A cat may begin avoiding the litter box after painful elimination or a frightening event near it. The box can become linked with discomfort or fear, and that learned association may persist even after the original problem improves. Because house-soiling can also signal illness, pain, mobility trouble, stress, or conflict between cats, arrange a veterinary assessment before treating it as a behavior problem.
How a negative association with the litter box can develop
Cats can connect the place where an unpleasant event happened with the discomfort or fear they felt at the time. If urinating or defecating hurts, or a startling event occurs near the box, a cat may later avoid that location. The association is one possible explanation—not a diagnosis—and can continue after the original illness or event has improved.

Pain or urgency during elimination
Urinary problems can make urination painful or urgent; digestive problems can make defecation painful or urgent. A cat may associate the box with that experience. Pain in the joints or nervous system may also make stepping into a conventional box or holding a posture uncomfortable. Cornell’s house-soiling guidance discusses medical and behavioral causes, including pain-related avoidance: Cornell Feline Health Center.

A sudden noise or another upsetting event near the box may also contribute to avoidance. The American Veterinary Society of Animal Behavior describes pain and startling noises as possible triggers; the ASPCA also notes that upsetting experiences can play a role. Neither possibility can be confirmed from the location of an accident alone. See the AVSAB discussion of feline elimination and the ASPCA’s litter-box guidance.

Why avoidance does not identify the cause
Avoiding the box can have medical, environmental, or social causes, and more than one may be present. The cat might dislike the litter’s scent or texture, the box’s design, its cleanliness, or its location. It may prefer another surface or spot, have difficulty reaching or entering the box, or feel stressed or blocked from access by another cat. The pattern alone cannot establish a urinary infection, cystitis, constipation, arthritis, or a behavioral disorder.
The Merck Veterinary Manual and the AAFP and ISFM house-soiling guidelines cover medical, environmental, and behavioral factors. Observing where and when the cat eliminates can help your veterinarian assess the situation, but it should not replace an examination.

What to do if your cat stops using the box
Arrange a veterinary assessment first
New or persistent house-soiling warrants veterinary attention. A clinician can examine your cat and decide whether tests or treatment are appropriate. The American Animal Hospital Association’s guidance on cats not using the litter box also recommends considering medical causes. Do not start medication or infer a diagnosis from the behavior; consult a veterinarian before giving medication for a behavior problem.

Review the box and its surroundings
Once medical factors are being assessed, look for setup issues that could make the box unpleasant, uncomfortable, or hard to reach. Consider the cat’s own observed preferences rather than assuming one option suits every cat.

- Litter: Note its scent, texture, and depth. Cornell advises non-scented litter, but an unscented option is a trial, not a guaranteed fix.
- Box design and access: Check whether your cat can enter, turn, and posture comfortably. An open or low-entry box may help some cats, particularly when mobility or access is a concern; choose based on the individual cat.
- Cleanliness and location: Consider whether the box is kept clean and whether its location is accessible and comfortable for your cat.
- Household dynamics: In homes with several cats, consider whether another cat or an obstruction makes access stressful or unreliable.
The sources do not establish one universally preferred box or litter. A change in setup should address a plausible preference or access problem, not substitute for veterinary assessment.
Clean soiled areas and address ongoing stress
Clean soiled locations as part of managing the problem; an enzymatic pet-urine cleaner is one possible cleanup tool. Cleaning alone is not established as a way to prevent recurrence. If avoidance persists after medical causes and practical setup issues have been addressed, ask your veterinarian about a behavior plan or referral to a veterinary behaviorist. The ASPCA advises consulting a veterinarian or veterinary behaviorist before using medication for behavior problems.

A learned association can outlast the pain or frightening event that helped create it. If your veterinarian has addressed medical concerns and the box is accessible and acceptable to your cat, continued avoidance may need a tailored behavior plan. The plan should account for the cat’s preferences, the home environment, and any stress or conflict—not assume that changing litter or buying a new box will solve every case.
