intact female, should spaying be planned after discussing disease control and timing?
Common mistakes that prolong a recurring case
Judging cure by appearance alone
Coat regrowth and less redness are useful observations but cannot replace serial scrapings in generalized disease.

Stopping as soon as mites disappear once
Merck’s endpoint requires two consecutive negative scrapings one month apart, while CAPC describes an additional one-to-two-month treatment period after mites are no longer found. A single negative result is not a universal stop signal.

Using an unapproved product without a plan
Extra-label use is a veterinarian’s decision. Product choice depends on the puppy’s age, weight, breed background, health and other medicines.
Ignoring infection
Bacterial complications can drive swelling, crusts and discomfort. Mite treatment alone may not resolve those lesions.
Assuming every relapse has the same explanation
Persistent mites, reinflammation, bacterial infection, incomplete monitoring and an underlying illness require different responses. The recheck is what separates them.

When to contact the veterinarian again
Contact the clinic whenever treated lesions return, spread to new areas or develop the widespread inflammation, crusting, swelling or footpad changes associated with generalized disease. A return of signs during the first year can occur, but it should still be documented and assessed. Keep follow-up appointments even when the puppy looks normal if the veterinarian has prescribed serial scrapings; those results, not appearance alone, determine whether treatment is complete.
What a realistic recovery timeline looks like
There is no single clearance date that applies to every puppy. Limited localized juvenile disease often resolves without treatment, whereas generalized disease may require several months. The mite life cycle is approximately 20 to 35 days, and monitoring intervals are designed to account for mites that may not be captured in one sample. Your puppy’s duration depends on the extent of disease, infection, treatment response and scraping results.
For United States readers, CAPC’s treatment and label-status information was last updated March 13, 2019; product approvals and veterinary protocols can change. Use current instructions from your veterinarian and the relevant national regulator rather than treating an old label or internet schedule as a prescription.
