A cat with the tip of its tongue poking out is most often relaxed, distracted, or sleeping, and a brief tongue-out moment is usually nothing to worry about. The picture changes when the habit is new, stays out for long periods, or comes with drooling, trouble eating, signs of pain, blood around the mouth, or effortful breathing. Those signs call for a veterinary check rather than a guess based on the expression alone.
What “blepping” means
Cat owners often call the behavior “blepping,” a term for the tip of the tongue left poking out of the mouth. Cats Protection, the UK cat welfare charity, describes it as something cats usually do unconsciously. In its article “Why do cats blep?” (first published 11 August 2015), its Behaviour Manager Nicky Trevorrow explains: “Many cats seem to poke out the tip of their tongue particularly if they get distracted during grooming. Other cats will rest or sleep with the tip of their tongue out.”
The usual reasons a cat bleps
Most tongue-out moments fall into one of three everyday contexts. None of them requires treatment.
After grooming or a distraction
Grooming is a long, repetitive task, and a cat that is interrupted partway through may leave its tongue out as it turns to something else. The tongue simply has not been tucked back in yet. A cat that bleps while washing and then carries on normally is showing ordinary behavior.

While resting or sleeping
Relaxed muscles do not always keep the tongue inside the mouth. Many cats sleep with the tip showing, and the same cat may do it on different days or in different positions. As long as the cat wakes, eats, and behaves as usual, this is typically the sort of blep that owners see without reason for concern.
While investigating a smell
Purina’s guide to the behavior (its US article “Is Your Cat Sticking Its Tongue Out? Cat Blepping Explained,” listed as updated 15 July 2026) describes cats using the tongue together with the scent organ in the roof of the mouth when they explore an interesting smell. This is one possible explanation for a tongue-out expression while a cat sniffs something new, such as a food packet or a plant. It does not explain every tongue-out pose, and it is not a reason to assume anything is wrong.

When a tongue-out habit may point to a problem
The tongue is held in place partly by the teeth and the normal shape of the mouth. If that structure changes, the tongue can stay out more often than before. Cats Protection notes that missing teeth can make it harder for some cats to keep the tongue inside the mouth, so a cat that has lost teeth may bleb for reasons unrelated to mood or grooming.
Oral pain is another reason to look more closely. A cat with a painful mouth may change how it eats, drool, or hold its head, and the tongue may be visible as part of that wider change. Neither Cats Protection nor Purina can tell you from the tongue alone which problem is present; that requires an oral examination by a veterinarian.

Continuous protrusion
A tongue that is out most of the time, rather than briefly, is different from an occasional blep. Persistent protrusion is the reason to book a check, particularly when the change is recent.
Drooling and difficulty eating
Drooling, dropping food, chewing on one side, or going to the bowl and walking away are signs that the mouth may be causing discomfort. Pair these with a tongue that stays out and the case for a veterinary visit is strong.

Signs that need a veterinary visit
Contact a veterinarian promptly if any of the following applies:
- The tongue-out behavior is new for this particular cat.
- The tongue stays out continuously rather than briefly.
- The cat drools, has difficulty eating, or drops food.
- There are signs of mouth pain, such as pawing at the face or reluctance to be touched around the head.
- There is blood on the lips, gums, or paws, or in the saliva.
- The cat breathes with its mouth open, appears to be working hard to breathe, or seems distressed.
Open-mouth or laboured breathing should be treated as urgent. Do not frame it as a normal blep and wait to see whether it passes.

What to do at home before the appointment
- Note when it happens. Record the situations in which the tongue is out (after grooming, asleep, at the food bowl) and how long it stays out.
- Record a short video. A clip showing the tongue, the cat’s mouth, and any drooling gives the veterinarian more to go on than a description.
- Check eating and drinking. Note whether the cat is eating its usual amount, dropping food, or favouring one side of the mouth.
- Look only if the cat is calm. A gentle look at the lips and gums is reasonable. Do not pry open a painful mouth, and do not try to force a closer examination at home.
- Book a veterinary appointment and bring the notes and video, especially if any warning sign above is present.
What not to try as a fix
Neither source supports giving a dental product, toothbrush, toy, grooming item, or supplement as a remedy for unexplained tongue protrusion. A product cannot tell whether the cause is a missing tooth, a sore mouth, or a normal sleeping habit, and treating the wrong problem can delay the right diagnosis.
Why the change matters more than the tongue
Cats are good at hiding illness, and Cats Protection’s Behaviour Manager makes the point directly: “Cats are extremely subtle in their behaviour when they are not well so it can be very hard to tell if something is wrong.” This is the reason to judge a tongue-out habit against the cat’s own baseline. Ask whether the cat is eating, grooming, moving, and sleeping as it normally does. A brief, familiar blep in an otherwise well cat is rarely a concern. A new, persistent tongue-out, or one that appears alongside any of the signs above, is worth a veterinary opinion.
Neither Cats Protection nor Purina publishes figures for how often cats blep or how often the habit reflects disease, so there is no reliable percentage to weigh against your own cat’s pattern. Cats.com’s article “Why Do Cats Stick Their Tongue Out?” (veterinary author Dr. Nat Scroggie MRCVS, listed as updated 11 August 2026) takes the same cautious line on persistent protrusion, blood, pain, and breathing difficulty.
