Psychogenic alopecia is hair loss caused by excessive grooming when a behavioral or emotional cause is supported. But a bald belly or shortened, “barbered” fur does not prove stress is to blame: itch, parasites, allergy, infection, pain, and other medical problems can look the same. A veterinarian should investigate those causes before diagnosing a behavioral condition.
What psychogenic alopecia looks like
Cats with self-induced hair loss may lick, chew, or pull fur from the medial forelegs, lower abdomen, groin, tail, or lower back. The affected areas can be symmetric, and the remaining hairs may look clipped or broken. Owners may not witness the grooming itself.

Cats groom routinely. Cornell’s feline health center attributes to animal behaviorist Pamela Perry, D.V.M., an estimate that cats spend between 30 and 50 percent of their day grooming. That is a general estimate, not a threshold for diagnosing a problem. A pattern of hair loss cannot reveal on its own whether the trigger is behavioral, itchy, or painful.

Excessive grooming can be a response to discomfort. Possible medical explanations include fleas and other parasites, flea allergy, food or environmental allergy, bacterial or fungal infection, inflammation, and pain. Some systemic or endocrine disorders may also contribute to hair loss. The Society of Feline Medicine’s 2023 proceedings describe behavioral overgrooming as uncommon and likely overdiagnosed, and emphasize ruling out pruritic skin disease first.

A 2020 clinical review cites a group of 21 cats referred for presumed psychogenic alopecia: 16 had medical abnormalities, and three had both medical and psychogenic causes. This illustrates why the label should follow an evaluation, rather than be inferred from a cat’s home environment or the appearance of its coat.
How veterinarians diagnose the cause
Evaluation usually begins with a detailed history and physical examination. The veterinarian may ask when grooming began, where it occurs, whether it varies over time, what changes have occurred at home, and whether the cat has other skin or health signs. The next tests depend on the findings; not every cat needs every test.

- Parasite checks: flea combing and skin scrapings may help look for fleas or other ectoparasites.
- Skin testing: smears or cultures may be used to investigate infection.
- Other medical testing: blood or urine tests may be considered when the history or examination points to a systemic problem.
- Further investigation: a veterinarian may recommend a skin biopsy in some cases.
As VCA Animal Hospitals puts it, “The diagnosis of psychogenic alopecia as a compulsive disorder is reserved for those cases in which no underlying medical problem is evident.” That principle matters because treating a presumed behavior problem alone could leave an itchy or painful condition unaddressed.
What treatment may involve
Treatment depends on what the evaluation finds. Identified medical problems should be addressed first. If a behavioral or compulsive component is supported, a veterinarian may recommend a plan combining environmental management and behavior modification. Medication may be considered when appropriate, but decisions about it require veterinary oversight.

Evidence about which behavioral treatment works best is limited. In a retrospective 1999 case series, environmental stress was reported to initiate or worsen signs in 9 of 11 cats; alopecia resolved in 6 of 11 after medication, environmental modification, or both. Because this was a small case series rather than a controlled comparison, it does not show which intervention caused improvement or predict an individual cat’s response.

Enrichment can support a veterinarian-guided behavior plan, but it does not diagnose or treat an underlying medical cause by itself. Options described by veterinary sources include:

- Keeping daily routines predictable and introducing household changes gradually.
- Providing cat-friendly vertical space and places where the cat can feel safe.
- Scheduling interactive play and social interaction that suit the individual cat.
- Offering stimulating object play or food-filled and treat-filled toys.
Watch for patterns in when and where grooming occurs and share them with the veterinarian. Avoid assuming that a stressful event explains the hair loss, and do not delay an examination while trying enrichment alone.
When to contact a veterinarian
Arrange a veterinary assessment if your cat is losing fur, repeatedly licking or chewing one area, or developing skin changes. The sooner the cause is investigated, the sooner treatment can target the problem actually affecting the cat. A smooth bald patch, a symmetrical pattern, or grooming that seems to follow a household change is not enough to distinguish psychogenic alopecia from medical discomfort.
